This booklet describes the program’s services in
more detail, as well as its rules and regulations,
policies and procedures. Please read this booklet
thoroughly so you understand each program and
how to use them. The Union’s principal purpose
is to serve your needs. The Member Assistance
Program (MAP) is one more way in which your
Union is working for you.

Financing
What about member’s eligible dependents?
An important element of your MAP is money!
Prescription/patient assistance
services and information
regarding community
resources are available to the
member’s eligible dependents.
Where it comes from and how it may be
used should be of interest to you.
Who Pays for MAP?
The governing body of your Union,
the Executive Board of Local
338 RWDSU/UFCW, approved
the creation of the Member
Assistance Program and
authorized the expenditure of
the Union’s assets to pay for
the cost of the Program. The
Union’s assets come from your
dues and initiation fees and from
investment income.
• Your lawful spouse,
unless legally separated
(by virtue of a legal
separation agreement, or
court decree or judgment)
Eligibility
Who is Eligible for MAP?
All members of Local 338
RWDSU/UFCW who are
members in good standing
(i.e., initiation fees paid
and dues are up to date),
and who are actively
employed immediately
prior to seeking MAP
services are eligible to
participate in most of
the MAP programs.
• Your children from
birth to age 26. (A child who
reaches age 26 who is physically or mentally
incapable of self-support at the time and who
is covered under the Plan on the date he/
she reaches 26, may continue to be eligible
provided the child remains incapacitated. Proof
of incapacitation must be received by the Union
Office within 31 days of the child’s 26th birthday.
If requested, you will be required to submit
additional proof of continued disability before
eligibility will be extended.)
Are there other Restrictions?
The Alcohol and Substance Abuse services are
available ONLY to full-time members of Local 338
RWDSU who are members in good standing, and
who are actively employed immediately prior to
seeking MAP services.

Program Services
When will eligibility terminate?
MAP provides services
intended to assist you when
confronted with a number of
different crises or serious
circumstances, and include:
• Domestic Violence Referrals
• Mental Health Referrals
• Referrals regarding community resources
• Emergency Housing Referrals
• Prescription/Patient Assistance
• Alcohol and Substance Abuse Assessment and
Treatment Referrals

MAP Prior Assessment
The member must access
the MAP program by first
contacting the Local 338
MAP Director for preassessment; failure to
obtain pre-assessment for
any services will result in the
member’s ineligibility for the
MAP program. The MAP
Director will conduct
an assessment of the
member’s needs and
make the appropriate
referral to a contracted
provider of
covered services.
Domestic Violence Assistance Referrals
If you are facing violence at the hands of
a family member or domestic partner,
the MAP Director can help assess the
situation and make referrals to community
services that may be able to intervene on
your behalf or provide a temporary
safe haven.
Emergency Housing Referrals
At times you may find yourself
confronting eviction, facing homelessness,
or another housing crisis, such as the loss of your
dwelling due to fire or flooding. While we are not a
real estate office and can’t directly help you find new
housing, the MAP Director may offer appropriate
referrals for assistance and information to help you
plan for long term housing stability. MAP does not
handle general housing needs, only emergency
housing issues.

Your eligibility for MAP services terminates on
the day when any of the following occurs:
• You cease to be employed by an employer
in a unit covered by a collective bargaining
agreement between your employer and
Local 338 RWDSU/UFCW;
• Your employer ceases to have a collective
bargaining agreement with
Local 338 RWDSU/UFCW;
• The MAP program, or the applicable portion
thereof, is discontinued or eliminated; or
• You fail to adhere to the policies and proce
dures of the program as set forth in this
document or by the MAP Director.

When your prescription benefits are not
available — for example, because you
are not yet eligible — or you do
not have prescription benefits
through a Local 338 Health &
Welfare Fund or other health
coverage, yet you need life
sustaining or critically important
prescriptions, the prescription/
patient assistance program may
be able to find an alternative,
low or no-cost source of such
medications. More over, in limited situations,
copayment assistance is sometimes available.

Alcohol and Substance Abuse Services
At times you may find yourself combating the
disease of addiction to alcohol or drugs, often
endangering your job, health, and relationships.
MAP provides alcohol and substance abuse
assessments and referrals to treatment or counseling
for eligible full-time members only. The program
can provide family members with information about
community or other resources offering alcohol and
substance abuse treatment, but family members
are not covered by the program for assessment,
treatment, or counseling.The program includes the
following services, subject to pre-assessment by the
MAP Director:
• Detoxification
• In-patient rehabilitation
• Outpatient aftercare
• Group/Individual session
• All subject to prior assessment by the MAP
Director, and MAP’s assessment of medical necessity
and member compliance with treatment regimen.
Payment for Alcohol and Substance
Abuse Services
When a referral for counseling or treatment is
required, there are costs incurred. Such costs are
paid by the MAP Program only if:
• The MAP Director conducted a prior assessment of the member’s needs and the services provided
are medically necessary
• The member receives services in a facility that is
contracted with the Local 338 RWDSU/UFCW’s
MAP program to provide such services and to
which the MAP Director has referred the
member, and
• The member otherwise complies with the MAP
program guidelines as communicated by the
MAP Director.
MAP Periodic Sessions for Alcohol
and Substance Abuse Service
Once per week, or as needed
according to the MAP Director’s
assessment, the member must also
meet with the MAP Director
to review his or her status
for reassessment. Such
sessions commence upon
acceptance into the MAP
program and continue until
released by the
MAP Director.

Mental Health Referrals
With depression and anxiety becoming more
prevalent, the Member Assistance Program is here
to assist individuals who are struggling with mental
health issues such as anxiety, stress, depression, PTSD
and other related mental health diagnosis. Individuals
who suffer from mental health, have access to mental
health housing, SSI benefits, care coordination and
other services to help you in the community.

The MAP Director is a certified mental health
professional with social work and substance abuse
background. The MAP director has access to
contacts and resources in the community that can
assist you and your family if you are in need of
services and help. To receive information on what
community resources are available contact the
MAP Director.
Privacy and Confidentiality
The Members Assistance Program is run under the
guidance of a certified mental health professional.
The MAP Director is required to adhere to the
strictest standards of confidentiality. Without your
consent, no information you reveal
to the MAP Director can or will be
released or disclosed to any other
party, including your employer,
your union representative, or any
other union staff. Rest assured,
your information is held in the
strictest of confidence by the
MAP Director.
