YARM MEDICAL PRACTICE Job Title: Nurse practitioner Qualifications: Registered (First level) nurse. Additional post Registration UKCC/NMC qualification/status. Current registrations on UKCC/NMC register. Nurse Practitioner degree or equivalent. Minimum of three years experience in primary Care. Relevant prescribing qualification Grade: Yarm Medical Practice salary band 8a Hours: Up to 37.5 per week Accountable to: General Practitioners (Clinically) Practice Manager (Administratively) Role Summary: Responsible for attending to patients who wish to be seen by a health care professional; diagnosing and treating patients presenting with minor illness, seeing and advising patients in respect of their continuing medical and nursing needs. Responsible for providing advice, support and liaison with other practice staff. Specific and general duties/tasks are described overleaf. This list is not comprehensive but intended as a guide to the work expected of the post holder. The usual place of work will be Yarm Medical Centre. In addition, the post holder will be required to attend ‘Time-in’ and ‘Time-out’ sessions as part of their personal development. All working conditions are supplied in the practice induction package. The post holder will be expected to adhere to the practices Health and Safety regulations. The post holder must be aware of the practices requirement for confidentiality at all times. The practice operates a yearly appraisal system to review staff training and development needs. The practice will provide training to enable the postholder to carry out their duties. YARM MEDICAL PRACTICE Clinical Nursing Practice Assess, plan provide and evaluate nursing care to meet the care needs of individuals and groups in the practice population Assess diagnose and treat specific diseases in accordance with agreed medical/nursing protocols Undertake diagnostic health screening, health surveillance and therapeutic interventions Provide direct access to specialist nursing care for undifferentiated patients within the practice population Undertake diagnostic, health screening, health surveillance and therapeutic interventions within a broader health promotion / public health strategy. Triage Nursing Opportunistic screening in all areas of patient care. Health Promotion, to include advising on healthy eating, smoking cessation, exercise and alcohol consumption. Vaccinations and Immunisations Ear Care Infection Control Emergency Management i.e. anaphylaxis and resuscitation Child Health / Child Protection Mental Health Travel Health Advice Care and Programme Management: Men’s Health Family Planning Women’s Health Diabetes Respiratory Disease Management Learning disabilities management Cardiovascular Disease Management Initiate and carry out programmes of health screening Support and enable individuals and carers to take action to meet their priorities for health Manage programmes of care for patients with chronic disease by planning and evaluating care Develop profile of practice population in order to initiate, contribute and promote improved health and prevent disease in individuals and groups Initiate and carry out programmes of health screening Manage programmes of care for patients with acute and chronic diseases by planning providing and evaluating care. YARM MEDICAL PRACTICE Clinical Practice Leadership Support practice development plans Assist in clinical audit and the setting and monitoring of standards of care Teach and direct others in clinical settings, including unqualified staff Recognise the development needs of others and take appropriate action Advise and support other members of the nursing team Act as a mentor for other staff Support practice development plans Develop and guide members of the practice nursing team in their education programmes as a mentor Clinical Practice Development Demonstrate clinical expertise in practice Ensure clinical supervision is in place for staff Implement strategies for quality assurance and quality audit Develop and implement evidence based practice Develop the management of skilled holistic care for patients Initiate and lead practice developments to enhance the nursing contribution and quality of care Identify apply and disseminate research findings relating to specialist practice Develop and lead evidence based practice Liaison As well as the nursing team, there is a need to work closely with reception and office staff to ensure the smooth running of the practice, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the nursing team. There is also the need to establish and maintain good liaison wit other surgeries and agencies including secondary care. It will be necessary to attend and contribute to various practice meetings are requested.
