Approved Treatment Required for Bortezomib for Relapsed/Refractory Mantle Cell
Lymphoma – 2nd or subsequent line

All 4 conditions must be met
1. Application made by and first cycle of systemic anti-cancer therapy to be
prescribed by a consultant specialist specifically trained and accredited in the
use of systemic anti-cancer therapy
2. Pathologically confirmed mantle cell lymphoma
3. Relapsed disease after one or more prior chemotherapies (including
Rituximab), or autologous stem cell transplantation
4. To be used within the treating Trust’s governance framework, as Bortezomib is
not licensed in this indication
