The Board’s vision of improving health and healthcare for the people of Forth Valley is underpinned by its Integrated Healthcare Strategy. The principles upon which this strategy is founded have led to significant change and development in recent years. With further service reconfiguration planned, this is an exciting time to work with NHS Forth Valley. NHS Forth Valley is the largest employer in the area, with approximately 8000 staff from a wide range of professional and support occupations. Currently, services are provided in one acute hospital, four community hospitals and fifty-six health centres. Acute Services are based in the purpose built 860 bedded Forth Valley Royal Hospital, which opened in 2011 and provides inpatient and outpatient services for a population of approximately 310,000. THE FORTH VALLEY AREA The working environment in Forth Valley Royal Hospital is excellent, as is the quality of life that the Forth Valley area has to offer candidates and their families. The area has stunning scenery including the Trossachs and Loch Lomond and is steeped in history with Callander House in Falkirk, Stirling Castle, Bannockburn and the Wallace Monument to name but a few of the sites in the area. It also has many cultural and leisure facilities such as the Helix Ecopark, home to the 100ft Kelpies, the Falkirk Wheel, the MacRobert Arts Centre at Stirling and the National Sports Academy at Stirling University. Forth Valley lies equidistant between Scotland’s two largest cities; Edinburgh and Glasgow. Their respective airports are little over half an hour from most parts of Forth Valley and road and rail links are excellent. House hunters in the Forth Valley area are spoiled for choice. On offer is a wide choice of established and new properties in a variety of attractive settings. Education in the area is first class, with excellent provision of pre-school, primary and secondary education. State schools in the area have strong reputations and there is also easy access to a number of independent schools in Bridge of Allan, Dollar, Crieff and beyond. On the further education front, Stirling boasts an excellent University and access to the University cities of Glasgow and Edinburgh is easy. The requirements of this post will change through time in line with clinical advances and developments in services provided by the NHS in Forth Valley. 6 CONTRACTUAL DETAILS Contracted activities for this appointment will be defined and agreed with the post holder and the General Manager, Medical Directorate. The post holder will be professionally responsible to the Associate Medical Director, Dr Allan Bridges and managerially responsible to the General Manager, Mr Ian Aitken The Consultants in the department will agree arrangements for cover of cardiology duties. This Consultant post is available on a full-time basis but the Board welcomes applications from individuals who are unable to undertake a full time commitment and who may wish to work on either a part-time or job share basis. The terms and conditions of service applicable to this appointment are the Consultant grade terms and conditions of service as amended from time to time. NHS Forth Valley will require each Consultant to agree with the General Manager and Associate Medical Director, a job plan of fixed and other commitments, which will be reviewed on an annual basis and revised as necessary to meet the requirements of the local and national service issues. Consultants will be required to participate in an annual appraisal. The post holder will normally be expected to reside within 30 minutes travelling distance of Forth Valley Royal hospital. If NHS Forth Valley Health Board considers that the there is a requirement for the post holder to relocate his or her home in order to take up the responsibilities of the post, relocation expenses will be available in accordance with the Board’s Relocation Policy. Support to the level of 15% of starting salary up to a maximum of £10,000 may be available, subject to the provisions of the policy. Formal appointment to this post will be conditional upon satisfactory completion of all preemployment checks, including satisfactory medical screening. The Cardiology Team aims to provide safe, patient-focused and sustainable services for the people of Forth Valley. The post holder will replace sessions released by colleagues pursuing alternative local and regional duties and will work within the existing team of 5 Consultant Cardiologists. The development of an applicant’s subspecialty interest will be encouraged and supported. In particular, we would look favourably on applicants with an interest in heart failure, pacemaker and cardiac device therapy and cardiac CT/MRI. Candidates will be expected to have a wide range of experience in General Cardiology and possess the MRCP (UK) or equivalent. Candidates with additional accreditation in General Medicine who wish to pursue involvement in general medical services will be supported to do so. NHS Forth Valley provides acute services for a population of 310,000 people in Central Scotland. The £300m Forth Valley Royal Hospital became fully operational in 2011 and is one of the most modern and well-equipped hospitals in the country. It is situated in Larbert with excellent road and rail links to Stirling, Edinburgh and Glasgow. Within FVRH we have a purpose built cardiology unit hosting outpatient, inpatient and investigation services. We adopt a multidisciplinary approach to deliver rapid access chest pain clinics, specialist valve surveillance clinics, heart failure and genetics clinics. We have ambulatory cardiac services, specialist echocardiography, CT coronary angiography and a chest pain assessment unit. Inpatients are managed in a 14-bedded cardiology ward with additional beds within our adjacent high dependency unit. Within this integrated setting we provide a team-based approach to patient care and this appointment will play an important role in the development and continuous quality improvement of Cardiology services. The Cardiology senior team comprises five Consultants, one Consultant Nurse / Department Manager and a Cardiology Investigations Manager. The department is supported by one Specialty Doctor, three Advanced Nurse Practitioners providing inpatient and outpatient heart failure care and a Cardiac Rehabilitation Team including Physiotherapy and 8 Specialist Nurses. We also have a team of Specialist and Highly Specialist Clinical Physiologists (EHRA and BSE accredited). There is currently no routine out-of-hours cardiology service. There may be a contribution to acute general medical receiving for those candidates with accreditation in General Medicine, by negotiation. The General Medical receiving model has one Consultant available in the Acute Admissions Unit from 8am until 8.30 pm and a second Consultant available at different times to 8 support the emergency take. There are currently twenty-three Consultants on the General Medical rota. These posts are open primarily to applicants who do not require a work permit to take up employment in the UK. Clinical Lead Cardiology/Ischaemic Heart Disease/ Specialist PCI Specialty Doctor Dr Linda McShane: Cardiology Outpatients/Rapid Access Atrial Fibrillation Clinic Visiting Staff Dr Ruth McGowans: Consultant in Clinical Genetics Outpatient Services Consultant Cardiologists serve general cardiology and specialty patients supported by our specialty doctor and specialist nurses. In addition there are a host of multidisciplinary and nurse led out-patient services that are supported by the Consultant Nurse and Consultant Cardiologists with clinics in Heart Failure, Coronary Intervention Follow-Up, Ischaemic Heart Disease, Atrial Fibrillation, Aortic Valve Disease Surveillance and Suspected Angina. A Cardiac Genetic service is provided by Dr Ruth McGowan and the West of Scotland Genetic team monthly. We are currently developing a specialist Inherited Cardiac Conditions Clinic. Suspected Angina Assessment Service - 6 clinics per week. Patients with suspected angina are offered a prompt appointment. The clinic is run by a Specialist Nurse and Senior Clinical Physiologist. The clinic is supported by the duty Cardiologist of the Week. Specialist Valve Clinic - 2 clinic sessions per week for patients with mild to moderate Aortic Valve disease or Aortic Valve prostheses who require ongoing surveillance. The clinic is run by a Specialist Nurse and Highly Specialist Clinical Physiologist and supported by a Consultant Cardiologist. Post-Coronary Intervention Clinic - 3 clinics per week. All in-patients undergoing coronary intervention are reviewed by a Specialist Nurse addressing both pharmacological and nonpharmacological aspects of care and rehabilitation. Heart Failure Service – Our model of heart failure care involves advanced nurse practitioners in inpatient care as well as follow up (7 sessions/week). There is also provision for patients requiring outreach at home and palliative care. Weekly MDT and monthly Heart Failure/Cardiac Devices meetings. Ischaemic Heart Disease Clinic – A nurse-led service offering rapid out-patient assessment of patients with known ischaemic heart disease and deteriorating symptoms. Investigations Electrocardiography, ambulatory electrocardiography including event monitors. Ambulatory blood pressure monitoring. Exercise Tolerance Testing. Echocardiography including Transoesophageal echocardiography, Exercise and Dobutamine Stress echocardiography and Contrast echocardiography. CT Coronary Angiography (Limited service with plans for expansion) Coronary Angiography and percutaneous coronary intervention performed in either the Edinburgh Royal Infirmary or Golden Jubilee National Hospital. 10 Cardiac MRI provision within Scotland is limited. We have an MR scanner on site in FVRH with future plans to include a cardiac package to permit local Cardiac MRI. Arrhythmia and Device Service The device implantation service currently spans Forth Valley and Lothian. Cardiac loop recorders (Linq®) are implanted in our ambulatory cardiology unit by specialist Advanced Nurse Practitioners. Permanent Pacemakers are predominantly implanted at Edinburgh Royal Infirmary. Implantable Defibrillators are implanted on site (FVRH) and in Edinburgh Royal Infirmary. Cardiac Resynchronisation therapy is, for the most part, provided in partnership with the National Advanced Heart Failure Service in the Golden Jubilee Hospital in Glasgow. Follow up of all devices takes place within FVRH by a team of highly specialist physiologists who are EHRA accredited. We employ remote follow up widely. The device service works closely with the heart failure service to ensure optimal follow up of this patient group. General Medicine and Acute Medical Receiving Those candidates with accreditation in general medicine may wish to continue general medical practice including acute receiving. Since the centralisation of the emergency services, there has been an average of 37 emergency admissions per day (of all ages) with 25 patients admitted under general medicine. There is a 16 bed Combined Assessment Unit (CAU) shared with surgical specialties and adjacent to the A&E Department. There is a 36 bed Acute Admissions Unit adjacent to CAU. Patients admitted to hospital from AAU and triaged to the relevant specialty specific clinical area. There is a large Day Medicine unit which provides a range of options for ambulatory management of patients. Patients likely to be discharged within 48 hours of admission are discharged from the Acute Admissions Unit by either the Acute Medicine team or the on call team responsible for the admission. Patients likely to stay longer will be transferred to a specialist inpatient ward. Those patients requiring longer periods of rehabilitation are transferred to the Community Hospital facilities under the care of colleagues in Ageing and Health medicine. There are 25 Consultant Physicians participating in Acute Medical Receiving with a 1:25 on call overnight frequency and participation in General Medical receiving 1:12. The process of refining acute medical care will evolve over the coming months. Medical Staffing General medical staffing includes consultants from acute medicine, cardiology (Drs Bridges, Glen and Hargreaves), Diabetes and Endocrinology, Gastroenterology and Respiratory Medicine. A number of other specialties support General Medicine including Ageing & Health. Medical patients services are also supported by three Consultant Haematologists, three Consultant Rheumatologists and an Associate Specialist in Rheumatology, three Consultant Neurologists and one Consultant in Rehabilitation Medicine. There are visiting Consultants from Nephrology and Oncology. The successful applicant will be expected to participate actively in the day to day working of the Cardiology service. The unit is a busy one and supports a number of specialist clinics and investigation services. NHS Forth Valley Consultant Cardiologists provide an on call service during working hours for one week in every five with prospective cover, accepting new inpatients and supporting the wider hospital teams as well as local general practitioners. There is no Cardiology out-of-hours service in FVRH with patients being cared for, in the first instance, by the general physician on call. Support, where required, comes from our tertiary service providers with whom we have service level agreements (NHS Lothian and NHS Greater Glasgow and Clyde) and is predominantly but not exclusively concerned with the provision of primary PCI services. It therefore vital that in-patients are cared for with clear management plans and there is good communication with tertiary centres where required. 11 Our new colleague will be required to work closely with Consultant colleagues and the wider Cardiology multi-disciplinary team and support service development and continuous quality improvement. The focus will be General Cardiology, however, the development of a subspecialty interest will be encouraged and supported. Forth Valley has an award-winning Heart Failure service and our team would particularly value the addition of a clinician with an interest in Heart Failure to drive forward and build on our success in Heart Failure service provision. Additional skills in pacemaker, cardiac device implantation and/or cardiac MR imaging would be viewed positively. Forth Valley NHS aims to deliver high quality, patient-centred care and we aim to provide outpatient services in a timely fashion and in accordance with current waiting time directives. It is expected that the successful candidate will provide two general cardiology outpatient clinics per week plus a specialty outpatient clinic (minimum of 126 clinics per year although this along with other aspects of the job plan will be dependent on any potential general medical component) and support the work of nurse-led outpatient services. Postgraduate and Developmental Facilities Forth Valley Royal Hospital has a Postgraduate Centre with Lecture Theatre and modern high quality audio-visual facilities, a well-equipped medical library and a Quality Improvement Hub. It offers opportunities for a wide range of development activities for all staff groups. A wide range of full text and e-journals are freely available. Internet access is available in the centre and also in Consultant accommodation. There are regular postgraduate meetings with telemedicine links to Glasgow and Edinburgh with their Royal Colleges. University medical schools and postgraduate facilities are within easy reach. Forth Valley Royal Hospital is the site of the Scottish Centre for Simulation and Clinical Human Factors. 12 JOB PLAN The post holder is required to agree a detailed job plan which will be reviewed at least annually. Efforts will be made to complete job planning prior to the date of commencement in post, or, if this is not possible, as soon as possible following this date. The detail of fixed commitments, including any special interest, will be finalised following appointment of the successful applicant. This is a 10 Programmed Activity post of which one SPA will be allocated for Personal Development. Following commencement in post, depending on the needs of the service and the particular attributes of the post holder, it may be possible to negotiate additional time for Supporting Professional Activities to include audit, teaching and training. This will require the approval of the General Manager and Associate Medical Director. Supporting professional activities will include all of the Consultant’s professional duties and commitment. The exact timing and location of the Supporting Professional Activities will be agreed during the job plan review with the General Manager and Associate Medical Director and included in the prospective job plan. Candidates will be have experience in high volume Electrophysiology and would be expected to develop a practice focussing on more complex procedures including ablation for Atrial Fibrillation, ventricular tachycardia and ablation in adults with congenital heart disease. The post also includes sessions for implantation of pacemakers, implantable defibrillators and cardiac resynchronisation therapy devices. It is situated in Larbert with excellent road and rail links to Stirling, Edinburgh and Glasgow. Within FVRH we have a purpose built cardiology unit hosting outpatient, inpatient and investigation services. We adopt a multidisciplinary approach to deliver rapid access chest pain clinics, specialist valve surveillance clinics, heart failure and genetics clinics. We have specialist ambulatory cardiac services including specialist echocardiography, CT coronary angiography and a chest pain assessment unit. Inpatients are managed in a 14-bedded cardiology ward with additional beds within our adjacent high dependency unit. There is currently no routine out-of-hours cardiology service. NHS Forth Valley Consultant Cardiologists provide an on call service during working hours for one week in every five with prospective cover, accepting new inpatients and supporting the wider hospital teams as well as local general practitioners. There is no Cardiology out-of-hours service in FVRH with patients being cared for, in the first instance, by the general physician on call. Support, where required, comes from our tertiary service providers with whom we have service level agreements (NHS Lothian and NHS Greater Glasgow and Clyde) and is predominantly but not exclusively concerned with the provision of primary PCI services. It therefore vital that in-patients are cared for with clear management plans and there is good communication with tertiary centres where required. Our new colleague will be required to work closely with Consultant colleagues and the wider Cardiology multi-disciplinary team and to support service development and continuous quality improvement. The focus will be General Cardiology, however, developments in the arrhythmia and cardiac devices service will be encouraged and supported alongside dedicated electrophysiology sessions within the Golden Jubilee Hospital. Forth Valley NHS aims to deliver high quality, patient-centred care and we aim to provide outpatient services in a timely fashion and in accordance with current waiting time directives. It is expected that the successful candidate will provide two general cardiology outpatient clinics per week plus a specialty outpatient clinic (minimum of 102 clinics per year although this along with other aspects of the job plan will be dependent on any potential general medical component) and support the work of nurse-led outpatient services. Outpatient Services Consultant Cardiologists serve general cardiology and specialty patients supported by our specialty doctor and specialist nurses. A Cardiac Genetic service is provided by Dr Ruth McGowan and the West of Scotland Genetic team monthly. We are currently developing a specialist Inherited Cardiac Conditions Clinic. Suspected Angina Assessment Service - 6 clinics per week. Patients with suspected angina are offered a prompt appointment. The clinic is run by a Specialist Nurse and Senior Clinical Physiologist. The clinic is supported by the duty Cardiologist of the Week. 18 Specialist Valve Clinic - 2 clinic sessions per week for patients with mild to moderate Aortic Valve disease or Aortic Valve prostheses who require on going surveillance. The clinic is run by a Specialist Nurse and Highly Specialist Clinical Physiologist and supported by a Consultant Cardiologist. Post-Coronary Intervention Clinic - 2 clinics per week. All in-patients undergoing coronary intervention are reviewed by a Specialist Nurse addressing both pharmacological and nonpharmacological aspects of care and rehabilitation. Heart Failure Service – Our model of heart failure care involves specialist nurses in inpatient care as well as follow up (7 sessions/week). There is also provision for patients requiring outreach at home and palliative care. Weekly MDT and monthly Heart Failure/Cardiac Devices meetings. Ischaemic Heart Disease Clinic – A nurse-led service offering rapid out-patient assessment of patients with known ischaemic heart disease and deteriorating symptoms. Investigations Electrocardiography, ambulatory electrocardiography including event monitors. Ambulatory blood pressure monitoring. Exercise Tolerance Testing. Echocardiography including Transoesophageal echocardiography, Exercise and Dobutamine Stress echocardiography and Contrast echocardiography. CT Coronary Angiography (Limited service with plans for expansion) Coronary Angiography and percutaneous coronary intervention performed in either the Edinburgh Royal Infirmary or Golden Jubilee National Hospital. Cardiac MRI provision within Scotland is limited. We have an MR scanner on site in FVRH with future plans to include a cardiac package to permit local Cardiac MRI. Arrhythmia and Device Service The device implantation service currently spans Forth Valley and Lothian. Cardiac loop recorders (Linq®) are implanted in our ambulatory cardiology unit by specialist Advanced Nurse Practitioners. Permanent Pacemakers are predominantly implanted at Edinburgh Royal Infirmary. Implantable Defibrillators are implanted on site (FVRH) and in Edinburgh Royal Infirmary. Cardiac Resynchronisation therapy is, for the most part, provided in partnership with the National Advanced Heart Failure Service in the Golden Jubilee Hospital in Glasgow. Follow up of all devices takes place within FVRH by a team of highly specialist physiologists who are EHRA accredited. We employ remote follow up widely. The device service works closely with the heart failure service to ensure optimal follow up of this patient group. It has recently also opened The Beardmore Centre of Health Science, and recently established Scottish Innovation Centre, facilities which provide educational and research capabilities.
