OFFICE POLICY

If you are 15 minutes late for an appointment you may need to reschedule for
the next available time. Wellness visits will need to be scheduled for the next
30 minute visit appointment opening.

Please contact our office a minimum of 24 hours in advance to change or
cancel an appointment. Failure to comply with this could result in a “No
Show” notice sent to you. (See policy below.)

Walk-in appointments are accepted for acute patients only based on
appointment availability and severity of the patient. They will be placed in
the next available appointment slot; however, a phone-in appointment should
be attempted when possible.

Blood pressure checks need not schedule appointments; however, if you want
prescription refills or have questions for the practitioner you must make a
scheduled appointment.

Allergy injections must be monitored 20 minutes after administration. They
can be given Monday-Friday 8:30 am – 11 am and 1 pm – 4 pm.

Telephone request for medication refills must be called in 48 hours before
refills are needed. Monitoring your medication quantity is your
responsibility.

Medications, other than heart and blood pressure prescriptions, will not be
refilled early in the event of loss or theft.
NO SHOW POLICY
There is to be a 24-hour notice of appointment cancellations. This includes any
referral appointments to other facilities that this office has scheduled for you.
Failure to comply with this policy will result in a NO SHOW notice being mailed to
your current address or a fee for administrative costs. Habitual failure to show for
appointments may result in a withdrawal of medical services letter. You will have
30 days from that time to find a new physician. This office will provide necessary
medications (diabetic, hypertension, thyroid, cardiac, etc.) during this 30-day period.
