FDAACTIONS
New Wrinkle Therapy Approved. A first-of-its kind wrinkle therapy
is available to patients, following FDA’s approval last month of the
BLA for LaViv (azficel-T, Fibrocell Science) personalized aesthetic
cell therapy.
LaViv was approved for treatment of nasolabial folds, based on two
identical Phase III multi-center, randomized, double-blind, placebo-controlled studies involving 421 patients. Treatment was
administered in three sessions approximately five weeks apart.
Based on investigators’ and patients’ assessments, a significantly
greater proportion of patients demonstrated a positive response
to treatment with LaViv than with placebo. Treatment was shown to
improve the appearance of nasolabial fold wrinkles for six months
after the third and final treatment.
The approval of LaViv offers an attractive new option for wrinkle
treatment, suggests study investigator Robert A. Weiss, MD,
Clinical Associate Professor, Johns Hopkins School of Medicine and
Director, Maryland Laser Skin and Vein Institute. He stresses that
LaViv is not a volume filler like Juvéderm or Restylane, rather it is
intended for finer wrinkles closer to the surface of the skin. The
injection contains no collagen but consists of fibroblasts to
encourage endogenous collagen production.
According to Dr. Weiss, the process is relatively straightforward.
The area behind the ear is anesthetized with lidocaine before 23mm punch biopsies are taken. The biopsy materials are placed in
a transport medium and then shipped out for processing. The
roughly six-week processing technique purifies and multiplies
fibroblasts, which are then harvested and shipped back to the clinic for arrival one day before the patient’s scheduled treatment.
Materials need only be refrigerated, not frozen. When the patient
presents, fibroblasts are injected superficially in the dermis. Dr.
Weiss explains to patients that the “collagen producing cells rev
up in that laboratory environment. They are revved up and ready
to produce collagen when re-injected.”
Dr. Weiss predicts strong acceptance of the process, as patients
“really do embrace the concept of using their own cells.” He says
that the procedure won’t be intimidating for patients or physicians
and that physicians will receive training on harvesting samples
and injecting firboblasts. He also indicates that use of a standard
anti-aging regimen, including topical retinoids that also stimulate
fibroblasts, should be used in conjunction with the technique.
