Re: Expanding university employee health benefits to include comprehensive infertility treatment coverage Below is a list of colleges and universities in the United States that include infertility coverage in their employee health insurance as of January 1, 2017 Brown University, Rhode Island (BCBS): This plan provides coverage for medically necessary services for the diagnosis and treatment of infertility for women. This plan covers donor gametes if provided through a program. This plan only covers these services if you are: unable to conceive or sustain a pregnancy during a one (1) year period and a presumably healthy individual. Copayment is 20% Clemson University, South Carolina (BCBS): Lifetime maximum payment of $15,000. Included in the $15,000 maximum are diagnostic tests, prescription drugs and up to six cycles of Intrauterine Insemination (IUI), and a maximum of three completed cycles of zygote or gamete intrafallopian transfer (ZIFT or GIFT) or in vitro fertilization (IVF) per lifetime. Prescription drugs for treatment of infertility are subject to a 30 percent coinsurance payment through both the Savings Plan and the Standard Plan Columbia University, New York (United Healthcare): Unlimited benefit for diagnosis and basic medical treatment including artificial insemination; $30,000 lifetime maximum for advance treatments for assisted reproductive technology including IVF, GIFT and ZIFT Cornell University, New York (Aetna): Artificially assisted fertilization (varies depending on plan level) 90% ($20,000 lifetime max per family); or 80% after deductible ($20,000 lifetime max per family); or 90% after deductible ($20,000 lifetime max per family) Duke University, North Carolina (Coventry and BCBS): Employees and their covered spouse/partner are eligible for infertility benefits after two years of continuous service; treatment options include discounted IVF (patient pays $4000), IVF with ICSI ($4500), and Frozen Embryo Transfer ($800); employees pay 50% of the cost of infertility medications purchased at in-network pharmacies Johns Hopkins, Maryland (CareFirst BCBS, EHP, Kaiser Permanente, BlueChoice): Fertility testing covered 80% after deductible; artificial insemination: 80% covered after deductible; limited to 6 approved attempts per live birth. In addition, coverage is limited to you and your spouse or same-sex domestic partner. All infertility treatments are subject to a separate annual deductible. There is a separate lifetime maximum for infertility services of $15,000 and plan will cover eligible services at 50% after a $1000 deductible is met. Any prescription medications included in this treatment are applied to the lifetime maximum. Expenses for infertility treatment do not apply toward the annual out-of-pocket maximum. All infertility treatments are subject to OSU Health Plan guidelines as outlined in the Schedule of Benefits section for your medical plan.
