Purpose
To diagnose and localize myocardial infarction and ischemia, utilizing two
intravenous injections of a radiopharmaceutical on consecutive days. This scan is
performed on selected patients who are unable to exercise adequately.
Applicability
This policy and procedure applies to Covenant Health Edmonton Acute Care
facilities, patient care provider staff and physicians.
Procedure
Duration
Injection will take 15 minutes, imaging will take 40 minutes to complete.
Booking
Procedure
1. By the Nursing Unit or attending Internist or Cardiologist.
2. Must be booked in advance (minimum one day).
At the GNCH:
Bookings are made through the Electrodiagnostic Services Department.
At the MCHE:
Book through Outpatient Booking Office.
3. Generate an order using electronic order entry system. Indicate pertinent
history, patient allergies, infection precautions, if patient is diabetic, has renal
insufficiency or any other special care requirements.
4. Ensure the patient has had no other Nuclear Medicine procedures scheduled
within 48 hours preceding or following this examination.
5. If the patient has undergone an angiographic procedure, the Persantine
procedure should be scheduled 24 hours post-angiography or 48 hours postangiography with angioplasty.
Preparation
The attending physician should explain the procedure to the patient.
If the patient requires an interpreter, the nursing unit will make arrangements for
Interpretive Services to be available for the procedure. The nursing unit will then
advise Diagnostic Imaging that these arrangements have been made.
a) Asasantine and Persantine must be stopped 48 hrs prior to examination.
b) Certain cardiac and lung medications (i.e. Theophylline) should be discontinued
24-36 hours prior to the examination. Sprays can be continued. Attending
physician to assess.
c) Diabetic patients' diet and medications must be monitored by the attending
physician.
d) Fasting from midnight prior to the examination. No caffeine products 12 hours
prior to the rest test (i.e. coffee, tea, chocolate, colas, etc.) and 36 hours prior to
the stress examination.

e) The patient should be dressed in a hospital gown, pajama pants and wearing
footwear appropriate for walking on a treadmill for the stress portion.
f) Ensure the patient's chart is complete with doctor’s order, history of allergy and
pertinent history. This must accompany the patient to Nuclear Medicine.
g) Transportation as warranted by patient condition.
h) This procedure requires an IV to be initiated with normal saline or D5W
(preferably in the right arm) for the stress examination (MCH only).
Post
Procedure
Care
1. The patient may resume normal diet after imaging each day.
2. Nuclear Medicine will advise Nursing Unit of time of imaging and patient
preparation for the portion of the examination to be performed on the following
day.
