What is a Hospital Patient Portal?
• A hospital patient portal is an online tool that allows patients to access and view the records of care from their
hospital encounter. From within the portal, patients can download, save, print and share this information.
How much does it cost?
• Crozer-Keystone is happy to provide access to our Hospital Patient Portal at no charge to you.
What do I get from Crozer-Keystone Health System's Hospital Patient Portal?
• Safe, secure access to your health information from Crozer-Keystone:
– Demographic Information
– Laboratory Results
– Radiology Results
– Medications
– Allergies
– Medical Conditions
– Discharge Instructions
• Health resources such as events, health calculators, and education
• Ability to share your health records with your doctors, caregivers and family
How do I get to the Crozer-Keystone Patient Portal website?
• Go to www.ckhospitalportal.org in any internet browser.
How do I access my account?
• Visit www.ckhospitalportal.org
• Enter the user ID and password that you may have received from hospital staff during your inpatient stay.
• Once you have logged in for the first time you will be required to change your password.
• If you did not receive your account information during your hospital stay, select “Register Here” and
provide the requested personal verification information. After submitting the information, you will receive
a confirmation email verifying the status of your account.
What if I forget my password?
• If you added an email address to your account, visit www.ckhospitalportal.org and click "Forgot your
password?" on the login screen. Enter your email address to have your password sent to you.
• If you did not add an email address to your account, submit a support ticket from the login screen at
www.ckhospitalportal.org. Click on the link, “Having difficulty or a question about the portal? Contact support
by clicking here.” Be sure to include your contact information and someone will follow up with you.
How do I change my password?
• Select “My Setup” from the left menu then click “Password Change.”
• You will be asked to verify your UserID and current password, then
to enter your new password two times.

How do I view my results, visit summaries and demographic information?
• Login to www.ckhospitalportal.org and select “My Health Record,” then
“Health Summary” from the left menu. You can then select from the following:
• Lab/Outpatient Radiology Results: View, trend or graph your laboratory results.
• Reports: View your transcribed reports such as discharge summaries, progress
notes, emergency department reports, etc.
• Health Summary: View or download your health summary documents in a PDF
format. The Health Summary will include all of the following care records and results
provided during your hospital stay: laboratory and radiology results, medications,
allergies, medical conditions and discharge instructions.
• Visits: View recent visits.
• For instructions on how to use each of the screens, such as how to download your
documents, please reference the tutorials that will appear at the top of the screen.
Is my information secure?
• Yes! All information within the Crozer-Keystone Hospital Patient Portal is secure, meeting the highest healthcare
standards. Only you, your healthcare providers, and those you provide your information to will see your information.
The font is too small. How do I control the zoom of the page?
• Find the word Zoom in the top right corner of the page.
• Then click “+” to increase the zoom or click “–” to decrease the zoom
on the page.
I don’t see my information (such as a recent blood test) in the
Crozer-Keystone Hospital Patient Portal. What do I do?
• Log on to www.ckhospitalportal.org and click on “Having difficulty or a question about the portal? Contact
support by clicking here.”
I walked away from my computer, and when I came back, I had to log in again. What happened?
• The Crozer-Keystone Hospital Patient Portal “times out,” after a period of inactivity, to protect your information from
unauthorized access. Log in again to see your information securely.
Can I bookmark the Crozer-Keystone Hospital Patient Portal on my computer?
• Yes, you can bookmark the Crozer-Keystone Hospital Patient Portal using your Internet browser's bookmark function.
How do I save a shortcut to the Crozer-Keystone Hospital Patient Portal on my computer?
• For Windows: Right click on the mouse and a menu pops up. Then click on “create a shortcut.” A window will pop up
that says “a shortcut to the current page” will be placed on your desk top "click on “OK.”
• For Mac: Go to the webpage and right click: choose on “create shortcut.”
