KATHARINE HOUSE HOSPICE JOB DESCRIPTION Staff Nurse Level 4 Day Therapies (Generic) Post holder: Area of Work: Day Therapies Responsible to: Advanced Nurse Practitioner Accountable to: Director of Care Services Mission “To offer the best care so that people in our community affected by progressive illnesses can live their lives to the full” Department Objectives The purpose of the Day Therapies is to be an integral part of the specialist palliative care provision by Katharine House Hospice to adult patients with an advanced progressive illness, their carers and friends within the palliative care philosophy framework identified by the World Health Organisation (2003). Overall Aims of the Post The Staff Nurse will have responsibility for the assessment of patient care needs, and the planning, implementation and evaluation of plans of care to meet their identified needs. They will be expected to carry out nursing care without direct supervision whilst acknowledging knowledge and skills limitations. Care that reflects and embodies the fundamental principles and philosophy of palliative care should be practised. The Staff Nurse will have a responsibility to develop and maintain their professional knowledge related to nursing and palliative care, through participation in the Individual Performance Review process, reading and attendance at training events. The Staff Nurse will have a responsibility to assist the Advanced Nurse Practitioner in the development of a day therapies that demonstrates tolerance, respect for individual needs and effective multi-disciplinary team work. The post holder will provide care and support to patients attending for day therapies and the Well Being Day service. Staff will be based within the Day Therapies but may due to demands on services be required to support other areas within Care Services. Key Tasks and Responsibilities In order to meet the objectives the post holder will have responsibilities in four main areas of work as follows: 1. 2. 3. 4. Clinical Managerial Education and Research Personnel Within these areas specific responsibility will be as follows: 1. Clinical An in-depth knowledge of the nursing process, promoting and supporting the use of the agreed model of individualised holistic care An understanding of the philosophy of palliative care Plan appropriate palliative nursing care for a group of patients with an appropriate level of support Delivery of high quality, evidence based, individualised nursing care to patients Provide an appropriate level of support to patients’ informal carers Demonstrate effective communication skills with patients and their family and friends. In addition effective communication skills with the multi-professional team and other hospice staff and volunteers Understand and apply a multi-professional approach to care Work co-operatively and in partnership with members of the multidisciplinary team and voluntary staff with the objective of providing the best quality of care for the patient and their family/friends throughout their stay at the hospice and at the time of discharge Ensure that prescribed treatments and medications are given in accordance with hospice policies and procedures and Nursing and Midwifery Council regulations and guidance Act as the patient’s advocate Manage a group of patients with an appropriate level of support Liaise with other health and social care professionals involved in the patient’s care Staff Nurse Level 4 Day Therapies 2015. The Act requires the post holder to attend infection prevention and control training on induction and at regular updates and to take responsibility for the practical application of the training in the course of their work. Infection prevention and control must be included in any personal development plan or appraisal. Smoking The hospice has a no smoking policy.
