STUDY INFORMATION DOCUMENT
I am Tracy Zhandire and I am undertaking a research project as part of the Masters in
Medical Informatics in the Department of Tele-Health at the University of KwaZuluNatal. I am conducting a study on Knowledge, attitude and perception of 4th and 5th
year students from UKZN Medical School towards the use of HIV drug resistance
interpretation algorithms.
The study involves completing a questionnaire. Your participation in the study is
voluntary and you may choose not to participate. Failure to participate will have no
negative consequences for you and will not affect your studies or result in any
sanction. You will be free to withdraw from the study at any time and you may
decline to answer any or some of the questions as you wish.
No personal information will be shared with anyone. Your name is not requested in
the survey. No information will be used that could identify you in any report or
publication resulting from this study.
You have been informed about the study by Tracy Zhandire.
You may contact Prof M Mars on 0312604543 at any time if you have questions about
the research.
If you agree to participate, you will be given a signed copy of this document and the
participant information sheet which is a written summary of the research.

My name is Tracy Zhandire, I am a student at the University of KwaZuluNatal (UKZN). I am
doing Masters of Medical Science in Medical Informatics in the School of Nursing and Public
Health. As part of my research work, I am conducting a survey on the Knowledge, Attitudes and
perception of 4th and 5th year students from UKZN Medical School towards the use of HIV drug
resistance interpretation algorithms. Please do not write your name on the questionnaire to ensure
