Medicines Management Achievements in the last 12 months Prescribing • Agreed formal service specifications with providers. • Maintained and enhanced communication with Thurrock CCG prescribers in order to drive the prescribing agenda and promote high quality, safe, evidencebased and cost effective prescribing. • Achieved a high level of local engagement with ScriptSwitch (a new prescribing system) which delivered savings significantly higher than the national average. Primary Care • Diabetes and Respiratory Networks have agreed health economy-wide policies for high impact Primary Care drugs. • Updated guidelines around Oral Nutritional Supplements and produced significant savings around Home Enteral Feeding. • Achieved the targets for Medicines Management QIPP for 2014/15. Acute Trust • Worked closely with Basildon Hospital to update the local Primary Care guidance for antibiotic prescribing. • Supported the development of new, more efficient pathways for Deep Vein Thrombosis and Atrial Fibrillation. Community • Worked closely with providers to develop a wound-care formulary which is due to be launched in south west Essex in September 2015. • Worked closely with providers to develop and launch an End of Life Formulary. Medicines Management Commissioning Intentions/Forward Plans • Development and/or update of formulary for other high impact areas such as insulins, emollients etc. • Develop and implement a strategy for antibiotics in line with national recommendations to reduce antimicrobial resistance. • Scope transformation projects for stoma products and incontinence products as part of the QIPP agenda. • Review processes for Basildon Hospital Medicines Management Committee and CCG Medicines and Safety Group. • Ensure delivery of High Cost drug challenges with acute trust. • When reviewing service specifications, or formulating new medicines etc ensure financial implications and clinical responsibilities are considered.
