year we will be celebrating our 40th consecutive year of providing the highest quality continuing

All the courses are full day programs, 9:00 A.M. to 4:30 P.M. (registration and continental
breakfast at 8:00 A.M.). This year we are again looking forward to valuable and informative
programs in many disciplines of dentistry. The September 16th program will feature the very
popular speaker, Dr. Corky Willhite, who will present an innovative and timely program on
contemporary transitional bonding. On October 23rd, we are very happy to present Dr. Pascal
Magne, who will discuss the state of the art in bonded porcelain restorations in the anterior
dentition. As always, there will be strong scientific programs, with the acclaimed Dr. Tim
Donley, with a new program on oral medicine and the management of periodontal disease. Your
staff will be invited to this timely program. On November 18th Dr. Joe Massad will bring us up
to date on overdentures. In January, Dr. Gary Alex will present the latest methodology in
placing direct posterior composites. On March 31st, we are again proud to present a great friend
of our program, the legendary Dr. Gordon Christensen, who can speak on any subject known to
dentistry. In April the great Gerard Chiche will be back to discuss smile design and occlusal
techniques update .The May 4th program will feature the always informative Dr. Roger Levin,
who will present the famous “Set Your Practice on Fire” lecture, and to round off this great
program, in February, we will have the motivating Dr. Jeff Okeson speak on TMD and orofacial
pain.
As you know, over the last 40 years, our mission is to provide the highest quality continuing

Transitional Bonding: Non-traditional Composite
Restorations for Major Occlusal and Esthetic Changes

Corky Willhite, DDS has achieved Fellowship by the Academy of General
Dentistry, the American College of Dentists and the American Academy of
Cosmetic Dentistry. He is one of only 60 Accredited Fellows in the AACD of over
6,000 members worldwide. He has served on their Board of Governors (currently
the American Board of Cosmetic Dentistry) and spent years as an Examiner for
Accreditation. He is an Adjunct Assistant Clinical Professor in the Department of
Comprehensive Dentistry and Biomaterials at L.S.U. School of Dentistry. He is

Bonded Porcelain Restorations in the Anterior
Dentition

The combination of both composites and ceramics seems theoretically appropriate
to reproduce the original stiffness of the tooth and modulate the tooth-restoration
strength. Therefore, modern concepts in restorative dentistry have brought new
solutions through bonded porcelain restorations (BPRs) that are stress distributors
and involve the crown of the tooth as a whole in supporting occlusal force and
masticatory function. Among these, the good overall clinical behavior of anterior
porcelain laminate veneers bonded tooth in terms of fracture rates, microleakage,
debonding and soft tissue response is generally well recognized and attested by
numerous clinical studies. Continuous developments in the field of adhesive
restorative techniques have permitted significant broadening of the originallydefined spectrum of indications for BPRs in the anterior dentition and thus
contribute to two of the major objectives of conservative restorative dentistry: the
maximum preservation of sound tooth structure and the maintenance of the vitality
of the teeth to be restored. Indications for BPRs are extending to more perilous
situations (worn-down, nonvital or crown-fractured teeth), resulting in
considerable improvements, comprising both the medical-biological aspect and the
socio-economical context (i.e., decrease of costs when compared to traditional and
more invasive prosthetic treatments). From this perspective, indications, treatment
planning, diagnostics, tooth preparation, laboratory procedures and adhesive luting
procedures of BPRs will be carefully detailed.

Pascal Magne DMD, PHD
Dr. Pascal Magne is an Associate Professor with Tenure and the Don and Sybil
Harrington Foundation Chair of Esthetic Dentistry in the Division of Restorative
Sciences, University of Southern California, Herman Ostrow School of Dentistry,
Los Angeles, CA.
He graduated from the University of Geneva Dental School, Switzerland, in 1989
with a Med. Dent. Obtained his Doctorate in 1992 and his Ph.D. degree in 2002.
Dr. Magne received postgraduate training in fixed prosthodontics and occlusion,
operative dentistry and endodontics, and was a lecturer at the same university
beginning in 1989 until 1997.
From 1997-1999, he was a Visiting Associate Professor at the Minnesota Dental
Research Center for Biomaterials and Biomechanics, University of Minnesota,
School of Dentistry. After concluding two years of research, Dr. Magne returned to
University of Geneva Dental School and assumed the position of Senior Lecturer
in the Division of Fixed Prosthodontics and Occlusion until he was recruited to the
University of Southern California in February 2004.
Dr. Magne is a recipient of multiple awards from the Swiss Science Foundation,
the Swiss Foundation for Medical-Biological Grants, and was the recipient of the
2002 Young Investigator Award from the International Association for Dental
Research as well as the 2007 and 2009 Judson C. Hickey Scientific Writing Award
(for the best research article of the year published in the Journal of Prosthetic
Dentistry). He is also the author of numerous clinical and research articles on
esthetics and adhesive dentistry and is an internationally known mentor and
lecturer on these topics. Furthermore, Dr. Magne authored the book Bonded
Porcelain Restorations in the Anterior Dentition - A Biomimetic Approach which
has been translated into twelve languages and is considered as one of the most
outstanding books in the field of adhesive and esthetic dentistry.
He is a founding member of the Academy of Biomimetic Dentistry and a mentor of
the Bio-Emulation think-tank group. In 2012, he launched a revolutionary
approach to the teaching of Dental Morphology, Function and Esthetics (the
2D/3D/4D approach) for freshman students at the Herman Ostrow School of
Dentistry of USC.

Dr. Massad is in private practice in Tulsa, Oklahoma, a Fellow of both
the American and the International College of Dentists, a Regent/Fellow
of the International Academy for Dental Facial Esthetics, and is an
honorary member of the American College of Prosthodontics. Currently,
he holds faculty positions at Tufts University School of Dental
Medicine in Boston, Mass; the department of Comprehensive Dentistry
at the University of Texas Health Science Center Dental School in San
Antonio, Texas; and the department of Prosthodontics at the University
of Tennessee Health Science Center, College of Dentistry, Memphis,
Tenn. He has previously held a faculty position at the Oklahoma State
University College of Osteopathic Medicine Oklahoma, is a past
Director of Removable Prosthodontics at the Scottsdale Center for
Dentistry in Arizona (2006 to 2010), and from 1992 to 2003 was an
associate faculty at the Pankey Institute in Florida.

DR. TIMOTHY DONLEY is currently in the private practice of
Periodontics and Implantology in Bowling Green, KY. After graduating
from the University of Notre Dame, Georgetown University School of
Dentistry and completing a general practice residency, he practiced general
dentistry. He then returned to Indiana University where he received his
Master’s Degree in Periodontics.
Dr. Donley is the former editor of the Journal of the Kentucky Dental
Association and is an adjunct professor of Periodontics at Western Kentucky
University. He is a lecturer with the ADA Seminar Series. Dentistry Today

About the Course:
Temporomandibular disorders (TMD) are a group of musculoskeletal disorders of
the masticatory system. These disorders are common and dentists are the primary
care providers. Therefore every dentist should have a sound understanding of TMD
so that the most appropriate care will be selected for the patient. Often simple care
can make big differences in the patient’s quality of life. In the past, dentists have
attempted to manage their TMD patients utilizing standard dental approaches,
which have not always proven to be successful. In fact our profession has many
concepts and theories that attempt to solve musculoskeletal problems with dental
therapies, many of which have little to no scientific supported. This presentation
will describe the present evidence that either support or disprove these concepts,
including how occlusion may or may not affect TMD. Other areas that will be
discussed are etiologic considerations and treatment strategies, including the use of
occlusal appliances. This presentation will attempt separate the facts from the
fantasies in the area of TMD.

Dr. Okeson is a 1972 graduate of the University of Kentucky College of Dentistry. After
graduation he completed two years with the Public Health Service in a rotating dental
internship and directing an outpatient clinic. He joined the faculty at the University of
Kentucky in 1974. Presently he is Professor, Chair of the Department of Oral Health
Science and Director of the College's Orofacial Pain Center, which he established in 1977.
The Center represents a multidisciplinary effort in the management of chronic orofacial
pain problems. Dr. Okeson has developed several post-graduate training programs in the
Center, including a Master of Science Degree in Orofacial Pain. He is currently working
with the Commission on Dental Accreditation establishing curriculum guidelines for
graduate training program in orofacial pain. Dr. Okeson has more than 220 publications in
the area of occlusion, TM disorders and orofacial pain in various national and
international journals. The seventh edition of Dr. Okeson's textbook Management of
Temporomandibular Disorders and Occlusion is published by C. V. Mosby (Elsevier)
Publishers. This text is used in most of the United States dental schools as well as in many
other countries. It has been translated into eleven different foreign languages. In addition
to this text, Dr. Okeson has authored the seventh edition of Bell’s Orofacial Pains which is
published by Quintessence Publishing Company. This text is translated into six different
foreign languages and widely used in orofacial pain programs throughout the world.
Dr. Okeson is an active member of many TMD and orofacial pain organizations holding
many offices and serving on numerous committees and boards. He is a past President of the
American Academy of Orofacial Pain and former editor of the Academy’s Guidelines. He
is a founding Diplomate and past President of the American Board of Orofacial Pain. Dr.
Okeson is a very sought after lecturer on the subject of TMD and orofacial pain and has
presented more than 1100 invited lectures on the subject of TMD and orofacial pain every
state and in 54 different countries. He has received the campus wide University of
Kentucky “Great Teacher Award”, the Provost’s Distinguished Service Professorship, the
American Academy of Orofacial Pain’s Service Award and the first ever “Distinguished
Alumni Award” from the College of Dentistry. Dr. Okeson has also received “The
International Dentist of the Year Award” from the Academy of Dentistry International.
This is the highest award recognized by this Academy and was given to him in recognition
of his worldwide efforts in providing education in the area of temporomandibular
disorders and orofacial pain.

Gordon J. Christensen is Founder and Director of Practical Clinical Courses (PCC), Chief
Executive Officer of Clinicians Report Foundation (CR), and a Practicing Prosthodontist in
Provo, Utah.
Gordon and Dr. Rella Christensen are co-founders of the non-profit CLINICIANS REPORT
FOUNDATION (previously named CRA), which Rella directed for many years. Since 1976,
they have conducted research in all areas of dentistry and published the findings to the profession
in the well-known CRA Newsletter now called CLINICIANS REPORT.
Gordon's degrees include: DDS, University of Southern California; MSD, University of
Washington; PhD, University of Denver; and two honorary doctorates.
Early in his career, Gordon helped initiate the University of Kentucky and University of
Colorado dental schools and taught at the University of Washington.
Currently, he is an adjunct professor at the University of Utah, School of Dentistry. Gordon has
presented thousands of hours of continuing education globally, made hundreds of educational
videos used throughout the world, and published widely.
Gordon and Rella's sons are both dentists. William is a prosthodontist, and Michael is a general
dentist. Their daughter, Carlene, is a teacher.

Dr. Gerard J. Chiche is the Thomas P. Hinman Endowed Chair in Restorative
Dentistry, and the Director of the Center for Esthetic & Implant Dentistry at
MCG School of Dentistry in Augusta, GA. He has given numerous programs
nationally and internationally and holds membership in the American College
of Dentists, the American Academy of Esthetic Dentistry, the American
Academy of Fixed Prosthodontics, the American Academy of Restorative
Dentistry and the Omicron Kappa Upsilon Dental Honor Society. He is a Past
President of the American Academy of Esthetic Dentistry and is also,
respectively with Alain Pinault and Hitoshi Aoshima the author of the
textbooks: Esthetics of Anterior Fixed Restorations, and Smile Design - A guide
for Clinician, Ceramist and Patient both published by Quintessence Pub. Co.
He serves as adjunct faculty at the Pankey Institute; he is the recipient of the
2003 LSU Alumni Award, the 2003 Educational Community Achievement
Award of the Seattle Study Club for best dental educator of the year and the
2007 Distinguished Lecturer Award of the Greater New York Academy of
Prosthodontics. He became in 2009 the first recipient of the Endowed Chair
sponsored by the Thomas P. Hinman Dental Society.
