Role Summary:
Responsible for attending to patients who wish to be seen by a health care
professional; diagnosing and treating patients presenting with minor illness,
seeing and advising patients in respect of their continuing medical and
nursing needs. Responsible for providing advice, support and liaison with
other practice staff.
Specific and general duties/tasks are described overleaf. This list is not
comprehensive but intended as a guide to the work expected of the post
holder. The usual place of work will be Yarm Medical Centre.
In addition, the post holder will be required to attend ‘Time-in’ and ‘Time-out’
sessions as part of their personal development.
All working conditions are supplied in the practice induction package. The
post holder will be expected to adhere to the practices Health and Safety
regulations. The post holder must be aware of the practices requirement
for confidentiality at all times.
The practice operates a yearly appraisal system to review staff training and
development needs. The practice will provide training to enable the postholder to carry out their duties.

Clinical Nursing Practice















Assess, plan provide and evaluate nursing care to meet the care needs
of individuals and groups in the practice population
Assess diagnose and treat specific diseases in accordance with agreed
medical/nursing protocols
Undertake diagnostic health screening, health surveillance and
therapeutic interventions
Provide direct access to specialist nursing care for undifferentiated
patients within the practice population
Undertake diagnostic, health screening, health surveillance and
therapeutic interventions within a broader health promotion / public
health strategy.
Triage Nursing
Opportunistic screening in all areas of patient care.
Health Promotion, to include advising on healthy eating, smoking
cessation, exercise and alcohol consumption.

Ear Care
Infection Control
Emergency Management i.e. anaphylaxis and resuscitation
Child Health / Child Protection
Mental Health

Initiate and carry out programmes of health screening

Support practice development plans
Assist in clinical audit and the setting and monitoring of standards of
care
Teach and direct others in clinical settings, including unqualified staff

Liaison



As well as the nursing team, there is a need to work closely with
reception and office staff to ensure the smooth running of the practice,
reporting any problems encountered to the relevant person and
ensuring everyone is aware of the different roles within the nursing
team.
There is also the need to establish and maintain good liaison wit other
surgeries and agencies including secondary care.
It will be necessary to attend and contribute to various practice
meetings are requested.

privacy and act appropriately. All such information from any source is to be
regarded as strictly confidential. Information relating to patients, carers,
colleagues, other healthcare workers or the business of the practice may only
be divulged to authorised persons in accordance with the practice policies and
procedures relating to confidentiality and the protection of personal and
sensitive data.
