In total IUSO currently has 14 residencies with 19 resident positions. IUSO has five internal residencies, (Cornea/Contact Lenses, Ocular Disease, Pediatrics, two residencies in Primary Care). IUSO has external
residencies in Primary Care and Ocular Disease. We are in three VA hospitals, Huntington VA, Huntington, WV, Illiana VA, Danville, IL, and Lexington VA, Lexington, KY. We have two residencies in major medical
centers, Gundersen Health Care System in La Crosse, WI, and University of Kentucky Department of Ophthalmology, Lexington, KY. We also have three private practice residencies; Bennet and Bloom Eye Center,
Louisville, KY, John Kenyon American Eye Institute, New Albany, IN and Ossip Optometry and Ophthalmology, Westfield, IN.

The Michigan College of Optometry has 2 in house residency positions: 1 in Cornea and Contact Lenses and 1 in Pediatrics and Binocular Vision. There are an additional 7 affiliated sites which offer 10 positions: Ocular
disease sites at Battle Creek VAMC (2 positions), Wyoming, MI VA (1 position), Detroit VAMC (2 positions), The Laser Center, Jackson, MI (1 position); Ocular disease/Primary Care at Fort Wayne VAMC (2 positions);
Primary Care at Henry Ford OptimEyes, West Bloomfield, MI (1 position); Community Health/Primary Care at Cherry Health, Heart of the City Health Center (1 position, accepting their first resident for 2017-2018).

MCPHS will begin one residency program in July 2017. The position will be in Community Health Optometry. The resident will see patients primarily at Family Health Center of Worcester, but will rotate through local
Ophthalmology practices to observe surgical procedures and manage pre and post operative surgical patients.

There are 15 residency programs offering a total of 38 positions. They are based at the New England College of Optometry Center for Eye Care, VA and non-VA hospitals and Outpatient Centers throughout New England
and in Boston area community health centers.

The NOVA program has a total of 35 positions. NOVA has nine (9) in-house residency positions including one (1) full pediatric and binocular vision position and eight (8) primary care positions which allow an for area of
emphasis in either pediatrics/binocular vision, cornea/contact lenses, ocular disease, or low vision. In addition, NOVA sponsors eleven (11) external residency sites with twenty six (26) positions located within both
private practices and veteran’s hospitals throughout Florida and Atlanta, GA.

NSUOCO has 23 affiliated residencies including Family Practice, Cornea and Contact Lenses, Ocular Disease, Refractive Surgery and Primary Eye Care. Each residency has a strong clinical emphasis and is well rounded
with didactic and scholarly activities. All residencies have strong support from both the affiliate and the sponsor. Clinical training includes hands-on experience in ocular disease management, laser surgical procedures,
optometric surgical procedures, primary care and other advanced clinical procedures. Residency sponsors include the Oklahoma College of Optometry, tertiary care referral centers, VAMC and the US Army. The US
Army provides two Optometric Management Education programs, one which combines a Family Practice/Ocular Disease residency and Master of Business Administration degree and the second which combines a
Primary Care Residency and Masters in Hospital Administration degree.

Eye Associates of Mexico: Residency in Primary Care and Ocular Disease. Will be participating in a site visit in the spring of 2017. Fort Defiance IHS: Residency in Ocular Disease. An affiliation has been established with
this site and the resident is expected be in place August 1, 2017.
The Primary Eye Care Residency Program at IAUPR provides post-graduate training to one resident per year to develop proficiency in areas of primary care optometry, gain experience with multidisciplinary settings
including pre- and post -ophthalmic operative care and rotation in the following specialty areas: Contact lenses, VT, Low Vision, Electro-diagnostics, Prosthesis, Learning Disabilities, Ocular Disease, Ocular Disease comanagement. The program also provides the resident with experience in teaching, research and scholarly activities. For the school year 2016-2017, the selection process of the resident is expected to be concluded at
the end of October 2016.

SCCO at MBKU is affiliated with 24 residency programs / 48 residency positions across 6 different states. We offer residencies in Primary Eye Care, Cornea and Contact Lenses, Geriatric Optometry, Pediatric
Optometry, Low Vision Rehabilitation, Vision Therapy and Rehabilitation, Ocular Disease, Brain Injury Vision Rehabilitation. Our on-campus residencies include the Cornea and Contact Lenses and the Pediatric
Optometry and Vision Therapy programs. We have a unique affiliation with 8 Indian Health Service and 9 Veteran Administration residency programs.

SCO has 32 residency programs with space for 54 residents. We have five on-campus residencies that accommodate 7 candidates with intensive, state of the art training in Cornea, Contact Lens and Refractive Surgery,
Primary Eye Care, Pediatrics and Vision Therapy, Low Vision and Vision Rehabilitation and a specialized two year program in Primary Eye Care and a Masters of Business Administration degree through Christian
Brothers University. We have 23 residency slots at our 8 VA affiliated sites with emphasis on Primary Eye Care/Ocular Disease and Low Vision/Ocular Disease. The remaining 19 programs offering 24 residency positions
are at private clinics and hospitals and offer advanced clinical training in Pediatrics, Vision Therapy and Rehabilitation, Ocular Disease, Refractive and Ocular Surgery and Family Practice.

The residency programs affiliated with and at SUNY, State College of Optometry include programs in the following areas: Cornea and Contact Lenses, Low Vision Rehabilitation, Ocular Disease, Family Practice
Optometry, Primary Eye Care, Pediatric Optometry, Vision Rehabilitation (Neuro-Optometric Rehabilitation) and Vision Therapy and Rehabilitation. There are 9 residency programs (17 residents) sponsored by SUNY
and 7 programs (21 residents) sponsored by affiliated sites. All programs affiliated with or at SUNY are fully accredited or pending accredition. Along with the clinical component of the residency, there are formal
didactic activities which are an integral part of the residency program.

Consistent with its educational mission, the University of Alabama at Birmingham School of Optometry (UABSO) places significant emphasis on residency education. UABSO strives to develop, maintain, advance, and
promote post-doctoral clinical training programs of excellence in the major areas of optometric care. Currently, seven residency programs offer a total of thirteen residency positions. The in-house residency programs
at UABSO include Family Practice Optometry, Cornea & Contact Lenses, and Pediatric Optometry. Our affiliated programs offer advanced clinical training in the areas of Geriatric and Low Vision Rehabilitative
Optometry, Hospital-Based/Primary Care Optometry, and Ocular Disease. All UABSO residency programs are accredited by the Accreditation Council on Optometric Education and all utilize the ORMatch residency
matching service for application.
