RESOLUTION NO. 28 SERIES OF 2014
A RESOLUTION EXTENDING SINCEREST THANKS AND PROFOUND GRATITUDE TO
VISION MEDIX EYE CLINIC, ANGONO, RIZAL THRU DRA. SYLVIA LAURA MILAN
MIRASOL, PROPRIETRESS, FOR SPEARHEADING THE CONDUCT OF EYE CARE MISSION
FOR MUNICIPAL OFFICIALS, EMPLOYEES AND CITIZENRY OF STO. TOMAS,
PANGASINAN ON FEBRUARY 05, 2014.
WHEREAS, the Local Government Unit, Sto. Tomas, Pangasinan thru Honorable
Mayor Timoteo “Dick” S. Villar III has envisioned an efficient and A1 public service
managed by able, healthy, fit and strong employees and leaders;
WHEREAS, as a continuing effort of the LCE, steps and procedures has been
undertaken since his assumption into office last July, 2013 as support mechanism of the
above vision-mission;
WHEREAS, several activities lined up were (1) Zumba every Monday and Friday
afternoon after office hours participated herein by all officials and employees for PHYSICAL
FITNESS (2) thanksgiving masses, ecumenical services and day of prayers participated
herein by all religious sect leaders for MORAL AND SPIRITUAL FITNESS (3) medical and
dental missions, eye care clinic as spearheaded by VISION MEDIX EYE CLINIC thru Dra.
Sylvia Laura M. Mirasol thus providing free eye check-up, discounted ocular lenses for
MENTAL AND AGILITY FITNESS;
WHEREAS, in every endeavor, good deeds and noble acts done and exhibited, the
sweetest whisper, the most beautiful sounds and the greatest response by the recipient to
the giver are the words “ T-H-A-N-K Y-O-U”;
WHEREAS, truly, words of consolation and gratitude are due to the above
undertaken and a prayer that they may continue to be of grace and help to the humble
town of Sto. Tomas, Pangasinan;

NOW THERFORE, on motion of Coun. Tomas S. Castañaga, Jr. duly seconded by all
members present, it was;
RESOLVED, as it is hereby RESOLVED, to extend sincerest thanks and profound
gratitude to Vision Medix Eye Clinic, Angono, Rizal thru Dra. Sylvia Laura Milan Mirasol,
Proprietress for spearheading the conduct of Eye Care Clinic for municipal officials,
employees and citizenry of Sto. Tomas, Pangasinan on February 05, 2014.
RESOLVED, FINALLY, that copies of this resolution be forwarded to Dra. Sylvia
Laura Milan Mirasol for her information, guidance and reference.
