INTRODUCTION
WorkSafe Victoria
WorkSafe Victoria works with the community to deliver outstanding workplace
safety, together with quality care and insurance protection to employers and
workers.
This Booklet
VCode is WorkSafe’s system for classifying the nature of injury/disease
associated with workers’ compensation claims in Victoria. This booklet clearly
sets out the codes required to be entered into ACCtion for each claim. These
codes have been applied to all claims (including existing ongoing claims) from 1
July 2002.

CODING GUIDELINES FOR VCODE

These guidelines will assist coders/claims managers in classifying, or determining the correct nature
of injury/disease code, for a workers’ compensation claim.
Nature of Injury/Disease
• Use of the Index to Nature of Injury/Disease (see page 33) will provide the correct code quickly and
easily when coders are unfamiliar with the VCode system.
• This Index will identify the probable classification. Always confirm the classification by referring to
the ‘includes’ and ‘excludes’ of the classification as well as related areas.
• Sprains of joints and strains of muscles are only relevant to traumatic events (see “Traumatic
joint/ligament and muscle/tendon injury” on page 12).
Certificate of Capacity
If a Certificate of Capacity is received with the claim form, it must be used as the basis for assigning a
nature of injury code.
Coding Guidelines
• There are two boxes on the Certificate of Capacity; one box is for the description of the
injury/condition, the other is for the diagnosis. The preferred source of information for classifying the
nature of the injury/disease is the first injury mentioned in the diagnosis section of the Certificate of
Capacity. However, it may be necessary to use information in the description box as well as the
diagnosis box to determine the code.
• Coders should look for the required information in the box marked ‘diagnosis’ on the certificate (and
the description box if necessary) and then look for the named injury/disease in the Nature of
Injury/Disease Index. If a new term, which is not listed in the Index, is encountered, consult an
authoritative medical dictionary for clarification or contact the issuing doctor for advice.
• Each and every valid Certificate of Capacity must be individually classified and recorded on the
computer system. This allows for the nature of the injury/disease to be monitored over time by
WorkSafe. It is common for a diagnosis to be refined as test results and further information become
available to the treating doctor. Problems that were initially secondary to the primary problem may
come to the fore. If this occurs, DO NOT update the original classification code unless clerical error
is detected.
• Certificates of Capacity that are blank, marked ‘As previously documented’ or illegible, cannot be
coded.
No Initial Certificate of Capacity
Valid claims may arrive without an attached Certificate of Capacity. If the claim does not require a
Certificate of Capacity for registration, the following guidelines should be adopted using information
from the worker’s claim form.
Coding Guidelines
• If the worker states a single nature of injury/disease, classify the claim to that injury/disease.
• Workers often leave the nature of injury/disease section blank and just state a body part. If there is
no evidence of trauma (like a fall or cut), classify the symptoms to the most relevant
musculoskeletal disorder. For example, “Back”, “Back Injury” or “Sore back” should be coded to 459
Back pain, strain (non traumatic), lumbago, sciatica.
• Descriptive terms such as “bad”, “hurts”, “sore” and “painful”, without evidence of trauma, are all
taken to indicate a musculoskeletal disorder.
• Workers often list many body parts/injuries on the form which makes it difficult to determine which
one is the most serious. If there is no evidence of trauma or similar (e.g. a fall or cut), classify the
claim to 599 Diseases of the musculoskeletal system and connective tissue, nec.

Conditions, Diseases and Disorders
Diseases/conditions/disorders usually results from repeated or long-term exposure to an agent or
event, for example, a musculoskeletal condition may develop through long term exposure to
stressing of the body.
Some conditions may be the result of a single traumatic event. For example, a worker robbed at
gunpoint may develop a mental disorder, or a meat worker may suffer a single exposure to
leptospirosis.
An injury or poisoning may develop into a disease/condition/disorder. For example, a worker who
suffers a poisoning may go on to suffer liver failure and a traumatic joint/ligament or muscle
tendon injury may develop into a musculoskeletal disorder. When coding claims for which the
condition has developed and changed over time, always code the claim as it is NOW.

Diseases/conditions/disorders usually results from repeated or long-term exposure to an agent or

Classifying the Bodily Location of Injury/Disease
Once you have classified the primary Nature of Injury/Disease it is relatively straight forward to
classify the bodily location of the Injury/Disease.
The doctor completing the Certificate of Capacity will often state the location with words like back,
shoulder or elbow or use dialect like C3-C4 (parts of the spine). Occasionally the doctor will use a
technical term. Always look these up in a respected medical dictionary.
For some injuries, especially those resulting from motor vehicle accidents, there is no identifiable
primary injury and the doctor may note multiple injuries or fractures. Code these to the
appropriate multiple location code.
Amputations are to be coded to the site of amputation, not to the parts of the body amputated.
Exceptions are: ears, eyes and nose, fingers, toes, genitalia, breasts and foetus which should be
coded to the part lost.

The mechanism of injury/disease is most easily thought of in terms of an action, exposure or
event. Different types of mechanisms are actions, such as being struck by, or striking against, an
object, or lifting, handling or carrying objects. Other mechanisms can be exposures, such as to a
virus, environmental factors, mental stress or specific events such as motor vehicle accidents and
cave-ins. The mechanism that should be identified is the overall action, exposure or event
which resulted in the most serious injury or disease.

The Agency of Injury/Disease classification covers two concepts:
• The BREAKDOWN AGENCY concerns the object, substance, or circumstance that was
principally involved in, or most closely associated with, the breakdown event, and
• The AGENCY which concerns the object, substance, or circumstance which was the direct
cause of the most serious injury or disease.
