What is the Dental Domiciliary Service? The community dental domiciliary service is provided by East Cheshire NHS Trust. Domiciliary dental care provides access to dental services for those who cannot, through physical or mental disability, reach a dental surgery. The service is provided within a home setting, which may be the person’s own home, or within a residential accommodation, such as a nursing home. People who may be eligible for the domiciliary dental service include: individuals who are housebound due to complex medical histories individuals with mental disabilities, such as Agoraphobia or Alzheimer’s disease individuals with physical disabilities, such as mobility difficulties individuals in hospital or hospice, whether short or long term. What services can be provided in a domiciliary setting? There is a range of dental services which can be provided, including simple treatments, provision of dentures, simple extractions and preventative advice. Not all dental procedures can be provided in a domiciliary setting, in some cases a hospital or clinic environment may be necessary. Each case is individually assessed by the dentist. Preparing for the domiciliary dental visit: Prior to the visit, you will receive a domiciliary referral request form, a medical history form and an NHS declaration form, which should be completed and returned to the domiciliary team. Please ensure the forms are correctly filled in with a current address, postcode and telephone number. If you require assistance from a family member or other carer, please ensure that they can be present at the first appointment. If a patient suffers from a mental health disability, and is unable to consent to his or her own treatment, the dental team will require information as to who can legally sign on their behalf. If the patient does have a next of kin/power of attorney/family member or solicitor dealing with their care, please include this information at the time of completing the referral. Please contact the clinic if an interpreter is required. NHS dental charges apply for domiciliary dental services. Prior to the first appointment the dentist will need to know whether you are exempt from NHS dental charges. No payment will be taken at the appointment time. Identification of staff Two members of staff will identify themselves with NHS name badges. If the patient has concerns about who the caller is please contact our head office on 01270 275807. Environment We would request that you assist the dental team by providing a location in which treatment can be provided with dignity and privacy. Access to water and electricity will be required during our visit. Please make sure that all pets are removed from the treatment area, and that dogs are safely secured in an alternative room whilst the dental team make their visit. How to access the service You can be referred into the domiciliary service by a health care professional, for example: a doctor, general dentist or district nurse. Self referrals can also be made by yourself or by a carer on your behalf.
