EVOLUÇÃO DE CASO CLÍNICO DE ENTERITE ANTERIOR EM EQUINO Clinical case’s evolution of previous enteritis in equine Bruna S. 1 Teixeira , 1 Magalhães , Layane Q. Carolina dos 1 1 1 Delfiol , João P. E. Z. de Medicina Veterinária – FAMEV, UFU, Uberlândia/MG CASE REPORT The proximal duodenum jejunitis (DJP), a proximal catarrhal inflammation of the small intestine, has unknown etiology. They show depression, intermittent pain with stomach overload, orange-brown color reflux production, foul odor and alkaline pH; tachycardia, hyperthermia and distension of the small intestine to rectal palpation. In clinical pathology there’s leukocytosis, neutrophilia and metabolic acidosis. The diagnosis is based on clinical signs, and laboratory evolution of the picture changes, differing of small bowel obstructions. The objective was to report the case of a mare, Quarter Horse, 10, 450kg, with abdominal discomfort there’s one day. Kept in boxes and power feed and hay (3kg and 15kg, respectively). At the appointment, was observed apathy, congested mucous membranes, moderate dehydration, body temperature 38.6°C, tachycardia, tachypnea, intestinal hypomotility and distension of the small intestine to rectal palpation. In the nasogastric probe, was detected orange-brown reflux, totaling 15L. The peritoneal fluid revealed citrus color and absence of fibrin. Blood count, there’s hypochromic anemia without leukocyte change. The treatment was instituted gentamicin (6,6mg/kg/IV/SID), benzathine penicillin (40.000UI/kg/IM/48h), flunixin meglumine (1.1mg/kg/IV/BID), dimethyl sulfoxide 10% (1g/kg/IV/SID/3days), omeprazole (4mg/kg/VO/SID), calcium gluconate (200mg/kg/IV/SID), and amino acid supplement intestinal flora replenisher (4g/VO/SID) for 10 days. Gastric emptying was conducted at 4 hour intervals. There’s slow and progressive improvement. From the 12th day showed appetite, wheat was offered moistened, switched to hay (700g/12hours) and water ad libitum. After 13 days the animal defecate, receiving medical discharge in 23th day of hospitalization. Early diagnosis is essential for a favorable prognosis, rapid recovery without secondary complications. . * * * * Figura 1: Hemograma inicial. A duodeno jejunite proximal (DJP), uma inflamação catarral proximal do intestino delgado, possui etiologia desconhecida. Os animais apresentam depressão, dor intermitente com sobrecarga estomacal, produção de refluxo de cor castanho-alaranjada, odor fétido e pH alcalino; taquicardia, hipertermia e distensão do intestino delgado à palpação retal. Na patologia clínica há leucocitose, neutrofilia e acidose metabólica. O diagnóstico baseia-se nos sinais clínicos, evolução do quadro e alterações laboratoriais, diferenciando-se de obstruções de intestino delgado. Objetivou-se relatar o caso de uma égua, Quarto de Milha, 10 anos, 450Kg, apresentando desconforto abdominal há um dia. Mantida em baia e alimentação de ração e feno (3Kg e 15Kg, respectivamente). Ao atendimento, foi observado apatia, mucosas congestas, desidratação moderada, temperatura corporal 38,6°C, taquicardia, taquipnéia, hipomotilidade intestinal e distensão do intestino delgado à palpação retal. À sondagem nasogástrica, detectou-se refluxo castanhoalaranjado, totalizando 15L. O líquido peritoneal revelou cor citrina e ausência de fibrina. No hemograma, verificou-se anemia hipocrômica, sem alteração leucocitária. Como tratamento, instituiu-se gentamicina (6,6mg/kg/IV/SID), penicilina benzatina (40.000UI/kg/IM/48h), flunixim meglumine (1,1mg/kg/IV/BID), dimetilsulfóxido 10% (1g/kg/IV/SID/3dias), omeprazol (4mg/kg/VO/SID), gluconato de cálcio (200mg/Kg/IV/SID), suplemento aminoácido e repositor de flora intestinal (4g/VO/ SID), durante 10 dias. O esvaziamento gástrico foi realizado em intervalos de 4 horas. Houve melhora lenta e progressiva. A partir do 12° dia demonstrou apetite, foi oferecido trigo umedecido, alternado ao feno (700g/12 horas) e água ad libitum. No 13° dia o animal estercou, recebendo alta médica no 23O dia de internação. O diagnóstico precoce foi essencial para um prognóstico favorável, recuperação rápida, sem complicações secundárias.
