Alert for Zomig-ZMT®:
As of August 1, 2013, Zomig-ZMT® orally disintegrating tablets and Zomig® tablets will no longer be part of the Plan’s preferred drug list
(Express Scripts’ formulary), due to the availability of a generic equivalent. As a result, this medication will be more expensive. If you
continue to purchase this medication on or after August 1, 2013 you will pay a higher non-preferred brand name copayment, subject to the
below-mentioned ancillary charge if you request the brand name.
Below is an illustration of the cost difference in the brand name non-formulary and the lower cost generic alternative:
