Heart failure is a growing problem that affects about 5 million U.S. adults.
The UAB Advanced Heart Failure Program offers comprehensive, specialized
treatment options for end-stage heart disease.

State-of-the-art treatment from the UAB Advanced Heart Failure Program
gives patients a chance for improved survival and quality of life. Treatments
available at UAB include cardiac transplantation, mechanical circulatory

The UAB Advanced Heart Failure Program team is comprised of nationally
ranked, specialized physicians and nurses who work together to provide
comprehensive patient care.

Available treatments address risk factors for heart failure and include medical
therapy, pacemakers or defibrillators, ventricular assist devices and heart
transplantation. UAB’s success in the diagnosis and treatment of advanced
heart failure makes it a premier choice for patients needing specialized,
multi-disciplinary care.
Services Offered
• Heart and heart-lung transplantation
• Mechanical circulatory support devices for patients who are symptomatic
despite medical treatment or have recurrent hospital admissions
• Medical management of heart failure, including treatments for advanced
heart failure, as well as new and investigational therapies
• Evaluation and management of diastolic heart failure, including pericardial
diseases
• Placement of a pulmonary artery sensor to monitor volume status
• Evaluation and management of pulmonary hypertension
• Management of cardiomyopathies and other rare cardiac conditions (for
example, cardiac amyloidosis or Fabry’s disease)
• Right heart catheterization and hemodynamic assessment

Cardiologists in the UAB Advanced Heart Failure Program work closely with
cardiac surgeons to treat patients who fail medical management of their
conditions. Surgical treatments include implantation of VADs and heart
transplantation.

VADs are used in patients who need a mechanical assist pump to support
proper heart function. These devices are used to bridge patients to another
therapy, such as a bridge to heart transplantation, or as a permanent or
“destination” therapy as an alternative to transplantation. Sometimes a VAD is
used temporarily to help a patient’s heart regain sufficient strength and return
to normal function.
UAB’s Mechanical Circulatory Support Device Program implants more VADs
annually than other regional facilities and is the only program of its kind
in Alabama. The UAB Mechanical Circulatory Support Device Program is
Medicare approved and has the Joint Commission’s Gold Seal of Approval™
for meeting required standards and performance measures.

UAB’s Heart Transplant Service is one of the top international centers in
the management of advanced, end-stage heart failure. It has accumulated
extensive experience in the management of heart transplantation with more
than 1,000 patients transplanted in the past 25 years. The transplant
team is made up of experienced, specialized cardiologists, surgeons and
nurses who are experts in providing care from initial evaluation to long-term
follow up of transplant patients and their families.

Selection Criteria
Patients who may benefit from consultation with the Advanced Heart Failure
Program include those with:
• Symptoms of heart failure despite usual medical therapy
• .Frequent hospitalizations for decompensated heart failure
• Impaired quality of life due to heart failure
• .Evidence of end-organ involvement like renal failure or liver congestion
• End-stage coronary disease not eligible for revascularization
• Ventricular tachycardia and implantable cardioverter defibrillator shocks,
unresponsive to therapy
