Presenters:
Michelle Cantwell, DMD
Dr. Cantwell is a graduate of the University of Pittsburgh, School of Dental Medicine and
graduated in 1998. Dr. Cantwell received a U.S. Navy scholarship and served at Sewell's Point
Dental Clinic, NDC Mid-Atlantic in Norfolk, Virginia. She completed her active duty service as
a Lieutenant Commander in 2004. Dr. Cantwell opened Cantwell Dental Group located in
Lancaster, Pennsylvania in 2005 and in 2012 she opened the Center for Dental Sleep Medicine to
create a practice devoted solely to the management of her sleep medicine patients. She
completed the requirements for Diplomate through the ABDSM in 2014.
Mitchell Levine, DMD
Dr. Mitchell Levine is both a Diplomate of the American Board of Orthodontics and the
American Board of Dental Sleep Medicine. A member of the prestigious Schulman Study Group,
he presently chairs both Health Policy and Ethics Committees of the AADSM. His orthodontic
practice is located in Jacksonville, Florida.

Kathleen Bennett, DDS, Diplomate, ABDSM
Dr. Bennett is a 1979 graduate of Georgetown University and a 1983 graduate of University at
Buffalo School of Dental Medicine. She completed her GPR at Buffalo General Hospital in 1984
and practiced restorative dentistry for 19 years. She became an ABDSM Diplomate in 2002 and
served on the education committee for many years. She continues to serve as a facilitator with
the AADSM study club series. She served on the AADSM Board of Directors since 2006 and
became President of the AADSM in June of 2014. She has limited her practice to oral appliance
therapy since 2005 and is credentialed in the department of internal medicine division of
pulmonary sleep medicine at the UC Health West Chester hospital. She works with the sleep
medicine fellows in the Cincinnati Children’s Hospital Medical Center sleep fellowship program
training the physicians in oral appliance therapy.
Harold Smith, DDS, Diplomate, ABDSM
Dr. Smith is a practicing dentist in Indianapolis, Indiana. He graduated from Indiana University
and Indiana University School of Dentistry. He has served as a dental consultant to five major
Indianapolis hospital sleep centers and has been active in their accreditation process. He is the
Clinical Director of Dental Sleep Medicine of Indiana. Dr. Smith holds the designation as a
Diplomate of the American Board of Dental Sleep Medicine. He has served as President of the
American Academy of Dental Sleep Medicine and the American Board of Dental Sleep
Medicine. He is a recipient of the American Academy of Dental Sleep Medicine’s Distinguished
Service Award. Dr. Smith continues to be active in his local dental societies and the American
Dental Association. He is the President-Elect of the AADSM.
Gail Demko, DMD
Dr. Demko received her DMD from Boston University and was a hospital based dentist at Beth
Israel Deaconess Medical Center in Boston for 20 years. In 1997 she limited her dental practice
to the treatment of OSA and is an expert advisor to the FDA on oral appliance therapy. She is
the Immediate Past-President of the AADSM and is a recipient of the AADSM Distinguished
Service Award.
Leslie Dort, DDS, Diplomate, ABDSM
Dr. Dort is a Calgary dentist whose practice is limited to treating sleep disordered breathing and
TMJ issues. She is a clinical lecturer in the surgery department at the University of Calgary and
is a member of the American Academy of Sleep Medicine’s Practice Parameters for Oral
Appliance Therapy task force. She is also the current editor of the Journal of Dental Sleep
Medicine. Dr. Dort is affiliated with the University of Calgary and is currently involved in
research related to oral appliance therapy for sleep disorders and the development of best
practice guidelines in dental sleep medicine. She is the Secretary/Treasurer of the AADSM.
