Brief resume of the intended work 6.1 Need for the study: The Xerosis of conjunctiva is a symptomatic condition in which conjunctiva becomes dry, lustureless and non wettable. These patches almost always involve the inter-palpabral area of the temporal quadrants and often nasal quadrants as well. Typical xerosis may be associated with conjunctival thickening, wrinkling and pigmentation1.It occurs in two groups of cases: (i) as a sequel of a local ocular affection and (ii) associated with general disease. The first type is cicatricial degeneration of the conjunctival epithelium and glands following trachoma, burns, diphtheria, etc. commencing in isolated spots, ultimately involving the whole conjunctiva and cornea. Prolonged exposure due to ectropion or proptosis, where in the eye is not properly covered by lids, also in a dry eye2. Based on a deficient serum retinol, there are more than 125 million preschool age children with Vitamin A deficiency among whom close to 4 million have an ocular manifestation of some kind, termed broadly as Xerophthalmia3. Management as per modern medical science consists in relieving the dryness with artificial tears and mucomimetic agents. In the vitamin A deficiency variety, nutrition should be restored by administration of vitamin A2. In children being treated for Vitamin A deficiency according to the protocols, transient bulging of fontanels occurs in 2% of infants and transient nausea, vomiting and head ache occur in 5% of preschoolers. Acute toxicity of Vitamin A can result in increased intracranial pressure, vertigo, diplopia, bulging fontanels in children, seizures and exfoliative dermatitis, it may result in death3. Xerosis is correlated with Shukthika as per Ayurvedic reference. In this disease, a small elevated blackish spot resembling shell appears in shukla mandala4. Due to pitta, blackish, grayish and yellowish spots appear in almost whole shukla mandala along with burning sensation and pain in eyes. This is accompanied by jwara, atisara and trishna5. There are various non invasive as well as safe treatment modalities told in Ayurvedic classics for the treatment of Shukthika. These can be done on O.P.D. level and as well as by the patient himself and are cheap and convenient. Gambhari, Amalaki & Haritaki aschyotana and Katphala aschyotana are the two medicines mentioned in Susruta Samhitha in the management of Shukthika4. Hence an attempt will be made with such a set of simple and effective measures picked up from Ayurvedic treatise i.e. topical instillation of Gambhari, Amalaki & Haritaki eye drops and Katphala eye drops in Shukthika and an assessment will be made as regard to the efficacy of both and the same will be compared in the present study. 6.2 Review of Literature: a) Description of Shukthika Nidana and Lakshanas in Ayurveda Classics. b) Description of Shukthika Chikitsa in Ayurveda Classics. c) Description of Xerosis in Modern Science. d) Description of both the Gambhari, Amalaki & Haritaki Aschyotana and Katphala Aschyotana in Ayurvedic Literature. Previous Work Done: Dr. P.B.Subin – “A clinico-comparitive study on effect of internal medication of Triphala Ghrita and Thilvaka Ghrita in the management of Shukthika w.s.r. to Bitot’s Spots”-. 2010-11 Shri J G C H Society’s Ayurvedic Medical College and Research centre Ghataprabha. Belgaum District 6.3 Objectives of study: To evaluate the efficacy of Gambhari, Amalaki & Haritaki Aschyotana in the management of Shukthika. To evaluate the efficacy of Kataphala Aschyotana in the management of Shukthika. To compare the efficacies of both the groups in the management of Shukthika. 7.0 Materials and Methods Source of data: The patients attending the OPD and IPD of Shri J.G.C.H.S.Ayurvedic Hospital, Ghataprabha will be taken for the study. a) Literary source: The details of the literature of Shukthika and Xerosis will be incorporated in great detail from the Samhitas and other Ayurvedic and Modern reference books. b) Clinical Source: 1. Patients attending the OPD and IPD of Shri J.G.C.H.S.Ayurvedic Hospital, Ghataprabha will be taken up for the present clinical research. 2. The patients referred from other practitioners, hospitals, institutions will also be incorporated. c) Drug Source: Both the Gambhari, Amalaki & Haritaki Aschyotana and Katphala Aschyotana will be prepared as per the guidelines given in Ayurvedic literatures in Shri J.G.C.H.S. Ayurveda Medical College Pharmacy. 7.2 Methods of collection of data: It is a single blind clinical study with minimum of 20 patients in each group will be selected randomly. The clinical features will be recorded on the proforma designed for the study. A) Inclusive criteria: 1. The patients irrespective of age, sex, religion will be included for the study. 2. Patients with clinical features of Shukthika will be taken for study after proper screening . B) Exclusive criteria: 1. Patients who are on supplementation with Vitamin A. 2. Patients having any inflammatory disease of the eye. 3. Patients suffering from chronic debilitating systemic diseases. STUDY DESIGN (INTERVENTIONS): The screened patients of Shukthika will be randomly classified into two groups. Group A and Group B. Group A – Gambhari, Amalaki and Haritaki Aschyotana will be done. Dose – 7 or 8 drops once in a day6. Duration – 30 days. Group B Katphala Aschyotana will be done. Dose – 7 or 8 drops once in a day6. Duration – 30 days. FOLLOW UP: The patients of both the group will be followed up every 30 days for 2 months. ASSESSMENT CRITERIA: Assessment will be done on the basis of objective and subjective criteria before and after the treatment schedule and data will be subjected to student’s paired and unpaired ‘t’ test. SUBJECTIVE PARAMETERS:1. Dryness 2. Foreign body sensation. 3. Burning sensation. OBJECTIVE PARAMETERS:1. Photographs. 2. Schirmer's test 3. Rose Bengal test INVESTIGATIONS REQUIRED: 1.
