Patient Information Leaflet
About the medication you have
been given to take home
•
Please read the typed labels on all of the boxes and
bottles. This will tell you how to take your medication.
•
The boxes and bottles of medication will contain
information leaflets that will explain what the
medications are for and about any possible side
effects. Please read these leaflets or get someone to
read these to you.
•
You will usually have four weeks supply of medication
unless the medication is to be taken only when
necessary or as a short course or if we know you
already have a supply of this medication.
•
When you are running short of your medication you
must get another prescription from your G.P. You
should do this several days before you run out.
•
If you have any queries about your medication after
you leave hospital you should contact the ward, the
hospital pharmacy department, your G.P. or your
community pharmacist. All of these will be able to
help you with your query.
