JOB DESCRIPTION
JOB TITLE
Licensed Practical Nurse
ESSENTIAL RESPONSIBILITIES
Perform assigned duties in the Family Health Clinic (FHC), defined as:
1. Supports the nurse practitioner in coordinating care at the clinics.
2. Performs adult and pediatric intake, including health and developmental history,
screening tests, collecting specimens and vital signs.
3. Completes paper work and electronic data entry ensuring accuracy in documentation of
immunizations, patient encounters and procedure forms.

6. Collection of lab specimens, such as venipuncture, urine or stool, observing universal
precautions

Completion of an accredited Licensed Practical Nurse Program
Licensure as required by the state of Indiana
Current CPR Certification
Medication Certified, i.e. competency in administration of vaccines and other
medications, such as IM injections, oral medications, and inhaled medications
RESPONSIBILITIES / JOB FUNCTIONS
1. Supports the Mission and Vision of the Family Health Clinic.
2. Complies with all HIPAA rules.
3. Prepares exam rooms for clients/patients. Cleans rooms, stocks supplies: ensures that
exam rooms are patient ready. Checks appointment schedule and assists receptionist
with the Nurse Practitioner’s (NPs) schedule as needed. Washes instruments and cleans
equipment according to OSHA guidelines. Hands instruments and materials to NPs or
assists in procedures as necessary. Checks inventory and other medical supplies and
materials.
4. Prepares clients and admission paper work for nurse practitioners and nurse
coordinator. Greets clients/patients. Prepares clients for examination by measuring
height, weight, calculating BMI (body mass index), temperature, pulse, respirations,
blood pressure, vision testing, hearing testing, and urine specimens as indicated. Takes
history of present illness in a concise manner and documents according to clinic
protocols. Reviews client record to ensure that lab reports, immunization records, X rays, etc are present and available for the NP to review.
5. Performs general client care within scope of practice, as evidenced by formal
assessment at time of orientation and proven ability to perform the task. Performs
and/or assists with diagnostic procedures, specimen collection, and treatments as
directed by the NP and demonstrates competencies in said areas.
6. Schedules clients for testing procedures with clinics, hospitals, and other allied
agencies as directed by the NP. Schedules referrals to other health care providers as
ordered.
7. Effectively manages information and client flow in office. Receives incoming phone
calls and interoffice communications, takes messages from clients, families, hospitals,
etc. and relays to the proper personnel. Identifies those messages that need immediate
attention /interventions to the RN or NP; identifies those messages that are not urgent in
nature and communicates these to the appropriate staff at times which do not interfere
with client flow.
8. Assists the clinic secretary with client intake, phone calls, scheduling of appointments,
and medical record management (pulling client records, filing client records, copying
records as needed). Participates in daily huddles.
9. Scans clinical documents into chart.
10. Responds to patient requests via the patient portal, as directed by the provider or RN
Coordinator. Checks emails and desktop daily.
11. Copy and mail disability inquiries, chart requests, FAX records as needed (ex. CDLs),
ensuring that all appropriate record releases have been signed by the client befor , e
releasing records. This includes reviewing the clinic policies and HIPPA regulations re:
patient’s privacy rights.
12. Assists in quality assurance by completing various types of paper work to include:
 Recording and tracking patient test results. Assisting the RN in relaying test
results to clients (either by phone call or letter).
 Filing reports (ex. animal bites, reportable communicable diseases)
 Entering data into patient tracking programs (ex ISDH, CHIRP), and data
needed for grants (ex. pregnancy testing, demographics)
 Assisting in the immunization program by ensuring that all temperature log
sheets are kept up to date and refrigerator / freezer temperatures are recorded
at least once a day.
13. Sets professional goals with the RN and/or NP at least annually and attains them.
Maintains expertise by pursuing the required continuing education relevant to position.
Participates in optional professional activities: special projects, programs, activities,
health fairs, fundraising, etc.
14. Maintains all mandatory in-services as required by Purdue University.
15. Performs other related duties, which may be inclusive, but not listed in the job
description.
SUPERVISION RECEIVED
Works under the direction of the nurse practitioner and the registered nurse.
PHYSICAL DEMANDS
In performing the duties of this job, the employee is regularly required to perform intermittent
physical activity including walking, standing, lifting, and supporting patients. The employee is
occasionally required to reach with hands and arms, stoop, and kneel. The employee must be
able to communicate fluently in writing and verbally in English. The employee must
occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job
include close vision and ability to adjust focus.
