Approved Treatment Required for Crizotinib for Non-small Cell Lung Cancer
All 3 conditions must be met
1. Application made by and first cycle of systemic anti-cancer therapy to be
prescribed by a consultant specialist specifically trained and accredited in the
use of systemic anti-cancer therapy
2. Histologically or cytologically confirmed diagnosis of stage IIIB or IV NSCLC
that carries an ALK rearrangement
3. 1st line treatment (no previous 1st line systemic therapy for advanced/metastatic
NSCLC)
