EQUICARE CS Cancer Navigation and Survivorship EQUICARE CS is a web-enabled solution that creates a dynamic link between oncology providers, survivors, and their caregivers with the aim of improving post-treatment care and support. The Oncology Patient’s Journey From Diagnosis To Survivorship* Diagnosis Tre atment Beyond active treatment A powerful combination Varian Medical Systems, in partnership with Equicare Health, Inc., offers EQUICARE CS™ survivorship software to help cancer centers provide targeted and tightly-managed aftercare for cancer survivors. EQUICARE CS uses a rich set of interfaces to transfer patient information directly from your ARIA® oncology information system – creating a natural extension of the patient’s electronic medical record (EMR). EQUICARE CS includes: • A clinical knowledge base that models industry guidelines (e.g. ASCO and NCCN) for cancer survivorship In addition to radiation, chemotherapy and hormone therapy treatment summaries, ARIA automatically transfers the following information to EQUICARE CS: • An oncology-specific library of educational materials to help survivors gain a better understanding of the characteristics of their disease • A Meaningful-Use-certified patient portal that provides a reliable link between each survivor and the care team • Customizable questionnaires enabling you to document the long term health and wellness of survivors Lab results • Demographics • Transplant history • Family history • Medical notes • Continuity of Care Documents (CCDs) • *NCI defines survivorship in this context to mean the period from which an individual is initially diagnosed with cancer through the balance of his or her life. Su r v i v a l EQUICARE CS Highlights Post-treatment, EQUICARE CS becomes the personalized gateway to clinical information for patients, their families, and referring physicians. This communication link can serve as a surveillance channel for tracking secondary cancers or any late effects to ensure timely, appropriate treatment. • mooth the patient’s transition from active treatment to S survivorship by enabling the individual to play an active role in their recovery. Knowing what to expect after treatment may help to reduce anxiety patients may feel as they embark on the next phase of their lives. By incorporating EQUICARE CS into your survivorship program, you can: • Create structured, long-term survivorship care plans as recommended by the American College of Surgeons “Commission on Cancer”i and the Institute of Medicineii . The electronic transfer of information between ARIA and EQUICARE CS means care plans can be completed in minutes, not hours. • Conduct population-based research based on data collected in survivor questionnaires. The aggregation of this valuable information can help you measure long term clinical outcomes. • Retain patients in your care network by maintaining an active connection with survivors for follow-up procedures and services which may be needed, such as lab tests and imaging studies. • Routinely monitor patient symptoms to increase the likelihood of detecting disease recurrence or side effects in a timely manner. The software can also help facilitate better patient compliance with clinical instructions such as medication administration. How are you positioned for CoC accreditation? The 1,500 cancer facilities in the U.S. that are accredited by the American College of Surgeons Commission on Cancer (CoC) must have a formal plan for the delivery of survivorship care plans by 2014 and they must implement that plan by 2015 in order to maintain their accreditation. EQUICARE CS meets the requirements today and has been included in the CoC’s “Best Practice Repository.” EQUICARE CS also supports the standards the CoC has set forth for Psychosocial Distress Screening and Patient Navigation. Navigation and Psychosocial Distress Screening Secure behind your firewall EQUICARE CS is a turnkey application that runs on commodity servers. Proper use of the software can help you comply with regulations protecting personal health information. Security features include: • Individual accounts for patients and approved care providers with secure web access via https • Secure patient sign-up through use of a delivered pass code and up to two personal identification numbers • Encrypted passwords with configurable rules for password length, characters, and expiration To assist cancer facilities in meeting the Commission on Cancer’s accreditation standards, Equicare Health, Inc., is enhancing EQUICARE CS to support other Continuum of Care Services. EQUICARE CS provides clinicians with helpful tools to identify and track the causes of a patient’s distress in the context of support for Navigation.
