Lump Sum Cancer Policy
C20000
This policy pays a one time lump sum payment when an insured is first diagnosed with cancer.
Benefit Amounts of $10,000, $25,000 and $50,000 are available. **CAUTION** Please note that
NOT all “cancers” are covered. The following conditions are NOT considered Cancer for the
purposes of this policy:
a) Skin Cancer – basal cell carcinoma (less than 1.0 cm) and squamous cell carcinoma
(less than 1.5 cm)
b) Carcinoma in Situ – earliest stage of a cancer in which it is confined to its site of origin
c) Stage 1 of Hodgkin’s Disease – involvement of a single lymph node region
d) Stage A Prostate Cancer – when the tumor cannot be felt by rectal exam and was
discovered incidentally; usually by an elevated PSA.
e) Melanoma diagnosed as Clark’s Level I (confined to the epidermis) or II (confined to the
upper dermis) or Breslow less than .75 mm in thickness.
The policy pays a benefit only upon the FIRST diagnosis of cancer. There is also a 30 day
Waiting Period. That means no benefit will be payable to anyone who has cancer manifested
before the policy has been in force for 30 days after the issue date.
Benefits reduce by 50% at age 65 and the policy terminates at age 80. However, coverage for a
covered person terminates when a benefit is paid.
Issue ages are 18-64.
The policy may be written on one adult or one adult and spouse. Children’s coverage is also
available. The premium for children covers all children (not per child). The benefit amount of all
covered persons will be the same.
The application is AG-1353. This application is also used for other cancer policies so be sure and
mark “C20” or “C20000” and the benefit amount desired in the plan and units area of the
application. The rate sheet is AG-2445 and the Outline of Coverage is AG-2398. (There are
state variations of each) A sales brochure, AG-2417, is also available. These forms may be
ordered through CAS.
The policy is currently approved in the following states: AL, AK, AZ, AR, CO, DC, FL, HI, ID, IA,
IL, IN, KY, LA, ME, MD, MI, MN. MS, MO, NE, NV, NM, NC, ND, OH, OK, OR, PA, SC, SD, TN,
TX, UT, VT, VI, WA, WV, WY, Canada (except PQ) and NZ..
Not available: CT, GA, MA, NJ, VA
