The William F. Slagle Dental Meeting is one of The University of Tennessee College of
Dentistry’s greatest traditions.
The Slagle Dental Meeting is committed to offering extensive continuing education courses,
commercial exhibits, and additional educational programs advancing the growth and
development of the dental profession. The Planning Committee, UT Dentistry Alumni
Association, and College of Dentistry have worked diligently to present a program that
honors the meeting’s legacy by showcasing the highest caliber continuing education
speakers. Throughout its history, the meeting has provided more than two million dollars
in proceeds directly to the UT College of Dentistry. Your valued participation helps enhance the dental school
experience for current and future students. When I think about the upcoming plans for the college and its dental
students I am proud to be an alumnus of UTHSC College of Dentistry, and I am pleased to serve with this great group
of alumni.
If you have not traveled back to visit in some time, I encourage you to do so by attending the 2017 William F. Slagle
Dental Meeting. Memphis is a wonderful city with a rich history and much to offer. You will see many things that are

Continuing Education Credit
Online registration will be open until, February 21, 2017.
We encourage you to register early and take advantage of
the discounted fee for registration by February 14, 2017.
Advance registration for all programs is strongly encouraged
and appreciated. Check-in for the William F. Slagle Dental
Meeting begins Thursday afternoon at the Hilton Hotel and
will continue all day Friday, Saturday and Sunday morning.
All information, name tags, and meal tickets will be held for
pick-up at the registration desk. Your registration fee covers
your choice of sessions unless otherwise noted, Friday and
Saturday lunch, refreshment breaks, continuing education
credit, and admission to the Welcome Party and exhibit
booths.
The formal continuing education programs of The
University of Tennessee Health Science Center College of
Dentistry are accepted by AGD for Fellowship/Mastership
credit. The current term of acceptance extends from
11/01/2015 through 12/31/2019. Provider ID #146050

Cancellation Policy
In order to ensure adequate arrangements for courses, food,
and other events, no refunds will be given after February
21, 2017. Please submit a written request by email to UT
Conferences ([email protected]). A $30 processing fee
will be charged for each cancellation processed. Refund checks
will be mailed within three weeks of adjournment of the
William F. Slagle Dental Meeting. For more details, call tollfree (877) 216-0009.
Hotel Accommodations & Services
Please make your hotel reservations by phoning the Hilton
Memphis at (901) 684-6664 or toll free at (800) 445-8667.
You must identify yourself as a participant of the William
F. Slagle Dental Meeting to receive the special rate of $129
single/double. All room reservations must be made by
January 30, 2017. Hotel confirmation will be sent from the
Hilton Memphis. The hotel provides free shuttle service
from the Memphis International Airport. Please note: The
William F. Slagle Dental Meeting has been designated a
non-smoking conference. A smoking area is provided at the
Lakeside exit of the conference venue.
Location
The Hilton is conveniently located off Interstate 240 and
Poplar Avenue in one of Memphis’ most upscale and
residential areas. It is 20 minutes from the Memphis
International Airport.
The University of Tennessee Health Science Center College
of Dentistry is an ADA CERP Recognized Provider. ADA
CERP is a service of the American Dental Association to
assist dental professionals in identifying quality providers
of continuing dental education. ADA CERP does not
approve or endorse individual courses or instructors,
nor does it imply acceptance of credit hours by boards
of dentistry. The current term of approval extends from
November 2015 to December 2019.
Courses presented are acceptable by those states that
recognize the ADA (CERP) approved and AGD accepted
providers.
Upon completion of the course each participant will
receive a certificate of completion, which includes the
name, date of the course, and the credit earned.
Continuing education courses that are presented cover
all aspects of the practice of Dentistry including new
knowledge that may not be widely accepted by the
profession. In so doing, some presentations may include
controversial material or commercial reference. Please be
aware that the presentation of such course material should
not be considered as an endorsement or approval of a
particular philosophy, product, or procedure.
The University of Tennessee Health Science Center
College of Dentistry designates this activity for up to 19
continuing education credits. Up to 19 credit hours may
be earned by attending the maximum number of courses,
(16 hours), mini-sessions, (2 hours), and breakfast with
faculty, (1 hour).

Oral Sedation Made Easy to Swallow

Sedation and analgesia are frequently administered to
patients in-office to relieve pain, discomfort and anxiety
associated with dentistry. This four-hour course offers a
comprehensive look at the science of oral sedation for both
children and adults. Essential components that will be
reviewed include: pharmacology of commonly used oral
sedative agents, appropriate protocols and safety provisions
for planning an oral sedation, monitoring and recovery,
the recognition and management of airway problems, and
emergency management of adverse events. This course is
designed to provide reinforcement of recognized standards
for administering moderate sedation in order to maximize
the benefits of oral sedation to the patient while minimizing
risks to patient safety. The target audiences for this course
are pediatric dentists utilizing oral sedation and dentists
administering oral sedatives in an effort to obtain moderate
sedation in adults.
This course provides the required 4 hours of CE for
recertification.

Larry Weeda, Jr., DDS, Professor and Chairman
of the Department of Oral and Maxillofacial
Surgery and past Director of the Residency
Program in Oral and Maxillofacial Surgery
joined the faculty of the University of
Tennessee College of Dentistry in January of
1998. He is a graduate of the University of
Missouri-Kansas City College of Dentistry. He
received his training in oral and maxillofacial
surgery at the Naval Regional Medical Center
in Portsmouth, Virginia and retired from the
Navy with the rank of Captain in 1998 after
27 years of service.
Martha Wells, DMD, MS
Dr. Wells is an Associate Professor and the
Program Director of Pediatric Dentistry at
the University of Tennessee Health Science
Center. She is a graduate of The Dental
College of Georgia and completed her
specialty training and Master of Science at The
Ohio State University. She has had projects
published in the Journal of the American
Dental Association, the Journal of Pediatric
Dentistry, and the International Journal of
Pediatric Dentistry. Dr. Wells is an editor of the
textbook Pediatric Dentistry: Infancy through
Adolescence, 6th edition. She also serves on
the American Board of Pediatric Dentistry’s Examination Committee.

Oral Surgery for the General Dentist:

Karl R. Koerner, DDS, MS
Dr. Koerner is an Adjunct Professor of Oral
Surgery at Roseman University in Utah. He
has presented hundreds of didactic and
participation courses to GPs over the last
20 years. He is the author of many articles,
has authored or co-authored four books on
surgery, and has made several surgery DVDs
with Dr. Gordon Christensen. He is a general
dentist trained in the Army with a current
practice of only oral surgery. In 2010, he
received the National AGD’s Weclew award
for his contributions to dentistry and dental
education. Dr. Koerner is past president of the
Utah Dental Association.

Course Code: 004 / 6 hours CE
Many extractions look easy but can soon become difficult,
time consuming, and lead to problems. This course reviews
the best techniques and instruments to remove “surgical”
extractions easily and quickly while conserving bone. It
makes exodontia more enjoyable and predictable so it can
be performed with greater confidence and less stress. Dr.
Koerner presents an algorithmic process that progresses “by
the numbers” until the extraction is complete. If one thing
doesn’t work, try the next one. It covers multiple extractions
with alveoplasty and preventing complications such as
sinus misadventures, excessive bleeding, and preventing
tuberosity fractures.

• More easily do “surgical” or difficult extractions that are
frequently encountered in general practice

during or after oral surgery

and dental caries following cancer therapy, Dr. Ash has seen

Kyle Ash, DMD
Dr. Kyle Ash is the former owner of two
separate private dental practices. He is the
founder of South Texas Dental Oncology and
current President of the International Society
for Preventive Dentistry and Medicine (ISPDM).
In addition to his current private practice
dedicated to the treatment of individuals
suffering from head and neck cancer, he
participates in a weekly multidisciplinary head
and neck tumor board. Dr. Ash is a ‘key opinion
leader’ in the field of dental oncology lecturing
both nationally and internationally to all health
professionals. Dr. Ash has a profound passion
for oral systemic disease prevention and enhanced quality of life.
Learn current screening, staging, and treatment for the oral
cancer patient, along with prevention of fibrosis and the
latest on HPV. Participants will learn which drugs induce
ONJ along with the ability to properly diagnose and treat
osteoradionecrosis. Learn the major causes of Xerostomia
and how to treat its’ harmful effects.

Management of Common Esthetic
­Dilemmas: An Interdisciplinary Approach

What esthetic principles are fundamental to generating
consistently successful results? How can restorative/ortho
interdisciplinary treatments be optimized?
This presentation will address a variety of contemporary
topics in the realm of conservative esthetic dentistry.
Coordinated approaches to the diagnosis and treatment
of interarch tooth size discrepancies as well as appropriate
space distribution for cases involving undersized teeth will
be discussed with special emphasis on the ortho/restorative
interactions that are needed to optimize results. Both the use
of directly bonded restorations for tooth shape alteration as
well as insights and step-by-step procedures for successful
porcelain veneers will be chronicled.
Detailed treatment planning considerations will be
elucidated with regards to the management and treatment
of missing anterior teeth, including various approaches to
providing long-term esthetic provisional restorations. These
and many other salient topics will be presented in a manner
that highlights interdisciplinary interactions required

Distinguished Professor of Operative Dentistry
at the University of North Carolina School

and Past-President of the American Academy
of Esthetic Dentistry. Dr. Heymann is CoSenior Editor of Sturdevant’s Art and Science
of Operative Dentistry textbook and is a
consultant to the ADA. He has authored over
190 scientific publications, given over 1,400
lectures worldwide, is Editor-in-Chief of the
Journal of Esthetic and Restorative Dentistry,
and has received the Gordon J. Christensen
Award for excellence as a CE speaker.
Gavin Heymann, DDS, MS
Dr. Gavin Heymann earned his DDS and
MS in Orthodontics at the University of
North Carolina School of Dentistry. He is
a fellow of the American and International
College of Dentists. His areas of clinical
interest include interdisciplinary treatment,
use of temporary anchorage devices, and
applications of orthodontic dental materials.
Dr. Heymann has authored numerous peerreviewed publications, and is a reviewer for
multiple orthodontic and dental journals. He
is an adjunct assistant professor in the UNC
Department of Orthodontics, and has lectured
internationally to orthodontic and dental
audiences alike. He maintains private orthodontic practices in Chapel Hill

Chemical Dependency and Controlled
Substance Prescribing

Chemical dependency is a widespread condition that effects
men, women and children alike. Effective July 1, 2014, all
prescribers in Tennessee who hold a current federal drug
enforcement administration (DEA) license, and who
prescribe controlled substances are required to complete
a minimum of two hours of continuing education related
to controlled substance prescribing. This course will
provide the required two hours of continuing education for
chemical dependency and the proper prescribing of opiates,
benzodiazepines and other prescription medications based
on the guidelines by the State of Tennessee. The course will
also present an overview of substance abuse, addiction,
current rehabilitation strategies and the potential impact of
substance abuse on the dental practice.

problem in the United States and the state of Tennessee

Lynn Russell, RDH, BSDH, MEd, EdD
Lynn is a graduate of the Dental Hygiene
class of 1975 and completed her BSDH
in 1977. She has over 25 years of private
practice experience, and worked both in
Community and Preventive Dentistry and the
Dental Hygiene Department at the University
of Tennessee Health Science Center. She
received her M. Ed. from Christian Brothers
University and her Ed. D. in Leadership, with
a concentration in Adult Education, from the
University of Memphis.
Lynn launched the Dental Hygiene program
for Concorde Career College, a proprietary
school with 16 campuses, and in 2013
became the Director of Teaching and Professional Development for the
Concorde group. The program she developed there is now on eight of
their campuses. Sharing her expertise, she also wrote a dental hygiene
program for a school in East Tennessee.
Currently, Lynn is the Chair of the Department of Dental Hygiene and the
Interim Program Director for the Masters of Dental Hygiene program at the
University of Tennessee Health Science Center. She is an editor for Gavin
Publishing’s Journal of Dental Sciences and is active in the Memphis
Dental Hygienists’ Association and the Tennessee Dental Hygienists’
Association having served as Trustee for District 9. Currently, Lynn is the
President-Elect and will serve as the President of the TNDHA in 20172018.
James C. Ragain, Jr., DDS, MS, PhD, FICD, FACD
Dr. James C. Ragain, Jr., retired from the U.S.
Navy in 2013 and subsequently joined the
UTHSC College of Dentistry faculty where he
teaches biomaterials and operative dentistry.
Dr. Ragain is also Director of Continuing Dental
Education. He has authored or co-authored
over 75 professional publications and holds
three U.S. patents. Dr. Ragain has served as
a member of the UTHSC College of Dentistry
Alumni Board of Trustees for over twelve years.

Joseph J. Safirstein, DDS
Dr. Joe is a 1999 graduate of the UT College
of Dentistry and one of nine dentists in his
family. He continued his education in Portland,
Oregon, earning a certificate in orthodontics
in 2001. He currently maintains a private
practice in Portland. For the past 10 years,
Dr. Joe has taught practice management in
the orthodontic residency program at the
Oregon Health and Sciences University as
an associate professor. He has a special
interest in helping young doctors address the
pressures faced in transitioning from student
life to professional work life, while maintaining
happiness and longevity. In addition to private
practice and teaching, Dr. Joe regularly consults with dentists and dental
professionals in the areas of efficiency, systems and the financials of
running a dental practice.
Additionally, while attendees in this seminar are focused on
the beginning of their careers, we’ll spend time considering
exit strategy and retirement. More importantly, we’ll explore

Dr. Levy earned Mastership and five Lifelong
Learning Service Recognitions by the Academy

life’s work—your dental practice. As Brenda Thaxton’s
biographical sketch explains, she is the surviving spouse
of Steven Thaxton, DDS. In Tennessee, a surviving spouse
has two years as a non-dentist to own a dental practice
and transition it. However, the value of the practice can
decline rapidly losing both employees and patients while a
spouse is in incapacitating mourning. Suddenly, a spouse
owns a dental practice and all the federal, state, and local
regulations still apply and the spouse is liable. All the taxes,
payroll, HIPPA, OSHA, insurances, and emergency patient
considerations, to name a few, must continue. It is a great
act of love to preemptively set up the systems necessary.
This compassionate and informative seminar will discuss
what happens if there is an unexpected death.

In March of 2016, the Centers for Disease Control and
Prevention (CDC) published Summary of Infection
Prevention Practices in Dental Settings: Basic Expectations
for Safe Care based upon the recommendations of existing
evidence-based guidelines from 2003. The original
recommendations and the summary are meant to help
dental health care workers (DHCW) achieve compliance.
Time, however seems to be the enemy. This course will
look at ways to address time constraints and basic infection
control principles. Dental water asepsis, instrument
processing and cleaning and disinfecting will be reviewed
along with OSHA’s Hazard Communication.
Brenda P. Thaxton, RDH, BS
Brenda Thaxton is the surviving widow of
Steven F. Thaxton, DDS, MAGD, who died
suddenly in an accident in 2013. They were
married for 39 years. Immediately thrust
into the ownership of a dental practice, she
provided care for her husband’s patients
and successfully transitioned it. 95% of
the patients and 100% of the employees
transitioned with the practice and Brenda
received her asking price within five months.
Brenda received her BS degree in dental
hygiene from the University of Tennessee in
1975 and pursued post graduate studies
at Cumberland University. She worked as a
dental hygienist, office manager and corporate officer for 40 years. Since
Steven’s death, Brenda has devoted her time to helping others, who could
potentially find themselves in the same situation. She has lived it. Brenda
presents her survival story at numerous dental meetings throughout the
mid-south.

Katy Warren is a registered dental hygienist and
an Associate Professor in the Department of
Dental Hygiene, College of Health professions
at the University of Arkansas for Medical
Sciences. She has been active in teaching
clinical dental hygiene for over 30 years.
Katy is the president of an infection control
compliance company and has presented
numerous continuing education courses on
infection control. She serves as an infection
control evaluator for the Arkansas State Board
of Dental Examiners. Katy is a member of
the American Dental Hygienists’ Association
and the Organization for Safety, Asepsis and
Prevention (OSAP).
LEARNING OBJECTIVES
• Identify items that DHCW tend to avoid due to time

According to the National Eating Disorders Association,
20 million women and 10 million men in the United States
suffer from eating disorders sometime in their life time.
Children as young as 12 and 13 are affected by this disorder.
Unfortunately, since eating disorders are a mental health
condition and carry a negative social stigma many patients
affected by this disease are reluctant to disclose their
condition to medical and dental professionals.
Learn about the clinical, oral and psychological characteristics of Eating Disorders and the latest preventive
guidelines for treating this patient population. Guidelines
for early detection and multidisciplinary approach can
allow dentists to identify signs of eating disorders and help
patients to recovery.

Dr. Mojdeh Dehghan is a graduate of Baylor
College of Dentistry with over 25 years
of private practice, and university faculty
practice experience. Currently, she is an
Associate Professor and Associate Director of
the Division of Esthetics at the University of
Tennessee College of Dentistry. Dr. Dehghan
is the past president of the College Faculty
Organization. Her research in Dental Erosion
has won several awards including the 2014
AAWD Proctor and Gamble/Gillette Hayden
Memorial Foundation Research Award.
She has published in several scientific

of Cosmetic Dentistry, Journal of Dental

Nancy Johnson, EDD, LPC/MHSP
Dr. Nancy Johnson is Clinical Director for Eating
Disorders and Grief and Trauma Intensive
Outpatient Programs at Transformation
Center, Cordova, Tennessee. She has a
doctoral degree in Counseling and Education
and a Master’s degree in Community Agency
Counseling from The University of Memphis.
Dr. Johnson’s Bachelor of Science degree in
Psychology with minor in Women’s Studies is
from the University of Northern Colorado.
Dr. Johnson has been a licensed professional
counselor for ten years with over 25 years
of experience working in mental health
settings. She was given the West Tennessee
Counseling Association’s Private Sector Counselor of the Year Award in
2008. Dr. Johnson works with a variety of counseling issues that include:
addictions, eating disorders, depression, anxiety, grief and loss, and crisis
management.
“Throughout my career, I have been a strong advocate especially for
women and children.” Dr. Johnson is a member of the Tennessee
Counseling Association (TCA) and past president of the TCA division of
Tennessee Mental Health Counseling Association.

Were it not for an active
sports life, Dr. Dewayne
McCamish may have chosen a
different career. Interactions with
three people in particular—all
dentists—during high school and
college encouraged Dr. Dewayne
McCamish to study dentistry,
specializing in orthodontics.
The University of Tennessee
Health Science Center (UTHSC)
College of Dentistry (COD) Alumni Association has named
Dr. Dewayne McCamish an Outstanding Alumnus Award
recipient for 2017.
Dr. McCamish enrolled in the UTHSC COD following
his 1967 graduation from Middle Tennessee State University
(MTSU) and earned a DDS in 1970. He received a master’s
degree in orthodontics from the UTHSC Graduate Orthodontics program in 1973.
In March 2016, Dr. MCamish was installed as the 109th
president of the American Association of Orthodontics
(AAO). He has also served as past president of the Southern
Association of Orthodontics (SAO), the Tennessee
Association of Orthodontists (TAO), the University of
Tennessee Orthodontic Alumni Association, and the Third
District Dental Society (now named the Chattanooga Area
Dental Society). He has been inducted as a Diplomate of the
American Board of Orthodontics, a member of the College
of Diplomates of the American Board of Orthodontics, and
is a Fellow of both the American College of Dentists and the
International College of Dentists.
Dr. McCamish has been recognized for his professional
service with the Oren Oliver Distinguished Service Award
from the SAO and the Outstanding Alumnus Award by the
UTHSC Graduate Orthodontic Program.
Dr. McCamish is very proud of the patients he has
treated in 44 years of practice, and has four keys to success:
strive for excellence in all you do, keep your life in balance,
establish relationships with your patients, and always honor
your code of ethics.
Dr. McCamish continues the active sports life that led
him to dentistry. He is an avid tennis player, golfer, snow
skier and yoga enthusiast.
In recognition of his contributions to his alma mater

Dr. Sebelius was inspired to
attend dental school by his father
and grandfather, who were also
dentists. He is a 1964 University
of Tennessee Health Science Center (UTHSC) College of
Dentistry (COD) graduate and completed a residency in
oral and maxillofacial surgery at UTHSC in 1967.
In 1968, Dr. Sebelius joined the faculty in the COD.
He served on 18 COD committees during his 41-years
as a faculty member including the COD Faculty Senate,
the Administrative Council, and was chairman of the
Committee of Faculty. He retired in 2009 as Professor
Emeritus.
Dr. Sebelius maintained a private dental practice while
still a full-time member of the COD faculty. He served
on the staff of Baptist Memorial Hospital as chairman of
the Department of Dentistry and was courtesy staff with
LeBonheur Children’s Medical Center and Methodist
Healthcare until his retirement.
Dr. Sebelius has been elected president of virtually
every organization of which he is a member, including the
Memphis Society of Oral and Maxillofacial Surgeons, the
Memphis Dental Society, the Tennessee Dental Association,
Omicron Kappa Upsilon Psi Chapter of Xi Psi Phi Fraternity,
and the American Dental Interfraternity Council. He
served as a Regent for the American College of Dentists in
leadership positions for the American Dental Association,
the Tennessee Board of Dentistry and the Regional Medical
Center at Memphis (now Regional One).
Dr. Sebelius received numerous awards throughout
his career. He was awarded the Distinguished Service
Award from the UT Alumni Association in 1998 and
has additionally received awards from the International
College of Dentistry, the Tennessee Dental Association, the
American College of Dentists, Who’s Who in Dentistry, Xi
Psi Phi Fraternity, the International College of Dentists, and
the Memphis Dental Society.

Dr. Cory Glenn’s trajectory
to success since his 2008
graduation from the University
of Tennessee Health Science
Center (UTHSC) College of
Dentistry (COD) is described
as unprecedented by his
classmates and colleagues. In
the online dental community,
Dentaltown, Dr. Glenn’s posts
are among the most followed
by its virtual inhabitants. Dr. Glenn considers himself a
small-town dentist who loves to learn.
Dr. Glenn is the 2017 recipient of the UTHSC COD
Alumni Association’s Recent Alumnus Award.
Dr. Glenn completed a one-year residency program in
Advanced Education in General Dentistry with a focus on
comprehensive care and dental implants through UTHSC.
Immediately following, he purchased an established
practice in Winchester, Tennessee, and has found it much
easier to grow a practice in a small town rather than in a
larger city.
In his practice, Dr. Glenn focuses on utilizing
technology and innovative techniques to provide
treatment in an efficient and cost-effective manner. His
colleagues consider him a “MacGyver” of dentistry due to
his innovation.
Dr. Glenn is a graduate of the Georgia Maxi Course
in Implant Dentistry, the American Orthodontic Society’s
Comprehensive Ortho Program and is credentialed as an
Associate Fellow of the American Academy of Implant
Dentistry. He considers himself a perpetual student, and
in 2012 exceeded the required 20 hours of continuing
education by taking more than 380 hours.
Dr. Glenn is the continuing education director for the
Middle Tennessee Dental Study Club and vice president
of the Southern Tennessee Christian Medical and Dental
Association. His professional and communal affiliations
include membership in the American Dental Association,
the Academy of General Dentistry, the American
Academy of Cosmetic Dentistry, Rotary International,
Gideons International, and the Franklin County Chamber
of Commerce.

Service Award. This new award
is presented to Dr. Alfonso in
recognition of his pro bono
contributions to the dental
profession.
William Alfonso, DDS
’77, has lived both his personal and professional life
with service to others as his primary motivation and is
the founder of Operation New Life, a multi-disciplinary
surgical mission group.
Dr. Alfonso felt called to serve God by using his skills
to help those without proper access to dental care. In 1998
he sold his private oral and maxillofacial surgery practice
and for five years traveled with his wife, Sandra, on oneweek trips to Honduras several times per year performing
medical and non-medical missionary work.
In 2004, Dr. Alfonso met Little Rock plastic surgeon
Dr. Kris Shewmake and formed Operation New Life. The
pair traveled to Honduras and Hospital Escuala, the only
hospital to serve the indigent population in a country
with a poor diet resulting higher-than-normal incidence
of malformed skulls and faces.
In the past 11 years, Dr. Alfonso has made the oneweek trip between three and eight times per year, leading
a team of surgeons, nurses, and non-medical missionary
workers. He and other doctors with Operation New Life
recently completed training of two oral and maxillofacial
surgeons to continue the missionary work as full-time
staff with Hospital Escuala. Dr. Alfonso feels the treatment
program in Hospital Escuala is in excellent hands.
Dr. Alfonso believes he has completed his work in
Honduras but is not leaving missionary work altogether.
He and Sandra plan to continue serving in the Dominican
Republic. They will still travel just one week at a time —
working where needed — whether it is humanitarian,
medical or evangelistic.
