OBJECTIVES
Following Nursing Process in assessing, implementation and evaluating daily individual
patient care.





Identifies immediate needs of the patients through proper assessment upon admission.
Obtains and documents objectives and subjective cues such as vital signs taking,
performing simple physical examinations, interactions with patients and significant
others.
Collaborates with other health team members in the patients care. E.g. referral to
doctors, medical technologists, radio technicians.
Administers medications by observing the seven rights( right patient, right drug, right
dose, right route, right time, right documentation and right approach), and carries out
treatment and procedures promptly and accurately such as I.V. insertion, nebulization,
catheterization, NGT insertion. Administering drugs – oral, sublingual, intravenous,
intradermal, subcutaneous, intramuscular, intra – tracheal, administering drugs and
fluids via infusion pump.
Document and reviews all carried out orders.

















Instructs and explains to patients and their relatives’ preparation for specific procedures
and treatments such as (NPO) Nothing per Orem, low fat supper, etc.
Endorses admitted patients to the floor nurses with accuracy and comprehensiveness of
continuity of care.
Secures consents and assists doctors in minor surgical and diagnostic procedures
including emergency deliveries.
Makes supplies like operating sponge (OS), cotton balls, etc. for use.
Make daily reports in admitted patients and submits it to night duty nurse or supervisor.
Maintain cleanliness and orderliness of the unit in coordination with the housekeepers.
Sees to it that all equipments, supplies and other items in the unit are in good condition
and ready for use at all times.
Provides for and supervises the administration of medication and treatment ordered by
the medical staff to all patients who come for consultation and admission.
Sees that the physician’s orders are promptly carried out and properly implemented.
Utilizes all the opportunities for student nurses and other affiliates through orientations
and lectures.
Motivates personnel to actively participate in all the activities of the nursing service
department.
Carries out administrative procedures in conformity with hospital policies and
procedures such as unpaid bills, waivers and other problems that may arise during
shifts.
Respond immediately to the emergency codes by proceeding at once to the area with
emergency kit.
Does cardiopulmonary resuscitation in the collaboration with the doctors.
Prepare other gadgets like cardiac monitor, defibrillator, during codes.
Maintaining patent airway – performing tracheal, oral and nasal suctioning as required.
Adequate physiotherapy to mobilize the secretions. Two hourly changes of positions,
giving steam- inhalation, mobilization. Administering oxygen inhalation.
Care to the patients on ventilator.
 Maintaining patent airway
 Set the ventilator parameters according to the patient’s respiratory status and
constant monitoring of the patients.

Emergency patient care.
 Resuscitating during cardiac and pulmonary arrest.
 Assisting and performing CPR.
 Assisting in endo-tracheal intubation.


Monitoring fluid – electrolyte balance, intravenous canalization, administering
intravenous fluids, monitoring and maintaining adequate record of intake and output,
sending samples for investigation and evaluating the lab values.
Monitoring blood glucose level/values – Bedside blood glucose monitored with the use
of glucometer.









Hourly maintaining a bypass chart of chest tube drainage, urine output, IV fluids, heart
rate, respiratory rate, peripheral temperature, oral and rectal temperature.
Care of dangerous drugs and their records.
Doing admission and discharge of patients.
Transferring or accompanying of patient to their hospital of choice.
Monitoring Glasgow coma scale – the level of consciousness is assessed by Glasgow
Coma Scale, papillary response to light also monitored changes are daily recorded and
reported.
Meeting nutritional and elimination needs
 Inserting NGT
 Giving nasogastric food
 Administering of enema and suppositories
 Urinary catheterization
 Bladder training by periodic clamping and releasing
 Giving urinary antiseptics and bladder wash for catheterized patients
Assessing physical, psychological and psychosocial needs of the client. Collection of
personal, social, medical and general data by interviewing formally and informally
during various client interactions, observations, review of records, diagnostic tests, etc.
Preparing the patient for various investigations, such as CT scan, ultrasounds, x-ray.
Sample collection for various investigations like; sputum examination, blood and urine
culture, etc.
