McLin Curriculum Vitae Page 3 RESEARCH ACTIVITIES/PROJECTS Implementation of Automated Dispensing Systems in Five Free Standing Hospice Care Centers This technology provided a means for the organization to improve inefficiencies in the drug distribution process, improved drug security measures especially with opioids, advanced the organization to the current standard medication administration practices, incorporated a means to improve medication safety in the hospice care centers, and mandated pharmacist oversight. Utilized project management tools to develop a business model and presented it to executive leadership. It outlined the scope of the project, financial analysis, strategic planning, potential vendors, timeline, and implementation process. Project was successfully completed and has significantly reduced medication errors in all care centers. Patient Controlled Analgesia (PCA) Quality Initiative – A Focus on Safety Developed a company-wide policy and procedure on PCA usage in a hospice population with a focus on nursing and physician education; aspects included a mandatory continuing educational series on PCA pump programming with a required didactic portion and demonstrated competency on the PCA, pump features, appropriate patient selection, required monitoring, critical aspects to communicate to physicians for dosing adjustments, and the development of a patient/family educational brochure. Pain Management Consultation Service(s) Provided pain management consultation services for AvMed members referred by oncology case managers; services included serving as a liaison for the patient, direct communication to the prescribing physician(s), development of an ongoing medication action plan in conjunction with non-pharmacologic therapies and provision of an optimal pain management and palliative care plan to improve a patient’s quality of life. Assisted leadership with development of various policies and procedures surrounding pain management-related prior authorizations and/or formulary decisions. In addition, developed interventional pain management guidelines for AvMed medical director(s) to utilize for medical benefit exception requests for implantable PCA pumps and spinal cord stimulators. Demerol Project Designed and implemented a clinical intervention process to identify patients on chronic meperidine therapy. Provided education to prescribing physicians and patients in order to ensure optimal pain management and to reduce the risk of an adverse event. Identifying At Risk Patients for Drug Therapy Complications – Health Information Designs (HID) DURBase Integrated an established “at risk” database to the Workers Compensation patient population to identify and intervene in patients at risk for a drug-related adverse event; involved contrasting claims data against therapeutic criteria to identify at risk characteristics, evaluating the clinical significance of each patient’s case, generating intervention letters to physicians, and monitoring for drug therapy revision. Updating Physicians on Revised Product Boxed Warnings Monitoring FDA websites for revised drug product information relating to new cautions, concerns, or addition of boxed warnings. Once a product is identified applicable to the Workers Compensation patient population, a database query is conducted to identify specific patients in order to direct appropriate interventions (written or via telephone) to the prescribing physician(s).
