● Questions applicable to each care setting that are easy to
understand and to complete, providing the opportunity for
patients and carers to provide feedback on care and treatment
across all contact points.
● Questions devised to address some of the key indicators and outcomes of the
Vanguard, such as:
○ Respect for patient-centred values (including dignity, quality of life, shared
decisions)
○ Information and communication (on care and treatment and the involvement
of family and friends)
○ Coordination and integration of care across and within services
○ Access to care, in particular the timeliness of access to services
○ Demographics to enrich data analysis.
