OCCG Weekly Bulletin Wednesday 18 May 2016 Reminder of Information Required for MSK Referrals REMINDER – Weight Management Referrals Guidance for Doctors Treating Transgender Patients Information for Practices Reminder of Information Required for MSK Referrals: The MSK Hub needs to ensure that a minimum set of data is available for every patient referred into their service to facilitate efficient assessment, triage and treatment. It is important that the clinician includes the following information on all referrals: o o o o o o o o o o o o Presenting complaint Pertinent clinical information including indications, pertinent history, results of any diagnostic investigations and provisional diagnosis if available. Any relevant disease scores or patient related outcome scores Current symptoms including duration. Details of any previous treatment including medications given to the NHS Patient for the condition. Relevant past medical history. Details of current medications and any other known allergies (e.g. allergies to intravenous contrast). Details of any matter which the referrer considers could affect the nature of the service provided including social and local authority services. Any special needs (e.g. interpreter required, disabilities requiring special handling, carer support). Body Mass Index Weight loss advice already given and any programmes attended Referrals to smoking cessation if appropriate This minimum data set is included in the existing MSK Hub referral pro-forma. Referring GPs do not need to provide an Oxford Hip or Knee score. The surgeon would be expected to do that pre and post operatively as this is an essential part of the National Joint registry. REMINDER – Weight Management Referrals: The Morelife weight management service is commissioned by Oxfordshire County Council’s Public Health Directorate. The Morelife weight management referral hub takes referrals from GPs for patients who have a BMI of 30 or more, for either 12 weeks of Slimming World of Weight Watchers or a BMI of more than 40 for the Morelife 12 month programme. MoreLife is a 12 month evidence based weight management programme that is delivered throughout Oxfordshire by weight management practitioners. MoreLife is a psychologically focussed intervention, exploring the relationship between emotions and thoughts and how they link to eating behaviours. The goal of the service is for patients to achieve both moderate and sustainable weight loss along with long-term weight maintenance. Guidance for Doctors Treating Transgender Patients: NHS England has published information to help doctors respond to the needs of transgender patients. Ensuring that people who identify themselves as transgender receive appropriate health care is part of the NHS Public Sector Duty of the Equality Act 2010.
