Welcome to Crawcrook Medical Practice
This booklet has been produced to let you know about the services available at the surgery and help you
to make the best use of them. Please read it carefully and keep it somewhere safe and easy to find.
Our Practice is highly patient focused and we have a multidisciplinary approach to patients’ health care.
We aim to deliver the best for our patients by combining the skills of the practice team with those of other
health and social care workers in the community.
We provide modern health care facilities and aim to continually improve the health of our patients.
Practice History
Our Practice traces its origins to the first Doctor Sacco in Crawcrook during the First World War. His son
Frank joined him after the Second World War, and was a founding member of the Practice until his retirement in 1979. The Practice moved into new purpose built premises in Back Chamberlain Street in 1985
and subsequently to our new premises which opened in 2011. In addition we currently have a branch in
Greenside Community Centre.

Accessibility
We endeavour to meet the needs of our patients. Our premises in Crawcrook are fully accessible for
patients with disabilities, our Greenside branch surgery however, is not. As we do not own the site at
Greenside we are limited to what we can do to make the building more accessible.
If you have any queries about accessibility and your needs, or if you are a carer and the person you care
for requires additional help to access our services, please do not hesitate to contact us. We are always
happy to consider suggestions for improvement.
Parking
The parking spaces at the front (north) of the building are for use by the whole community when shopping
in the village and are managed by Gateshead Council. There is a maximum two-hour stay in force. The
practice parking spaces are behind and to the west side of the building. To access these, drive past the
entrance and follow the road around and to the left.
PLEASE LEAVE OUR EMERGENCY BAYS FREE FOR EMERGENCY
VEHICLES, DO NOT PARK ON THE PAVEMENTS AND DO NOT
PARK IN DISABLED BAYS WITHOUT A PERMIT.
Appointments
Appointments can be made by calling Crawcrook on 0191 4135473 during opening hours. You can also
register to make appointments online.
Remember, our Greenside premises are not easily accessible to people with mobility problems, so please
discuss any concerns with the receptionist when booking your appointment. We also have an online
booking system; please speak to a receptionist if you would like to register to use it.
You can express your preference of practitioner although you may get an earlier appointment if you do
not specify a particular person. We do ask that you try and stay with the same clinician for treatment
during a particular period of illness. When making an appointment you will be asked for your full name
and date of birth to make sure that we access the correct medical records.
Please let us know as soon as possible if you are unable keep your appointment so that someone else
can use it. Not doing so means longer waiting times and a waste of the NHS’ valuable resources.
Please note that appointment times are set for 10 minutes. It really helps to try not to take up the
clinicians time for longer than this to help clinics run on time. If you have a few things to talk about, ask
the receptionist about booking a double appointment.
Routine Appointments
Telephone lines are extremely busy in the mornings due to our morning emergency surgeries. Please
call the surgery after 10.30am for routine appointments, repeat prescriptions and other non-urgent
queries.
Routine surgery times may vary, so please check the times with our reception staff for late or early
availability. Clinical staff have duties off site, such as home visits, so are not available out of surgery
hours, even though the surgery building may be open.

Emergency Appointments
We run an emergency surgery at Crawcrook each morning from Monday to Friday. These slots are for
acute medical conditions that cannot wait for a routine appointment but that are not urgent enough to
warrant immediate hospital treatment.
You must ring us as early as possible to book an emergency appointment should you feel your problem
is urgent. You will be offered an emergency slot for that day. When no appointments are left you can
now be seen by Gateshead Community Based Care (CBC) at Blaydon Hub, ask the receptionist to book
you an appointment. You may also be offered a telephone appointment. We regret that emergency appointment slots cannot be booked in advance.
Telephone Appointments
Telephone appointments are available every day so if you think you can use a telephone slot instead of
a face-to-face appointment then please ask for one.
Extended Hours
We offer sessions on Tuesday and Friday mornings 7.30am – 8am and on a Tuesday and Wednesday
evening until 7pm. Our telephone lines are open from 8am to 12.30pm and 1.30pm to 6pm Monday to
Friday, however our reception is constantly manned during opening hours.
Out-of-Hours Emergencies
If you think you are in, or are witnessing a life threatening situation, please dial 999. If you think you
need to see a Doctor, but your problem does not warrant a 999 call, call 111 where trained staff will be
able to advise you and inform you where to go for treatment at any time, day or night.
Details of local Walk in Centre's and Gateshead CBC Hub are also included at the end of this booklet.
Repeat Prescriptions & Medication
We have a strict policy of asking patients to allow 48 hours / 2 working days (not a weekend) from ordering to collecting their repeat prescriptions. We aim to prescribe safely, effectively, and appropriately.
Our policy facilitates this by allowing enough time to review and process the considerable number of
prescriptions that are requested on a daily basis. We would like to emphasise that this policy is both
patient - and safety-centred.
Repeat prescriptions are issued from our Practice computer system and the approved medication details
will appear on an attached printed sheet. The left hand (green) portion bearing prescription details
should be handed to the chemist. The right hand (white) portion contains details of your authorised
repeat medication and you should keep this to use when you need to request further supplies, especially
if you have to take multiple medication. You can also re-order your prescriptions online by visiting our
website: www.crawcrookmedicalcentre.nhs.uk.
Patients can now nominate their regular Pharmacy. Prescriptions will then transfer via the secure NHS
internet to your chosen Pharmacy. This is quick and secure and allows prescriptions to be traced. If
your repeat is transferred electronically then the Pharmacy should give you a repeat sheet to use to reorder.

Repeat Prescriptions & Medication cont ...
You should clearly tick the box next to all items you require and strike through the items you do not
require. You, or a family member / friend, can hand in the re-ordering slip to a receptionist, or post them
into the repeat prescription box in the reception area. Alternatively, you can post your prescription to us
if that is more convenient but please include a stamped addressed envelope if you want us to post your
prescription back to you. You can also register to order online using our secure website
www.crawcrookmedicalcentre.nhs.uk.
You may be asked to make an appointment to consult a Doctor about continuing repeat medication near
the end of the authorised period of treatment.
If you wish to collect a repeat prescription on the day of your appointment, you should give us at least 48
hours notice in the usual way and collect the prescription when you attend for your appointment.
Ordering Once a Month
We appreciate that if you take multiple medications these can often be difficult to manage. However, if
you could try to request all required medications at the same time this would not only significantly reduce
the number of prescriptions requested, but also reduce the risk of any mistakes occurring. If your medication regime is stable you could perhaps consider batch prescribing (see next page).
Telephone Answering Service
The prescription request by telephone will cease on 31st March 2016 and we therefore would request
patients in preparation for this to begin to use the following methods to access prescriptions:
• Booking online using our Website
• Putting white copy of prescription into box in reception
• Consider repeat prescribing
• Hand in the white copy of your prescription at reception.
Batch Prescribing / Repeat Dispensing
Patients taking regular medications for stable conditions that are unlikely to change over months/years
can arrange for their monthly prescription to be automatically sent to the Pharmacy each month without
the need to order.
On collection the Pharmacy will check your medication has not been changed by the Dr or the Hospital
and also give you the opportunity to decline any items you do not need that month.
This works particularly well for those needing 6 monthly or annual reviews. Please ask our reception
staff for more details.
Chemist Delivery Service
Lloyds, Boots and many other chemists all run schemes that allow patients to order repeat medications
through them. This means that the chemists liaise with us rather than you. The advantage is therefore
that you only need to visit one place rather than two. Please ask our reception staff, or the respective
chemist, for more details.

Carers
A carer is someone of any age who provides UNPAID practical and emotional support to someone who
cannot manage day-to-day activities due to their age, illness, mental ill-health, physical or learning disabilities and alcohol or substance misuse. The Practice has 2 ‘Carer Champions’ who are available for
advice.
Carers are often family members, parents, children or friends. In partnership with Gateshead Carers
Association we have a new protocol which will enable us to help carers access local services and
support. We know that carers are vulnerable to depression, stress, ill health and social isolation.
Support for carers can come in many forms, including but not limited to, emotional support, counselling,
free flu vaccines, free hepatitis B vaccines, respite care, holiday breaks, take a break services, kids
breaks for children who are carers, housing advice, financial advice, equipment training and computer
skills training.
Carers assessment forms are available at the back of our waiting room or you can download this
from our website either for yourself or someone you know is a carer. This is not an assessment of the
carers skills or suitability to be a carer, but an assessment of needs and available support for the carers
themselves. Please complete the form and hand it back in to us as soon as possible to access the help
that is available.
Va c c i n a t i o n s
Adult Vaccinations
Routine immunisation and foreign travel immunisation can be arranged by appointment with our practice
nurse. Please make an appointment 6 weeks before departure to discuss vaccination requirements in
relation to timescale and payment if appropriate.
Child Vaccinations
Immunisations are given to babies and young children to protect them from serious childhood diseases
appointments can be made with our Practice Nurse here at Crawcrook Medical Centre, and information
leaflets about childhood immunisations are available at reception and in the waiting room. Batch
numbers are recorded for all vaccines administered.
Flu Vaccinations
Flu immunisation is needed each year, since immunity lasts only for the current flu season. We run an
extensive programme of flu immunisation each year, usually at the beginning in October. Immunisations
are carried out by our practice Nurses and HCAs. They are particularly recommended for patients who
are 65 and over, who suffer from asthma or lung disease, heart trouble, diabetes, kidney disease or drug
treatments which lower natural defenses and can make people more prone to flu and its complications.
We have a dedicated flu vaccination day for patients to attend, information about this will be available
from reception as well as on our website, however individual appointments are also available at the
practice.
Pneumococcal / Shingles Immunisation
Pneumococcal and Shingles vaccinations are available to patients over the age of 70 to help protect
them against pneumococcal infection which can cause diseases such as pneumonia, septicemia (blood
poisoning) and meningitis. If you have a heart or lung condition, diabetes, have no spleen, or have an
impaired immune system, you are also eligible for the pneumonia immunisation.
Unlike the flu vaccine, most people will only need to have this once.

Health Checks - NHS & Private
NHS
All patients aged between 40 and 74 are entitled to a free NHS health check. This is because people in
this age range are at an increased risk of high blood pressure, heart disease, stroke, kidney disease or
diabetes (collectively known as Vascular Disease).
The check will take 20 minutes (a double appointment with one of our Health Care Assistants) and it
includes some simple tests and questions about your lifestyle. You will also have the opportunity to
discuss any concerns or issues you may have and find out about the many new services available to
help support you in any lifestyle changes.
Once we have your test results back we will be able to calculate your vascular risk score and will contact
you if any further action is necessary. This could include discussion about your lifestyle, access to local
lifestyle services and/or appropriate treatment. Please check online at www.healthcheck.nhs.uk for more
details.
Please take advantage of this check as evidence suggests that small changes to your lifestyle can help
dramatically reduce your risk of developing vascular disease and maintain a good quality of life in the
future.
Private Health Checks
If you need a special fitness certificate, e.g. for sport, pre-employment, HGV licence or elderly driver
certificate, this will be carried out by special appointment outside of normal surgery hours and a fee will
be charged. Please ask at reception for details or look on our website for a list of charges
Medical examinations requested by Insurance Companies can be carried out by special arrangement,
usually outside of normal surgery hours, there will be a fee payable for this.
Home Visits
We comply with the National Visiting guidelines for home visits. Patients are eligible for a home visit if
they are:

bed/house bound
in the doctors opinion are likely to suffer a severe deterioration in the medical condition if they have
to travel to the surgery
have severe disabling conditions that render them housebound, for example, severe COPD and
are using oxygen concentrators; severe MS/rheumatoid arthritis
are terminally ill and are unable to leave the house.
These guidelines apply to all patients irrespective of age and include those who are resident in nursing
and residential homes.
If you are not eligible for a home visit, you will be asked to attend the practice.

Who to see at Crawcrook: A Patient Guide
General Practitioners (GPs)
GPs should be seen for:
•
complex medical and mental health problems
•
children under the age of two in emergency surgeries
Do not book an appointment with a GP for things such as bloods, blood pressure checks, injections,
dressings, smears or travel assessments unless you have been asked to do so.
Nurse Practitioners (NPs)
Nurse Practitioners are able to issue prescriptions and are capable of dealing with the vast majority of
conditions which arise on an urgent basis (except with children under two years of age) including:
•
acute back and musculoskeletal pains
•
urinary tract infections
•
vomiting and diarrhoea
•
sore throats and tonsillitis
•
chest infections
•
ear infections
•
side-effects or problems relating to contraception.
NPs can refer to a doctor immediately if necessary. Please note that children under two years of age
being seen in an emergency surgery must be seen by a GP so please inform the receptionist the age of
the child if you call for an emergency appointment for them.
You can also see a NP for things such as review appointments (as directed by GPs or NPs), diabetes
clinic, or blood pressure monitoring and treatment.
Practice Nurses
Our Practice Nurses can be seen for:

cervical smears; contraceptive pill checks (if experiencing side-effects or problems it would be best
to see a NP or GP) and Depo-Provera contraception injections. Provided you have no problems
with your choice of contraception you need only see the Practice Nurse twice a year

Confidentiality & Medical Records
The Practice complies with data protection and access to medical records legislation.
information about you will be shared with others in the following circumstances:

Identifiable
To provide further medical treatment for you e.g. from District Nurses and Hospital services.
To help you get other services e.g. from the Social Work Department. This requires consent.
When we have a duty to others e.g. in Child Protection cases anonymized patient information will
also be used at local and national level to help the Health Board and Government plan services
e.g. for diabetic care.
If you do not wish anonymous information about you to be used in such a way, please let us know.
Reception and administration staff require access to your medical records in order to do their jobs.
These members of staff are bound by the same rules of confidentiality as the medical staff.
Freedom of Information
Information about the General Practitioners and the Practice required for disclosure under this act can
be made available to the public. All requests for such information should be made to the Practice
Manager.
Access to Medical Records
In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients may
request to see their medical records. Such requests should be made through the Practice Manager and
may be subject to an administration charge. No information will be released without the patient consent
unless we are legally obliged to do so.

Complaints
We make every effort to give the best service possible to everyone who attends our Practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine
cause for complaint. If his is so, we would wish for the matter to be settled as quickly and as amicably
as possible.
To pursue a complaint please contact the Practice Manager who will deal with your concerns in the
appropriate manner. Further written information is available regarding the complaints procedure from
our reception.
Violence Policy
The NHS operates a zero tolerance policy with regard to violence and abuse. The Practice has the right
to remove violent patients from the list with immediate effect in order to safeguard Practice staff, patients
and other persons.
Violence in this context includes actual or threatened physical violence or verbal abuse which leads to
fear for a person’s safety. In this situation we will notify the patient in writing of their removal from the
list and record in the patient’s medical records the fact of the removal and the circumstances leading to
it.

Safer Families: Safer Families is a domestic violence intervention service, which seeks to increase
the safety of women and children as well as providing perpetrators with opportunities for change. It
