JOB DESCRIPTION POSITION TITLE: Communicable Disease Program Manager REPORTS TO: Medical Operations Manager DEPARTMENT: Operations EMPLOYMENT STATUS: Salary, Exempt - Professional LOCATION: Bellingham JOB PURPOSE: Coordinate all aspects of the Communicable Disease program including community, patient, and employee health. Responsible for setting and ensuring achievement of administrative and programmatic goals. Supervises the nutritionist and RN who work under the Communicable Disease Program, seeking input from the Nursing Supervisor and Medical Director on their clinical duties, as appropriate. Develop program budgets, monitor program revenue and expenses, and supervise the work of contracted service providers. Lead quality improvement initiatives and program development for service enhancement and delivery, and respond appropriately to Public Health initiatives and/or concerns locally and nationally. Represent ICHC at local, regional, and statewide planning, prevention and care activities around communicable disease response and strategy. Helps identify and apply for program funding opportunities. ICHC manages patient care using a team-based approach in our interactions with patients and working to achieve stated objectives and outcomes. JOB QUALIFICATIONS Education: Requires BA or BS degree or equivalent. Public health course work preferred. Experience: Requires three years experience in social services or public health and one year experience in program management, preferably in HIV/AIDS. Experience supervising employees preferred. Equivalent education, training, and experience may be substituted. Specialized Skills, Knowledge, and Abilities: Requires basic knowledge of the social and clinical aspects of living with HIV/AIDS and Hepatitis C. Ability to manage programs required. Experience with grant research, writing, and reporting is preferred. Must have ability to carry out supervisory responsibilities in accordance with the organization's policies and applicable laws. Must possess excellent computer skills, be proficient in Microsoft Office programs (Word, Excel, Outlook, and PowerPoint), and be able to use our practice management and EMR systems. Must demonstrate excellent communication skills, both verbally and in writing. Must have the ability to analyze and report clinical outcomes and participate in continuous quality improvement (CQI) initiatives. Must have the ability to understand and respond effectively and with sensitivity to special population groups, including those defined by race, ethnicity, language, age, gender, sexual orientation, economic standing, & others. Must have ability to travel to local meetings as well as occasional out-of-town conferences. Maintains working knowledge of medical issues and advancements related to the treatment of communicable diseases and related programs (i.e. federal Ryan White Program, HIV, Hep C, Tb, STD’s, employee health, etc), patient and population-based medical management, and social service resources and benefit programs. 2. Responsible for developing, revising, and improving communicable disease programs including setting and ensuring achievement of administrative and programmatic goals. Responsible for coordination with care teams to ensure clinical quality measures are achieved around communicable disease program initiatives. 3. Leads analysis of clinical quality measurement for purposes of quality improvement and program innovation and development. Page 3 of 7 4. Monitors state and local health departments and CDC advisories on communicable disease outbreaks. Leads ICHC’s assessment and response. 5. Supervises staff in accordance with ICHC’s policies and procedures and applicable laws. This includes hiring, orienting, training, delegating, coaching, mentoring, addressing complaints, resolving problems, disciplining, terminating, and performing timely performance evaluations. This also includes scheduling staff and their vacations appropriately. Seeks input from Nursing Supervisor and Medical Director on employees’ clinical duties as needed. 6. Monitors program revenue and expenditures and communicates changes in program budget to senior management. 7. Represents ICHC in local, regional and statewide planning, prevention and care activities, including coordinating prevention efforts with the Whatcom County Health Department, the Washington State Department of Health, the Center for Disease Control, Evergreen Wellness Advocates/Lifelong AIDS Alliance, Planned Parenthood, University of Washington AIDS Education and Training Center, and St. Joseph PeaceHealth Medical Center. 8. Assists in identification of program funding opportunities and grant writing, including narrative and budget components. 9. Develops and maintains strong partnerships with key communicable disease providers including ICHC providers, Evergreen Wellness Advocates, Whatcom County Health Department, infectious disease specialists, and specialty providers in the community. 10. Develops and maintains relationships with billing staff at specialty referrals offices. 11. Works with ICHC management staff to assure access to HIV, Hep C and other communicable disease care services for clients throughout Skagit and Whatcom Counties. 12. Attends the Ryan White/Hep C multi-disciplinary team meeting on the second Tuesday of each month and provides administrative support and assistance to team members where applicable. 13. Manages administrative program duties, including ensuring patient access, as needed, to financial assistance programs and ICHC’s sliding fee program, maximizing patient insurance coverage, and coordinating with ICHC pharmacy staff to ensure coverage for needed medications, 14.
