After many years of development and innovation this year we presented the Carriere Class III Motion appliance at the 2015 AAO meeting in San Francisco. The appliance changes the relation in which the mandible interacts with the maxilla harmonizing soft tissues and balancing the face of the patient. The appliance results in a harmonious facial profile with a minimally invasive treatment of Class III cases. We have created a design that is very clean and simple. It is intended to treat Class III malocclusions without extractions, painful orthodontic surgery or traditionally uncomfortable and obtrusive facemasks. The Motion Class III changes the occlusal planes and intrudes the lower molars. It also extrudes the canines. Therefore, in some part, changing the posterior occlusal planes brings it into a better functional position; while balancing the mandible and the maxilla helps to create Facial, Skeletal and Dental Harmony in our Class II and Class III cases. The practitioner is responsible for facially driven orthodontics for the patient. It is my goal as an orthodontist to be an expert in repositioning of teeth and bones in the correct position and balancing the patient’s profile. This year he received the Ontario Dental Association Merit Award for his work with his local dental society. 8 Class II Division 2 occlusion characterized by a retrognathic mandible and excessive overbite presents as a challenge for orthodontic treatment. Some orthodontists argue that by simply correcting the angulation of the maxillary central incisors the mandible will posture forward automatically correcting the class II malocclusion. However, in my experience, this very rarely if at all ever happens. After correction of the maxillary incisor angulation, the patient’s occlusion becomes class II division 1 and the clinician now has to correct the underlying mandibular retrognathic skeletal relationship. Treatment with the Carriere® Motion™ in conjunction with a mandibular lingual arch allows for simultaneous improvement in the class II molar relationship with improvement in both the overbite and the angulation of the maxillary central incisors. retrognathic mandible. Composite bite raisers were placed on the occlusal of the mandibular first molars to prevent the molar tubes from breaking off the bands of the lingual arch. Following several years in private practice, he pursued further training in Orthodontics and obtained his Masters degree from the University of Alberta in 2009. Dr. Sharaf is a fellow of the Royal College of Dentists of Canada, and a Diplomate of the American Board of Orthodontics. Dr. Sharaf is also a member of the Cleft Lip and Palate Clinic at the University of Alberta’s Stollery Children’s Hospital. He maintains a full-time practice at Horizon Orthodontics. 10 Blocked-out maxillary canines impose a challenge to the orthodontist as they erupt into inadequate space. The option to extract a maxillary premolar to address this unfortunate situation could be eliminated with successful molar distalization. The Carriere® Motion™ Appliance is a suitable option to achieve this orthodontic objective, especially if used during the mixed dentition stage, and prior to the eruption of the maxillary second molars. The following case was treated with the Carriere Motion Appliance and a mandibular first molar bracket (with hook) bonded in conjunction with an Essix Retainer. The following steps and progress took place over 6 months: INITIAL The Carriere Motion Appliance was bonded from teeth 14 to 16 and 23 to 26 respectively. Patient was 10 years 7 months at the time the Carriere Motion Appliance was bonded. The initial elastic protocol was full-time wear of 2.5 oz 1/4” elastics for the first month. PROGRESS Patient was instructed to increase the strength of the elastics on the right side to 4.5 oz 1/4” elastics (5 months) to achieve further distalization of teeth 14 to 16. Class I canine relationship was achieved with the distalization of teeth 23 to 26. FINAL Effective elastics wear allowed for the creation of adequate space for the eruption of tooth 13 into the maxillary arch and the correction of the left canine relationship to class I. The results were obtained within 6 months, paving the way for a predictable phase II treatment for a class I crowded platform. Dugoni School of Dentistry and earned his post-doctoral Master’s degree and certificate in orthodontics from the University of Colorado. He is a member of the Tau Kappa Omega Dental Honor Society. Dr. Frey is Board Certified in Orthodontics, is a Top 1% provider of Invisalign, and has earned fellowships from the World Congress of Minimally Invasive Dentistry and the American Academy of Facial Esthetics for his achievements in private practice. He maintained a private practice exclusive to cosmetic and therapeutic injectables during his orthodontic residency and is a national expert and lecturer on esthetic orthodontics, facial maturation, and injectable pharmacologics. Dr. Frey has pioneered the development of the Soft Tissue Orthodontics curriculum for Henry Schein Orthodontics, is a published scientific author, and is a reviewer for the Journal of Aesthetic Plastic Surgery. 12 The modern orthodontic practice is being asked to deliver more for our patients in less time than we ever have before. The Carriere® Motion™ appliance has been extremely effective in addressing this need for our practice. By initiating AP correction at the outset of treatment, when compliance is at its highest, we are able to provide excellent orthodontic outcomes while at the same time dramatically reducing the total time spent in full fixed appliances. Being able to deliver great results faster, allows us to command a higher fee for treatment, start more patients, and keeps smiles healthy by reducing cariogenic strain to the dentition. Bilateral Class II malocclusion with deep overbite. Mandibular retrusion due to radiographically evident condylar resorption. Reduced width of attached gingiva in lower anterior. Excessive buccal corridors & consonant smile arc. Paranasal deficiency and deficient Lower Esthetic Contour™. TREATMENT PLAN TREATMENT MECHANICS Maxillary arch development was initiated at the outset of treatment to address the transverse deficiency and prevent the development of a crossbite during AP correction. A Transforce expander was utilized for 2.5mo to achieve the desired arch width. Following Transforce removal, a Class II Motion appliance and full lower braces were placed; 3/16 2oz elastics were started. At 6 weeks, elastics were increased to 1/4in 4oz upon inserting rectangular CuNiTi archwires in the lower arch. At 12 weeks, elastics were again increased to 1/4in 6oz and final archwires were inserted in the lower. A class I platform was achieved after 20 weeks with the motion appliance and upper fixed appliances were bonded to close spaces and reduce remaining overjet. To hold the AP correction, 1/4in 4oz elastics were worn to bed as needed. During space closure, reverse curve of spee mechanics we used in the lower arch to reduce overbite and treatment was completed in 20 months, with only 13 months in full fixed appliances. With the proper wearing of rubber bands, this appliance will keep the patient from having to wear headgear or other bulky appliances later in the treatment. The Carriere Motion Appliance is able to handle normal chewing of food and drink but, like all appliances, if proper care is not taken then the appliance may become detached from the teeth. Activation and Reactivation Only Class II elastics needed for activation. Reactivation is required to maintain force levels and facilitate continued advancement. Staff Training Required Orthodontic staff can be quickly trained on placement of the appliance. System requires extensive, detailed training; a course is recommended. Patient Compliance Extremely high, because the appliance is used before bracketing, when patients are eager to start their treatment.
