This innovative pharmacist-directed wellness management program is designed to help individuals with chronic health conditions such as high cholesterol, hypertension (high blood pressure), chronic pain, depression, diabetes and asthma. The MTM program offers: Employees will: •Gain a better understanding of the medications they take •Receive continual guidance with their prescribed treatment •Achieve optimal health •Save on copayments •Incorporate other behaviors to enhance benefits of therapy Voluntary and Confidential P articipation in the program is completely voluntary and confidential. No personal information about you or your medical information is provided to any non-medical entity, including your employer, co-workers or human resources. All services, offered at the Center for Pharmacy Care and related to this program, are free of charge as a part of the Duquesne University benefits program to improve the overall health and quality of life of participants. How Does the Medication Therapy Management Program Work? I t’s simple. You meet regularly with one of Duquesne’s clinical pharmacists at the Center for Pharmacy Care. Our pharmacist will work in partnership with your doctor, who will continue to direct your care, to ensure your optimal health through the best use of medications and suggested lifestyle modifications. • Free health care visits to Center for Pharmacy Care • An initial health assessment • Review of medication regimen • Medication action plan to prevent and resolve adverse drug reactions and drug interactions, and to reduce the use of unnecessary medication • Goal setting • Communication with other health care providers • Lifestyle modification • Waiver of copayments for your covered prescription for high cholesterol, hypertension (high blood pressure), chronic pain, depression, diabetes and asthma. Employee must be member of University Cigna Open Access Plus or UPMC EPO medical plan. You must keep all of your scheduled appointments with the pharmacist to remain eligible for the program.
