2. HEALTH INFORMATION
2.1 General requirements
I, the undersigned official inspector, hereby certify that the ornamental aquatic
animals referred to in Part I of this certificate:
2.1.1 Have been inspected within seven days before loading and found to be healthy
and free from any clinical signs of disease.
2.1.2 Have not been kept in water in common with koi carp1 or farmed foodfish (fish
farmed for human consumption including recreational fishing).
2.2 Requirements for species susceptible2,3 to or can act as a vector to Spring
viraemia of carp (SVC)4, Koi herpesvirus disease (KHV)4, Epizootic ulcerative
syndrome (EUS)4, Epizootic haematopoetic necrosis (EHN)4 and White spot
disease (WSD)4
I, the undersigned official inspector, hereby certify that the ornamental aquatic
animals referred to above:
2.2.1
Originate from a country or a zone or an area or an aquaculture establishment that
has been subject to an official fish health surveillance system equivalent to the
relevant OIE standard and is officially recognized (by the competent authority of
the country) as being free from [SVC]4, [KHV]4, [EUS]4, [EHN]4 and [WSD]4.
2.3 4Requirements for species susceptible to or can act as a vector to Furunculosis
(Aeromonas salmonicida) and Goldfish haematopoetic necrosis virus (GFHNV)
I, the undersigned official inspector, hereby certify that the ornamental aquatic
animals referred to above:
2.3.1 Are absent from clinical signs suggestive of infection.

causes or by experimental exposures to the disease agent that mimics the natural pathways for infection as
defined by OIE.

Susceptible species are in accordance to the latest edition of the OIE Aquatic Animal health code
(http://www.oie.int/eng/normes/fcode/en_sommaire.htm) and the Manual of Diagnostic Tests for Aquatic
Animals (http://www.oie.int/eng/normes/fmanual/A_summry.htm).

2.4 Additional health

I, the undersigned official inspector, hereby certify that the marine ornamental finfish
referred to in Part I of this certificate:
2.4.1 Are not sourced from a population associated with any significant infectious
disease or pests and there have not been any unusual or substantial outbreaks of
infectious fish disease or pests in the areas from which the fish have been
collected during the 6 months prior to collection.
2.4.2 Were collected from an area at least 5 kilometres from any foodfish aquaculture
operation and the fish in the consignment have not come into contact with water,
equipment or fish associated with farmed foodfish (fish farmed for human
consumption including recreational fishing).
2.4.3 Are wild caught and have not been bred or hatched on a farm or other premises.
3. LABELLING REQUIREMENTS
3.1 Labelling of consignments
I, the undersigned official inspector, hereby certify that the consignment referred to in
Part I of this certificate:
3.1.1 Is identified by a legible label on the exterior of every packing bag or container
with the following information:
i. Name of establishment of origin or harvest
ii. Name of importer
