JOB DESCRIPTION POSITION: Specialist Speech & Language Therapist For Adult Traumatic Brain Injury with Challenging Behaviours REPORTS TO: Service Manager LOCATION: Fen House JOB PURPOSE The role is to act as an integral member of an interdisciplinary team within Fen House, part of the Brain Injury Rehabilitation Trust. Responsibility will be to act as lead specialist to individual clients who have sustained either an acquired and/or traumatic brain injury. To assess, diagnose and generate individualised, client-centred management plans that address the clients cognitive, behavioural, speech, language, communication, voice and swallowing difficulties as a result of their injury. To participate in a neurobehavioural, social model approach to client care and to function independently as part of their role. RESPONSIBILITY FOR CLIENT CARE 1. 2. 3. 4. To work independently in a variety of settings using a multidisciplinary and interdisciplinary client centred approach. To independently assess, differentially diagnose and develop and implement specialist care programmes for clients with complex communication and/or swallowing needs following stroke and/or acquired/traumatic brain injury. To provide clinical advice to others regarding the management and care of clients with speech/language/communication/voice and/or feeding and swallowing difficulties. To demonstrate clinical effectiveness by use of evidence based practice and outcome measures. To be responsible for the Speech and Language Therapy components of integrated programmes provided by the interdisciplinary team including the generation implementation and management of goals. To generate and implement specialised communication and behavioural guidelines following the neurobehavioural model for most of the clients. These are regularly discussed and monitored with the multidisciplinary team and Rehabilitation Support Workers and reviewed on a regular basis according to each individual client. To safely deal with verbal and physical aggression and complete all behavioural recordings associated with each individual client. To manage clients with challenging, socially inappropriate and/or self injurious behaviours which can be unpredictable and aggressive and closely liaise with the multidisciplinary team in formulating appropriate management strategies. To liaise with and act in partnership with other professionals and as part of the multidisciplinary team to ensure a well co-ordinated plan. To ensure effective and sensitive communication of assessment results to clients, carers, families, the multidisciplinary team and interagency professionals within clinical team meetings, funding reviews and individual sessions. To liaise with and work closely with clients, carers and their families to establish appropriate client centred, functional and context-specific goals and management plan based on their assessment results. To demonstrate empathy with clients and demonstrate effective communication with clients who have difficulties understanding and talking as a result of their injury. To provide frequent emotional support to distressed clients and their families in close liaison with the clinical psychologist. To work with and closely liaise with the clinical psychologist in the implementation of plans that are highly sensitive and distressing to clients. To write specialised reports that detail not only assessment results but also a short and long term management plan for the client. To use specialist knowledge to inform clinical judgement and independently manage a complex and highly specialised caseload. To ensure that clients provide consent to all interventions. To respect client confidentiality at all times. To make sensitive decisions as to whether it is appropriate to set further targets or end intervention. To recognise when clients need to be referred onto other professionals and agencies as appropriate. To provide regular functional and context-specific therapy to clients within the clinical setting and within the community in order to facilitate awareness of insight of difficulties and to assist generalisation of skills learnt. To demonstrate excellent negotiation skills in managing conflict within the organisation and provide support as appropriate. To co-ordinate and participate in the clinical training and supervision of Speech and Language Therapy students including final year assessment placements. To be responsible for the training and support of Speech and Language Therapy Assistants and/or Rehabilitation Support Workers who will be conducting and implementing Speech Therapy plans with the clients. To work closely with the multi-disciplinary team in delivering services to clients with cognitive-communication difficulties and to provide information related to leisure and/or vocational pursuits for individual clients. To act as clinical keyworker and clinical lead for individual clients including liaising with families, carers, PCT, Social Services and other funding agencies as well as co-ordinating and monitoring the clients overall progress in achieving their behavioural and other discipline specific goals and chairing funding reviews for the client. To liaise closely with the Occupational Therapist to jointly assist in setting up vocational placements for clients. To jointly assess and review those placements where appropriate. To be involved in the development and continuation of regular brain injury education evenings to families and carers. To be responsible for regular training and support to the Rehabilitation Support Workers about Speech and Language Therapy and other topics relating to individual clients and their goals. To support staff and provide an environment in which to model and demonstrate appropriate behaviours by regular meal eating with the clients throughout the week. To ensure manual handling regulations and restraining policies and procedures are adhered to and safe positioning of safety and others is ensured. To work within infection control and health and safety guidelines in order to maintain safety to the professional and client. To advise the Service Manager on issues of Fen House Speech & Language therapy service provision and service delivery including pressures and changes to current work practice. To evaluate own service delivery and provide progress reports and updates to the Service Manager and other Speech and Language Therapists within the local network. To resolve informal complaints and formulate responses to more formal complaints where necessary. Provide regular support to staff at lunch times through working with the clients during this time. In collaboration with members of the multidisciplinary team, develop quality standards and audit measures to monitor the current Speech and Language Therapy service to clients. Be involved in regularly assessing differing Speech and Language Therapy service delivery models being utilised based on current research. To collaborate in audit activities and analysis of outcome measures that measure client’s progress. To maintain accurate records of all client’s consultations and related work carried out at each clinical session, including computerised data collection. To keep up to date and apply relevant evidenced-based research findings into current practice. To contribute to multidisciplinary and interagency service and policy developments relating to client care and assume lead where appropriate. To facilitate and participate in peer supervision at a local level with Speech and Language Therapists and at the national level with Speech and Language Therapists within the Brain Injury Rehabilitation Trust. To demonstrate a working knowledge of BIRT policies and procedures including working with vulnerable adults and other legal frameworks. To implement RCSLT Clinical Guidelines and Communicating Quality-3 in practice and other evidence based research. To be responsible for a Speech and Language Therapy budget for delivering therapeutic services to clients within the community. To maintain the equipment inventory within the area of Speech and Language Therapy and inform the Service Manager of any replacement needs. To identify future equipment and resource requirements and inform Service Manager of these. To assist in the recruitment of new staff and ensure appropriate induction is provided with Fen House and regional units where appropriate. To provide mentoring, advice and support for external SLT’s when required. To coordinate support of students from other professions as appropriate. To be independent in seeking out and participating in training and courses relating to Speech and Language Therapy or related work within the BIRT organisation and/or externally to BIRT. To initiate, design and implement research, clinical audits and/or projects that will improve current Speech and Language Therapy practice. To attend mandatory training.
