Genome-wide pharmacogenetic investigation of a hepatic adverse event
without clinical signs of immunopathology suggests an underlying
immune pathogenesis
Supplementary note 1.
The highest-ranking SNP (rs2858869) in Phase 1 of EXGEN was not included in the set of
250,000 tag SNPs selected for the GWS. Hence, if DRB1 had not been included as a
candidate gene, the highest-ranking SNP in the HLA region from the tag SNP set would have
been rs707937 (p=1.95 x 10-4), which is 14kb from VARS2 and 841kb from rs2858869.
According to the strategy followed in this study, this SNP did not meet the threshold for
follow-up (p10-5) and hence was not selected for further study.
If, however, we had followed only a GWS strategy using these tag SNPs, this and
other SNPs in the MHC class II region would have been among the top-ranked 650
associations, and hence would likely have been carried forward for subsequent fine mapping.
The increased density of SNPs across the MHC region would then have uncovered the
primary association with the DRB1 and DQA1 genes.
Similarly, if the GWS and candidate gene study results had been combined and
analysed together, the rs2858869 SNP would still have been among the highest-ranked and
would have been carried forward for further fine mapping studies. It is likely, however, that
the FDR and other analyses may not have been as clear-cut, with the signal-to-noise ratio
being lower in the combined SNP set.
We conclude that the details of our analysis strategy were not vital in discovering the
primary association with the DRB1 and DQA1 genes, and that alternative strategies would
likely have resulted in the same discovery. We do recommend, however, that a dense map
of markers be used for initial screening (particularly in gene-dense areas like the HLA region)
and that fine mapping with an increased density of markers be carried out in the same
sample set, before selection of the highest-ranked markers for replication testing in an
independent sample set.

Subjects (cases and matched controls) were preselected from previous long-term treatment studies
with ximelagatran: ESTEEM, SPORTIF III, THRIVE III and THRIVE Treatment. The subjects were
selected from clinical databases by AstraZeneca. The study was carried out in centres in Sweden.
Subjects who had not previously taken part in EXGEN (10 cases from groups 1 & 2 and 16 controls
from group 3) were used for replication testing.

Supplementary Table 3. 690 candidate genes selected on the basis of (i) the mechanism
of ximelagatran, (ii) reported association with elevated liver enzymes and (iii) additional
preclinical studies. SNPs were selected for each gene (defined by NCBI build 35), and
10kb flanking sequence. A sliding window approach was used to pick SNPs at a spacing
of approximately 6 SNPs every 10kb (a total of 26,613 SNPs).

Genotype case-control test p-values of matched vs. unmatched analysis
for the top 500 SNPs (selection based on p_min) in the candidate gene
study.
In the comparison of the matched and unmatched analyses, 464 of the top 500 ranked SNPs
from both analyses were included in both lists, showing strong agreement between the
matched and unmatched analysis. Supplementary Figure 1 shows the log p-value (minimum
of the two genotype case-control tests including/excluding intermediates) of the matched and
unmatched analyses for the top 500 SNPs, selected on the basis of the minimum p-value of
all 6 single locus analyses. The majority of SNPs show good concordance between matched
and unmatched analyses.
