O seu tratamento consiste na remoção cirúrgica ou percutânea do obstáculo e apresenta excelentes resultados imediatos mas uma persistência significativa de problemas residuais. Objectivos: Descrever a apresentação, tratamento e evolução de longo prazo de uma população não seleccionada de 100 doentes consecutivos com CoAo isolada num único centro de Cardiologia Pediátrica. Material e Métodos: Estudo retrospectivo de todos os doentes com CoAo isolada tratados durante os últimos 21 anos (1987-2008). Resultados: Os doentes (n=100, 68.3% sexo masculino) foram diagnosticados com uma idade mediana de 94 dias (1 dia a 16 anos). A apresentação clínica diferiu entre os doentes com idade inferior e superior a um ano, os primeiros com insuficiência cardíaca e os segundos encontrando-se assintomáticos e com evidência de hipertensão arterial (88% e 63%, respectivamente; p<0.01). A terapêutica, em mediana 8 dias após o diagnóstico, foi cirúrgica em 79 casos (20 anastomoses topo-a-topo, 31 flaps sub-clávia, 28 patchs) e percutânea nos restantes 21 (15 angioplastias com balão, 6 com stent). A idade média dos doentes cirúrgicos foi mais baixa que nos percutâneos (3,4 versus. 7,5; p<0,01). A mortalidade imediata foi de 2% ABSTRACT Isolated aortic coarctation: experience in 100 consecutive patients Introduction: Coarctation of the aorta (CoA) is a stenosis usually located in the descending aorta. Treatment consists of surgical or percutaneous removal of the obstruction and presents excellent immediate results but significant residual problems often persist. Objectives: To describe the presentation, treatment and long-term evolution of a population of 100 unselected consecutive patients with isolated CoA in a single pediatric cardiology center. Methods: This was a retrospective study of all patients with isolated CoA treated during the last 21 years (1987-2008). Results: The patients (n=100, 68.3% male) were diagnosed at a median age of 94 days (1 day to 16 years). The clinical presentation differed between patients aged less or more than one year, the former presenting with heart failure and the latter being asymptomatic with evidence of hypertension (88 and 63%, respectively; p<0.01). Treatment, a median of 8 days after diagnosis, was surgical in 79 cases (20 end-to-end anastomosis, 31 subclavian flap, 28 patch) and percutaneous in the remaining 21 (15 balloon angioplasty, 6 with stenting). Não se registou mortalidade tardia, com um seguimento médio foi de 7,2±5.4 anos. Verificou-se recoarctação em 8 doentes (6 cirúrgicos, 2 percutâneos). Existem 46 doentes que têm actualmente HTA (19 em repouso, 27 com o esforço), notando-se nestes uma idade mediana de diagnóstico mais tardia que os outros (23 versus 995 dias; p<0,01). Conclusões: A CoAo isolada é uma patologia de excelente prognóstico a curto prazo, que mantém uma incidência significativa de complicações a longo prazo, pelo que não deve continuar a ser olhada como uma “simples” obstrução na aorta descendente, mas antes como uma patologia complexa que requer seguimento atento após o tratamento. A sua apresentação potencialmente insidiosa obriga a uma elevada suspeição clínica, salientando-se a importância da palpação dos pulsos femorais no exame objectivo. O atraso no seu tratamento tem um impacto na morbi-mortalidade tardia. Tendo em conta os dados actualmente disponíveis quanto aos resultados imediatos e tardios, a escolha definitiva da técnica terapêutica depende da idade do doente, lesões associadas e experiência da equipa médico-cirúrgica. Há que dar especial importância ao estudo da HTA no seguimento destes doentes, seus factores de risco e complicações. Immediate mortality was 2% and occurred in the surgical group. There was no late mortality, in a mean follow-up of 7.2±5.4 years. Recoarctation occurred in 8 patients (6 surgical, 2 percutaneous). There are 46 patients who currently have hypertension (19 at rest, 27 with effort), their median age at diagnosis being older than the others (23 vs. 995 days; p<0.01). Conclusions: Isolated CoA has an excellent short-term prognosis but a significant incidence of long-term complications, and should thus no longer be seen as a simple obstruction in the descending aorta, but rather as a complex pathology that requires careful follow-up after treatment. Its potentially insidious presentation requires a high level of clinical suspicion, femoral pulse palpation during physical examination of newborns and older children being particularly important. Delay in treatment has an impact on late morbidity and mortality. Taking into account the data currently available on late and immediate results, the final choice of therapeutic technique depends on the patient’s age, associated lesions and the experience of the medical-surgical team. Hypertension should be closely monitored in the follow-up of these patients, as well as its risk factors and complications. INTRODUÇÃO INTRODUCTION A C coarctação da aorta (CoAo) é uma diminuição do calibre da aorta, habitualmente localizada entre a emergência da artéria subclávia esquerda e do canal arterial. Representa 5-8% das cardiopatias congénitas, com uma incidência de 20 a 60 casos por 100.000 nados vivos (1), sendo mais frequente no sexo masculino (1,7:1)(2). oarctation of the aorta (CoA) is a narrowing of the aorta, usually located between the emergence of the left subclavian artery and the ductus arteriosus. É particularmente frequente a associação com válvula aórtica bicúspide (mais de 50% dos casos)(3). Considera-se que existe uma CoAo complexa quando se encontram associadas outras lesões cardíacas, como a comunicação inter-ventricular, obstáculos sistémicos adicionais ou outras mais complexas (3). A história natural da CoAo tem um mau prognóstico, verificando-se o óbito da maior parte dos doentes não tratados até à quarta década de vida, por insuficiência cardíaca, acidente vascular cerebral, doença coronária ou morte súbita (4). O espectro clínico varia com a idade dos doentes, desde a insuficiência cardíaca no recém-nascido até ao adolescente assintomático. O seu tratamento consiste na remoção cirúrgica ou dilatação percutânea do obstáculo e apresenta excelentes resultados imediatos mas uma persistência significativa de problemas residuais (5), incluindo a incidência de re-estenose e a hipertensão arterial (HTA). Os autores apresentam um estudo retrospectivo das coarctações aórticas isoladas observadas consecutivamente nos últimos 20 anos num centro de Cardiologia Pediátrica. O objectivo deste trabalho é contribuir para o conhecimento desta entidade com uma amostra homogénea seguida a longo prazo, chamando a atenção para o modo de apresentação, as formas terapêuticas e o seguimento. MATERIAL E MÉTODOS Estudo retrospectivo, por consulta dos processos clínicos dos doentes observados com o diagnóstico de CoAo isolada, no período entre 1 de Janeiro de 1987 e 30 de Junho de 2008 (21 anos). Excluíram-se todos os doentes que necessitaram de tratamento para outras anomalias cardíacas. Procedeu-se à colheita de dados demográficos, semiológicos, forma de tratamento, evolução e seguimento. Utilizou-se estudo de estatística descritiva dos diversos parâmetros usando o programa SPSS versão 11.5 para Windows. CoA may be isolated, or be associated with other congenital cardiac defects, particularly bicuspid aortic valve (over 50% of cases) (3). CoA is considered complex when associated with other heart anomalies, such as interventricular septal defect, additional systemic obstruction or other more complex lesions (4). The natural history of CoA has a poor prognosis, most untreated patients dying by the fourth decade of life due to heart failure, stroke, coronary artery disease or sudden death (5). The clinical spectrum varies depending on the patient’s age, from heart failure in newborns to asymptomatic adolescents. Treatment consists of the surgical or percutaneous removal of the obstruction, which produces excellent immediate results but significant residual problems often persist (6), including restenosis and hypertension. The authors present a retrospective study of consecutive patients with isolated aortic coarctation treated during the last 20 years in a single pediatric cardiology center. The objective of the study is to contribute to knowledge of this entity based on long-term follow-up of a homogeneous sample, focusing on form of presentation, therapeutic techniques and follow-up. METHODS This was a retrospective study, based on consultation of the clinical records of patients diagnosed with isolated CoA between January 1 1987 and June 30 2008 (21 years). All patients who required treatment for other cardiac anomalies were excluded. Demographic data and details of symptoms were collected, together with information on type of treatment, evolution and follow-up. A descriptive statistical analysis of the various parameters was carried out using SPSS for Windows, version 11.5. RESULTS Presentation The study population consisted of 100 patients, 63.8% male (1.78:1). A idade de apresentação variou entre o primeiro dia de vida e os 16 anos com uma mediana de 94 dias. Esta distribuição revelou incidência de 39% dos diagnósticos no período de recémnascido, com 18% adicionalmente diagnosticados entre o primeiro e o 12º mês de vida. Distribution of the sample according to age at diagnosis Com o objectivo de detectar diferenças quanto à apresentação e resultados consoante a idade de apresentação, dividimos a amostra em dois grupos: grupo I com menos de 12 meses e grupo II com mais de 12 meses à data do diagnóstico. O sinal mais comum na apresentação foi a diminuição dos pulsos femorais, presente em 100% dos doentes. No grupo I, a apresentação clínica inicial foi significativamente mais grave pela presença dos sinais e sintomas de insuficiência cardíaca e sopro (88 versus 33%; p<0,001). No grupo II, destaca-se a prevalência significativamente maior de HTA (28 versus 63%; p<0,01) (Tabela 1). 26 Terapêutica O tratamento decorreu em mediana oito dias (2-2070) após o diagnóstico. As técnicas cirúrgicas utilizadas foram anastomose topoa-topo (n=20), flap subclávia (n=31) e patch de pericárdio (n=28). Diagnosis in the other patients was distributed unevenly up to the age of 16 (Figure 1). With the aim of detecting differences in presentation and outcomes according to age at diagnosis, the sample was divided into two groups: group I ? under 12 months; and group II ? over 12 months at diagnosis. The most common sign at presentation was diminished femoral pulses, present in 100% of patients. Initial clinical presentation in group I was significantly more severe, with signs and symptoms of heart failure or murmur (88 vs. 33%; p<0.001). Group II had a significantly higher prevalence of hypertension (63 vs. 28%; p<0.01) (Table I). Table I. The surgical techniques used were end-to-end anastomosis (n=20), subclavian flap (n=31) and pericardial patch (n=28). Fifteen patients were treated by balloon angioplasty and six by stent implantation. The therapeutic approach in group I was mainly surgical throughout the period under analysis, while in group II, balloon angioplasty was introduced in 1998 and stenting in 2007. Median ages according to type of treatment are shown in Table II. Follow-up Mean follow-up of the patients in the study was 7.2 years (standard deviation 5.4). No grupo I, a abordagem terapêutica foi preferencialmente cirúrgica ao longo do período do estudo, enquanto que no grupo II se introduziram a partir de 1998 a angioplastia com balão e em 2007 a implantação de stent. A mortalidade global foi de 2% e ocorreu em dois recém-nascidos, no grupo cirúrgico. No seguimento houve nove doentes que deixaram de comparecer nas consultas. Nos doentes seguidos em consulta não houve mortalidade tardia. Verificou-se re-coarctação (gradiente hemodinâmico superior a 20mmHg) em oito doentes, sendo que um doente tinha sido submetido a flap subclávia, cinco a anastomose topo a topo e dois a angioplastia com balão. Quatro eram recém-nascidos à data do tratamento, e a idade média deste grupo à data do tratamento era de 3,3 anos. Constatou-se HTA (definida como valores superiores ao percentil 95 para idade e sexo) em repouso em 19 doentes, que se mantêm medicados com anti-hipertensores. Recoarctation (hemodynamic gradient of >20 mmHg) occurred in eight patients, one treated by subclavian flap, five by end-to-end anastomosis and two by balloon angioplasty. Four of the group with recoarctation were newborns at the time of treatment, with a mean age for this group of 3.3 years. Hypertension (defined as values above the 95th percentile for age and gender) at rest was found in 19 patients, who are currently under antihypertensive therapy. This group had a significantly older median age at diagnosis (p<0.01) than those who did not develop hypertension (Figure 2). Sixty-two patients underwent exercise testing, on average 9.6 years (2 months to 23 years) after initial treatment, of whom 44% presented an abnormal blood pressure profile during effort: hypertensive response in 8%, arm-leg pressure gradient in 23%, and both in 13%. Since our sample covered a period of more than two decades, we decided to compare early and late periods – 1987-93 (34 cases) and 2003-07 (24 cases) – in order to determine possible variations in the sample over time. No statistically significant differences were found in terms of the parameters studied, namely age (p=0.9) and severity (p=0.85) at presentation, and the presence of hypertension (p=0.5). The deaths occurred in the earlier group. Sessenta e dois doentes efectuaram prova de esforço, em média 9,6 anos (2 meses a 23 anos) após o tratamento inicial. Verificou-se que 44% apresentavam perfil tensional anómalo ao esforço que consistiu em desenvolvimento de resposta hipertensiva (8%), gradiente tensional entre membro superior e inferior (23%) ou ambos (13%). Relation between age at diagnosis and hypertension Considerando que a nossa amostra se prolonga por um período longo de duas décadas, optámos por comparar os seus dois extremos, 1987-93 (34 casos) e 2003-07 (24 casos), de modo a estudar possíveis variações e consequente heterogeneidade da mesma ao longo do tempo. Não encontrámos diferenças com significado estatístico quanto aos parâmetros estudados, nomeadamente a idade (p=0,9) ou gravidade da apresentação (p=0,85) e a presença de HTA (p=0,5). A mortalidade ocorreu no grupo mais antigo. DISCUSSÃO 28 Apresentação Durante o primeiro ano de vida, e em particular no período neonatal, a CoAo manifesta-se habitualmente por insuficiência cardíaca ou choque cardiogénico secundário a obstáculo sistémico crítico. After the first year, most patients are asymptomatic and the disease is diagnosed following detection of diminished femoral pulses, hypertension, headache, claudication, epistaxis or alterations on routine electrocardiography. We divided our sample into two groups in terms of presentation: early (newborns and those aged under one year) and late (those aged over one year). In those under one year of age (57% of the sample), CoA was manifested by heart failure or cardiogenic shock. The presentation of CoA may be masked by patent ductus arteriosus, which is the most common duct-dependent heart malformation that is undiagnosed in routine exams of newborns (7). An estimated 60 to 80% of cases of CoA that present in the neonatal period with heart failure were considered “asymptomatic” at the time of discharge from the maternity ward (7-9). In the group aged over one year (43%), the gradual development of obstruction was manifested later in the form of hypertension and left ventricular hypertrophy. Despite the fact that diminished and delayed femoral pulses are a universal finding that is both sensitive and specific, clinical trials show that delayed detection is common (10-12); referral to pediatric cardiology for suspected CoA prompted by findings of hypertension, low-intensity murmur or diminished lower limb pulses in routine observations occurs in less than 20% of cases (10). It is rare for congenital heart defects with hemodynamic significance to go undetected for years while having harmful effects on the cardiovascular system, and so palpation of femoral pulses and blood pressure measurement should be an integral part of physical examinations at all pediatric ages. CoA was diagnosed prenatally in only one child in our sample. This reflects the historical circumstances of our cohort, many of whom were diagnosed before fetal echocardiography was readily available, as well as the difficulty in prenatal diagnosis of this disease (13), achieved in only 19% of patients in one large center (14). We found that our sample was homogeneous over the period under analysis, with no Ana Peres, et al. Na nossa amostra, verificámos estas diferenças na apresentação nos dois grupos etários. No grupo com idade inferior a um ano, que consistiu em 57% da nossa amostra, a CoAo manifestou-se com insuficiência cardíaca ou choque cardiogénico. A apresentação da CoAo pode ser mascarada pela persistência de canal arterial, sendo a cardiopatia ductusdependente mais frequentemente não diagnosticada no exame de rotina do recém-nascido (6). Estima-se que 60 a 80% das CoAo que se manifestam no período neonatal com insuficiência cardíaca tenham sido considerados “assintomáticas” à data de alta da maternidade (6). No grupo com idade superior a um ano, que correspondeu a 43% dos nossos doentes, o desenvolvimento gradual da obstrução manifestou-se mais tardiamente com hipertensão arterial e hipertrofia do ventrículo esquerdo. Apesar da diminuição e atraso dos pulsos femorais ser um achado universal, sensível e específico, os estudos clínicos mostram que é comum este atraso no diagnóstico (7, 8), sendo a referenciação à Cardiologia Pediátrica motivada por achados (em observações de rotina) de hipertensão arterial, sopro de baixa intensidade ou pulsos diminuídos nos membros inferiores, com suspeição clínica de CoAo em menos de 20% dos casos (7). A palpação dos pulsos femorais e medição da tensão arterial devem fazer parte integrante da observação física, em todas as idades pediátricas. O diagnóstico pré-natal de coarctação foi efectuado em apenas uma das crianças da nossa amostra. Este dado reflecte por um lado as circunstâncias históricas da nossa coorte, quando o ecocardiograma fetal ainda não existia ou era incipiente, e por outro a dificuldade em efectuar o diagnóstico pré-natal desta doença (9), obtido em apenas 19% da prática de um grande centro (10). Verificámos que a nossa amostra era homogénea ao longo dos anos, sem diferenças si- significant differences in age at diagnosis, form of presentation or presence of hypertension. Treatment Management of the patients in our study has taken account of developments in the therapeutic approach to CoA over the last two decades. A surgical approach was employed during the initial years. The techniques used, some of which such as patch aortoplasty are now rarely indicated (15), reflect the historical options and personal preferences of the operators involved over more than two decades. Fifty years after the first surgical repair of CoA (16), the debate continues as to the best technique for surgical treatment: end-to-end anastomosis (more or less extended) (17), end-to-side anastomosis(18) or subclavian flap (19). All have similar immediate success rates, with morbidity of 0-25% (mean 11%) and operative mortality, including in newborns, of less than 2.5% (20, 21). The European Association of Cardiothoracic Surgery Congenital Database (22) reports mean mortality of 1.7 to 7.5%, depending on the surgical technique. Our study’s results compare favorably with these figures, with operative mortality of 2%, in two newborns in cardiogenic shock, treated in the early years of our experience. Percutaneous treatment, introduced over twenty years ago for residual CoA post-surgery (23) and later extended to native CoA (24), has become an important option for this entity (25, 26). In recent years, our series included native CoA treated by balloon angioplasty with or without stenting. Immediate results, with therapeutic success in all patients and no deaths, are comparable to those of other recent series, which show mortality of less than 1% and morbidity for balloon angioplasty of 14-24% (mean 19%) and 0-20% (mean 9%) with stenting (20, 27). In patients aged under one year, surgical treatment is preferred to a percutaneous approach due to the latter’s greater morbidity and mortality (24, 28-30). However, at older ages, there is still controversy as to the best thera- 29 Rev Port Cardiol Vol. 29 Janeiro 10 / Janeiro 10 gnificativas quanto à idade de diagnóstico, modo de apresentação ou presença de HTA. 30 Terapêutica O manejo dos doentes estudados acompanhou a evolução da abordagem terapêutica da CoAo ao longo das duas últimas décadas. A abordagem cirúrgica foi a opção terapêutica nos primeiros anos do estudo. Cinco décadas após a primeira cirurgia para correcção da CoAo (11), persiste o debate sobre a qual a melhor técnica operatória: anastomose topo-a-topo (mais ou menos alargada)(12), anastomose termino-lateral(13) ou o flap de subclávia (14). Todas apresentam sucesso imediato sobreponível, com morbilidade entre 0 e 25% (média 11%) e mortalidade operatória inferior a 2,5% (15, 16). A consulta da base de dados da Associação Europeia de Cirurgia Cardiotorácica (17) mostra uma mortalidade média de 1,7 a 7,5%, dependendo do tipo de técnica cirúrgica. O nosso estudo apresentou resultados que se comparam favoravelmente com estes, com uma mortalidade operatória de 2%, em dois recém-nascidos, na fase inicial da nossa experiência. O tratamento percutâneo, iniciado há duas décadas na CoAo residual pós-cirúrgica(18) e posteriormente alargado às CoAo nativas(19), assumiu um papel importante na abordagem(20). A nossa série inclui, nos anos mais recentes, CoAo nativas tratadas por angioplastia com balão com ou sem implantação de stent. Os resultados imediatos obtidos, com sucesso terapêutico em todos e ausência de mortalidade, são sobreponíveis aos das séries mais recentes, que apresentam mortalidade inferior a 1% e morbilidade para angioplastia com balão de 14-24% (média 19%) e com stent de 0-20% (média 9%) (15, 21). Nos doentes com idade inferior a um ano, a abordagem cirúrgica é preferida em detrimento da percutânea, pela maior morbi-mortalidade desta (19, 22, 23). Contudo, a partir desta idade, a semelhança entre os resultados das várias técnicas mantém a controvérsia sobre a melhor abordagem terapêutica para a CoAo(15, 24-33). Apesar das diferenças quanto ao tipo de morbilidade percutânea (lesão vascular periférica, peutic option for CoA (20, 31-40): balloon angioplasty, stenting, or different surgical techniques. The debate continues since the incidence of mortality and morbidity does not differ significantly between the various techniques. Although there are differences in the type of morbidity arising from percutaneous (peripheral vascular damage, aneurysm, aortic dissection) and surgical treatment (pain, scarring, neurological damage), major complications are rare. The only randomized trial, of 36 patients, reported similar efficacy for balloon angioplasty and surgery (41). A review of all recent publications (633 patients treated by balloon angioplasty, with or without stenting, and 213 by surgery) also showed similar immediate success rates, and lower morbidity in the group with stent implantation, followed by the surgical group (odds ratio [OR] 1.3) and balloon angioplasty (OR 2.4) (20). The results of a recent international prospective registry also demonstrated similar relief of obstruction, significantly shorter hospital stay in the percutaneous group, and no significant difference in periprocedural morbidity between balloon angioplasty (14%), stenting (13%) and surgery (7%) (37). In the light of these results and our own experience, our current approach is normally surgical in younger patients and in those with hypoplasia of the aortic arch and/or patent ductus arteriosus, and percutaneous in older children and adolescents. Follow-up Following the development of surgical repair of CoA, more and more patients are reaching adulthood. However, there is a significant incidence of late mortality, resulting in a significantly lower mean life expectancy than that of the general population, with 7280% of patients surviving 30-50 years after surgery (19, 42-46). There was no late mortality in our sample, but the follow-up was not as long as in some of the earlier series and did not take account of patients lost to follow-up. In 70% of cases, mortality is due to cardiovascular complications such as recoarctation, hypertension, premature atherosclerotic coronary artery disease and stroke (6). O único estudo aleatorizado (36 doentes) mostrou eficácia semelhante entre a angioplastia com balão e cirurgia (34). Uma revisão de todas as publicações recentes (633 doentes tratados por balão ± stent versus. 213 por cirurgia) mostrou igualmente sucesso imediato sobreponível e morbilidade mais baixa no grupo com stent, seguida do grupo cirúrgico (OR 1.3) e da angioplastia com balão (OR 2.4) (15). Os resultados de um registo prospectivo internacional recentemente apresentados mostraram igualmente um alívio da obstrução sobreponível, tempo de internamento significativamente mais curto do grupo percutâneo e ausência de diferença significativa entre a morbilidade peri-procedimento de angioplastica com balão (14%), stent (13%) e cirurgia (7%) (30). Em face da destes resultados e da nossa experiência, a nossa abordagem actual é habitualmente cirúrgica nos doentes mais novos, com hipoplasia do arco e/ou presença de canal arterial e percutâneas nas crianças e adolescentes. Seguimento Desde que a reparação cirúrgica da CoAo foi desenvolvida, tem-se assistido a um número crescente de doentes que atingem a idade adulta. Contudo, verifica-se uma incidência importante de mortalidade tardia, que condiciona uma esperança média de vida significativamente inferior à da população em geral, com sobrevida de 72 a 80% doentes, 30 a 50 anos após cirurgia (14, 35-38). Na nossa amostra, não houve registo de mortalidade tardia, o que resulta do tempo de seguimento não ser tão prolongado como nas séries mais antigas e um contributo eventual não contabilizado de doentes perdidos para o seguimento. A mortalidade é causada, em mais de 70% dos casos, por complicações cardiovasculares como reCoAo, HTA, doença aterosclerótica coronária prematura e acidentes cérebro-vasculares (5). Neste contexto, o seguimento cuidadoso a longo prazo assume uma importância central, na identificação e manejo dos factores de risco cardiovasculares. term follow-up is therefore essential in order to identify and manage cardiovascular risk factors. Recoarctation is a known complication during follow-up of patients initially treated successfully. Its prevalence varies according to age at intervention, surgical or percutaneous technique employed, geometry of the aortic arch and length of follow-up. The risk of recoarctation after surgery is between 0 and 26%, and is higher in newborns and infants and lower in more recent series, but varies depending on the surgical technique(17-21, 43-48). After the first year of life, its incidence following percutaneous treatment ranges between 0 and 27% (20, 26, 49, 50) and also depends on the technique employed (balloon angioplasty or stenting). In our sample, the overall recoarctation rate (6.3%) was similar to recent studies, for both surgical and percutaneous interventions. Hypertension requiring medication was found in 20% of our patients. Hypertension is a major concern during the long-term followup of patients with CoA. Even after complete and permanent elimination of the gradient across the stenosis, the prevalence of hypertension at rest in treated patients is 15 to 50%, whether treatment is percutaneous(28, 37, 49-51) or surgical (6, 52-55). Among the suggested mechanisms are congenital or acquired structural alterations of the aorta that result in decreased elasticity or reduced lumen, loss of the baroreceptor reflex and changes in the reninangiotensin system (6, 53). A combination of mechanical, neurological and hormonal damage may be the cause of such hypertension. The most important factor contributing to hypertension is late repair (43, 56-58), also seen in our sample. This highlights the importance of early diagnosis and treatment of CoA. The absence of hypertension or “recoarctation” at rest does not mean they may not occur during exercise. Almost half of the patients in our sample had abnormal blood pressure responses or pressure gradients during effort. Such findings have been reported in various series, with incidences of 10 to 57% (59-67). A sua prevalência varia com a idade do doente, técnica terapêutica, geometria do arco aórtico e tempo de seguimento. O risco de reCoAo após cirurgia (12-16, 35-38) ou cateterismo (15, 20, 39, 40) é sobreponível e situa-se entre 0 e 26%, sendo mais elevado nos recém-nascidos e lactentes, mais baixo nas séries mais recentes e variável com a técnica terapêutica e depende da técnica utilizada. Na nossa amostra, encontrámos uma taxa de reCoAo (6.3%) sobreponível aos estudos mais recentes, quer no que diz respeito às intervenções cirúrgicas quer nas percutâneas. A HTA, presente em 20% dos nossos doentes, é uma preocupação importante do seguimento a longo prazo. Apesar da resolução da estenose, a sua prevalência em repouso em doentes tratados é de 15 a 50%, independentemente da técnica utilizada, percutânea ( 22, 30, 39-41) ou cirúrgica (5, 42-45). Entre os mecanismos postulados, encontram-se a diminuição da elasticidade da aorta, a perda do reflexo baro-receptor ou alterações do sistema reninaangiotensina (5, 43). O factor mais importante a contribuir para esta incidência é a correcção tardia (46-48), algo verificado na nossa amostra. A ausência de HTA ou reCoAo em repouso não exclui o seu aparecimento com o exercício físico, algo verificado em quase metade da nossa amostra. Estes achados encontram-se descritos em várias séries, com incidências de 10 a 57% (49-53). Embora o significado clínico destes achados em doentes com CoAo não esteja esclarecido (53, 54), é sabido que a resposta tensional anómala ao esforço em indivíduos saudáveis está associada ao aparecimento subsequente de hipertensão arterial em repouso (55). CONCLUSÕES 32 A CoAo isolada é uma patologia de excelente prognóstico a curto prazo, que mantém uma incidência significativa de complicações a longo prazo. Although the clinical significance of such findings in patients with corrected CoA remains unclear (67, 68), abnormal blood pressure responses during exercise in healthy individuals are associated with subsequent development of hypertension at rest (69). CONCLUSIONS Isolated CoA has an excellent short-term prognosis but a significant incidence of longterm complications, and should thus no longer be seen as a simple obstruction in the descending aorta, but rather as a complex pathology that gives rise to a complicated cardiovascular syndrome and requires careful follow-up after treatment. The variability of symptoms and occasionally insidious presentation require a high level of clinical suspicion, showing the importance of a careful and thorough physical examination of newborns and older children, particularly femoral pulse palpation. Delay in diagnosis and treatment has a significant impact on late morbidity and mortality. Taking into account the data currently available on late and immediate results, the final choice of therapeutic technique depends on the patient’s age, associated lesions and the experience of the medical-surgical team. Hypertension must be closely monitored in the follow-up of these patients, as well as its risk factors and complications. Ana Peres, et al.
