I am writing as you have missed two consecutive antenatal/postnatal appointments. I hope
everything is progressing well, it is important that you attend for antenatal/postnatal care
so we can check your wellbeing and health. Therefore an appointment has been made for

If you are having difficulties with timing of appointment or venue, which is preventing you
from attending please contact your midwife via the community office on 01482 382658,
leaving an up to date telephone contact number.
Please ensure you make every attempt to attend, as receiving antenatal care is important to
ensure best care for mother and baby. Good communication is vital between partner
agencies to ensure everyone involved is aware of the failure to attend so the appropriate
follow up can be made. Failing to attend will involve midwifery services sharing information
with other agencies including Children’s Social care (as per hospital guidance).
