Position Summary:
The Nurse Practitioner (NP) or Physician Assistant (PA-C) will provide safe, effective, and quality
primary, preventive, and evidence based health care to patients in a culturally competent manner,
focusing on health promotion and disease prevention.
The NP or PA-C must demonstrate regard for the rights of all patients and provide respectful and
considerate care focused on the patient's individual needs. The NP or PA-C will provide a safe and
confidential environment for patients, families and personnel.
Standards of Performance:
 Promote the philosophy and mission of Summit Community Care Clinic (SCCC);
 Demonstrate accountability and responsibility for duties and tasks associated with the
position;
 Acquire, maintain and demonstrate appropriate knowledge level and competencies;
 Demonstrate a commitment to service excellence and quality improvement;
 Act as a representative of the clinic with patients, staff and the general public in a
professional and supportive manner.
 Comply with SCCC’s written standards, including its Standards of Conduct and policies and
procedures; compliance is a requirement of the position, and failure to comply will be met by
the enforcement of disciplinary action, up to and including possible termination, in
accordance with SCCC’s Policy and Procedure: Addressing Instances of Non Compliance.

Essential Functions:
 Interviews patients to obtain accurate, thorough health histories.
 Performs physical examinations and provides preventive health services.
 Orders, interprets, and evaluates diagnostic tests to identify and assess patients’ clinical
problems and health care needs.
 Establishes diagnostic impression.
 Formulates comprehensive health care plan based on findings.
 Prescribes or recommends medications or other forms of treatment.
 Refers and consults with other professionals as needed.
 Educates patients and family members about health care issues.
 Ensures appropriate patient follow up and reevaluation occur.
 Actively participates as a member of the integrated care team, working conjunctively with
School Based Health Centers, Care Team, dental and behavioral health departments to
ensure seamless, multidisciplinary patient care using a Team Based model.
Additional Tasks/Responsibilities:
These may change and additional duties may be assigned.
 Thoroughly and accurately document each patient visit and contact in Aprima, the electronic
health record.
 Work constructively with clinic staff to ensure smooth operation of the medical program.
 Performs other related job duties as assigned by the CMO.
Qualifications: To perform this job successfully, an individual must be able to engage in each
essential duty satisfactorily. The requirements listed below are representative of the knowledge,
skills, and/or abilities required. Reasonable accommodations may be made to enable individuals

Education/Experience: Certified Nurse Practitioner or Physician Assistant education
program. At least two years’ experience in a clinical setting and DEA preferred. Previous
non-profit and/or volunteer experience preferred. Experience in Federally Qualified Health
Center preferred.
Language Ability: Ability to read, write and clearly speak the English language. Ability to
interpret a variety of instructions and deal with multiple lingusistic cultural variables.
Ability to read, write and clearly speak the Spanish language preferred.
Reasoning and Mental Ability: Ability to define problems, collect data, establish facts, and
draw valid conclusions. Ability to exercise independent judgment. Self-directing and
organized. Ability to reason objectively. Ability to assess, project and plan for patients’
needs. Ability to interpret state/ federal/agency regulations. Ability to document concisely,
accurately and in timely manner. Ability to handle a variety of duties which may be
interrupted or changed by immediate circumstances.
Interpersonal Skills: Ability to relate cooperatively and constructively with patients, coworkers, administration, physicians and providers, community agencies, referral sources,
regulators and other health team members. Ability to enlist the cooperation of others. High
tolerance for stress.
Computer Skills: To perform this job successfully, an individual should have a solid
knowledge of word processing software, spreadsheet software and database software.
Certificates and Licenses: For Nurse Practitioner - Certification as a Nurse Practitioner
from American Nurses Credentialing Center or American Academy of Nurse Practitioners
and Licensure as a Nurse Practitioner with prescriptive authority from the State of Colorado.
For Physician Assistant - Graduation from an NCCPA-approved physician assistant program,
Passage of the NCCPA National Board Exam, and active Colorado PA-C license. For both DEA license, Current CPR card.
Supervisory Responsibilities: None.
Work Environment:
The work environment is in a health care setting and can be fast paced and involve working directly
with patients and health care professionals. Many of our patients with chronic disease have a
minimal level of health literacy and a significant number of barriers to accessing health care. The
individual may also work in a clerical setting. Reasonable accommodations may be made to enable
individuals with disabilities to perform the essential functions of the job. The noise level in the
work environment is usually moderate.
