The doctors, nurses and staff aim to offer the highest standard of patientcentred healthcare. We are here to help when you are ill, and run numerous clinics for patients with long term conditions (chronic diseases). We also offer a wide variety of other medical services such as antenatal and postnatal care, minor surgery, contraception, childhood vaccinations and wellperson check-ups. In addition to everything you need to know about the practice you will also find other healthrelated information. You can also order repeat medication through the prescription title above - the system has changed and there are now two ways to order prescriptions on line which are explained on the prescription page. Extended hours surgeries are also offered on most Tuesdays with appointments available up until 8.00pm. These appointments are primarily for those patients who find it difficult to attend the surgery during normal surgery hours. Please ask the receptionists when individual doctors are holding surgeries. When We Are Closed A doctor is available in an emergency 24 hours a day. When you telephone the surgery out of hours you will be automatically connected to the Nottingham out of hours service. This service is for emergencies that cannot wait until the surgery is open again. The doctor on call may give you advice or may ask you to attend the surgery the following day or make a visit to you in your home. Please remember to only phone out of hours if it cannot wait until the surgery is open again. Don't phone for routine enquiries. If you have a serious EMERGENCY call 999 to request an ambulance. NHS Direct (National) In the case of urgent need when the practice is closed you can also call NHS Direct (www.nhsdirect.nhs.uk/) (0845 4647) to speak to a triage nurse. Your needs will be assessed and advice offered or arrangements made for you to see a doctor. Walk in Centre The NHS Walk-in Centre is located on London Road next to the BBC. This is a nurse led drop-in service offering health advice, information and treatment of minor ailments by experienced NHS nurses and health information advisors. The centre is open 365 days a year from 7.00am to 9.00pm. Your local pharmacy can also offer advice about a range of medicines available without prescription to relieve minor illness and injuries. Appointments Routine Appointments All consultations are by appointment. Please make an appointment either by telephone or in person at reception. If more than one member of the family is to be seen, please book separate appointments to avoid delays for other patients. Urgent Appointments If you have a problem which you feel needs attention urgently on the same day, please explain this to the receptionist who will make an appointment for you. Urgent cases are always seen the same day by the first available doctor. Please help us with these consultations by ensuring we have an up to date telephone contact number. The doctors respect your time and endeavour not to keep you waiting too long. However, emergencies arise and have to be given priority. On these occasions, if you are inconvenienced we hope that you will understand. If you cannot keep your appointment please let us know as soon as possible. We can always offer the cancelled appointment to another patient. Home Visits If you are too ill to attend surgery please telephone before 10.30am and outline your problem to the receptionist to help the doctor assess the priority of each patient who requires a visit. A doctor usually visits after morning surgery but please let us know if it is more urgent. The doctor may phone before the visit. Telephone Appointments The doctors run a telephone appointmets system for things that do not require a face to face visit. You can make the appointment as normal via reception. Although we cannot say exactly when the doctor will call, it will usually be at the end of their morning surgery (around 11.30am). We will try to fit you in with the doctor you request, but if that doctors list is full for the day, you will be offered either another doctor on that day or your requested doctor on their first available appointment slot. Please help us with these consultations by ensuring we have an up to date telephone contact number. Help us to help you! Please inform us in plenty of time if you cannot keep an appointment so that we can offer it to someone else. Please be patient, if delays occur these are usually due to emergencies. Please remember that someone else may need urgent treatment - only ask for an out of hours visit if it is a genuine emergency. Only ask for a home visit if you are too ill to attend surgery and please ring before 10.30am. Please be considerate to other patients. Do not ask for an immediate appointment when surgery is fully booked unless it is absolutely necessary. Change of Personal Details If you change your name, address or telephone number please let our receptionist know. If you move out of our area it will be necessary for you to find a doctor in your new area. The local Primary Care Trust (PCT) will helpyou if you experience any difficulty finding a new doctor. About Repeat Requests We operate a computerised service for repeat prescriptions. We do not accept telephone requests for repeat prescriptions. You can also request further supplies of your medication either in person by posting your repeat slip to us (with a stamped addressed envelope) by liaising with your chemist. All requests will be available for collection within 48 hours after receiving (excluding bank holidays and weekends). These are all local pharmacies who can help you with your repeat prescriptions. Flu Vaccinations Seasonal Influenza Vaccinations We offer flu jabs to all over 65s and those under 65 deemed to be at increased risk of flu (i.e. those suffering from asthma and other chronic respirtory diseases, chronic heart disease, renal disease, liver disease, diabetes or immunosuppression). We run a day when you can just turn up and book in at reception on the day. This day is advertised beforehand in our local chemists, at the practice and on the website. It is usually held in October (dependant on when we get our supplies) so you can contact the surgery near that time to find out the date. Anyone missing this date will be contacted to see when they are able to get in to have one and we will possibly run a second date later on. Ante Natal and Maternity Care Our Ante Natal clinic is held in cooperation with our attached community midwife. Child Health Promotion We carry out all our Child Health surveillance. This is done in close cooperation with our health visitors. We also provide a full childhood immunisation service. Long acting contraceptives. - Please enquire at Reception Test Results Test Results Please call after 1:00pm to enquire about your test results as our reception staff will have more time to deal with your request between these times. Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate unless that person has given prior permission for the release of this data or they are not capable of understanding the results. When you take your test you will be told how long it will be before the results are returned to the practice. Chlamydia Screening Chlamydia Screening Chlamydia is the most common sexually transmitted infection in people under the age of 25. It affects both males and females and because it often has no early symptoms, it can go undiagnosed for some time. About 1 in 10 sexually active young people test positive, yet did not know they had it. If left untreated it can cause pelvic inflammatory disease and infertility. As part of the Central Nottingham Chlamydia Screening Programme, we are now offering Chlamydia screening to any patients aged 15 - 24. the test is self-taken and full instructions given. Kits are available from your nurse, doctor or reception, on request. If you have any concerns or questions with regards to sexual health, irrespective of age, please make an appointment with your doctor or nurse Cancer Screening Breast Cancer Screening Breast cancer risk is strongly related to age, with more than 80% of cases occuring in women over 50 years old. The highest number of cases of breast cancer diagnosed is in the 50-64 age group, the age group targeted for screening. Many of them are diagnosed as a result of the screening programme. Survival rates for breast cancer have increased, with 72% of women diagnosed with the disease now being likely to survive for 10 years. This is much improved on rates ten years ago when 64% of women would be expected to survive for 10 years. The NHS Breast Screening Programme continues to work closely with units across the region to deliver breast screening for all women aged over 50. Patients are targeted, through their GP surgery, on a three yearly rolling programme. This will therefore mean that you could be invited for screening at any point between the age of 50 and 53, depending on when our practice screening falls. Our next scheduled screening programme is due in September 2013. Bowel Cancer Screening Approximately one in twenty people in the UK will develop bowel cancer during their lifetime. It is the third most common cancer in the UK, and the second leading cause of cancer deaths, with over 16,000 people dying from it each year. It effects men and women equally. Regular bowel cancer screening has been shown to reduce the risk of dying from bowel cancer by 16%. From June 2008 a national bowel screening programme was introduced across Nottinghamshire County. This aims to detect bowel cancer at an early stage when treatment is more likely to be effective. If you are aged 60-69 years old and registered at St. George's Medical Practice, you will automatically receive a free self-screening kit on a 2-yearly basis. You will be sent a simple test kit to complete in the privacy of your home. This will involve collecting a small sample from three seperate bowel motions and, using a specially designed prepaid envelope, returning the kit to the laboratory for analysis. Prostate Cancer Screening Prostate cancer is a serious condition. The PSA test, which can give an early indication of prostate cancer, is available to you if you want to be tested. However, experts disagree on how useful the PSA test is. This is why there is a lot of research and why there is no national screening programme for prostate cancer in the UK. An information leaflet has been developed (see below) providing balanced information about the PSA test, which will hopefully help you to decide whether or not you wish to have the test. PSA Test Information Leaflet Registration New Patient Registration You will need to be within the catchment area of the Practice to register with us - please see below. The receptionist will provide you with a form to fill in and sign which you will then return to the surgery. You will also be given a short questionnaire to fill in to provide the Practice with vital information while awaiting your notes from your previous practice. You will be registered with the Practice and not a specific doctor. However, you may request a doctor of your choice when making an appointment Practice Catchment Area Our Practice area covers all of the West Bridgford, Gamston and Edwalton area with NG2 and NG12 postcodes. In order to provide a high standard of home visit care we are unable to take from outside this area. Also to maintain continuity of care we are unable to take NG11 postcodes as some of these fall under the umbrella of the Nottingham City Primary Care Trust and our practice comes under the Nottinghamshire County Primary Care Trust. Temporary Residents If any friends or relatives are staying with you and need a doctor we will be happy to see them as temporary residents. Confidentiality Confidentiality The practice complies with Data Protection and Access to Medical Records legislation. Identifiable information about you will be shared with others in the following circumstances: To provide further medical treatment for you e.g. from district nurses and hospital services. To help you get other services e.g. from the social work department. This requires your consent. When we have a duty to others e.g. in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care. If you do not wish anonymous information about you to be used in such a way, please let us know. Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff. However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible. However, if you do have a concern, we would encourage you to initially discuss it with one of the partners. GP Registrars are qualified doctors who are gaining experience in General Practice. Medical Students We often have medical students in the practice too. They may be in their first, second or final year of training and are with us to learn about general practice. You may find one accompaning your doctor during consultation, if you object to this please let your doctor know this and the student will be asked to leave. Chaperones Sometimes it is helpful to both the patient and the doctor to have another person in the room during a physical examination. Please understand that you may ask for a chaperone to be present and the doctor will not be offended. The doctor themselves may also request another person to be present. Violence & Abuse The practice supports the Government's NHS zero tolerance to violence campaign. Doctors and their staff have the right to care for others without the fear of being attacked or abused. A patient who has committed an act of violence or abuse against the doctor, the staff or any other person present, either on the practice premises or in another place where treatment is given by the practice, will be reported to the police and removed from the practice list with immediate effect. SystmOnline SystmOnline SystmOnline is a facility available to all registered patients. The system enables patients to: Request repeat prescriptions. To use this service you will need to obtain a user name and password, which will be issued to you by the receptionist on request. You will need to apply in person and provide photographic identification before your user name and password will be issued. Please use this facility responsibly The system keeps a complete audit trail and we reserve the right to remove the access rights of a patient found to be abusing the system. A carer is someone who spends a significant amount of their life providing unpaid support to family or potentially friends. This could be caring for a relative, partner or friend who is ill, frail, disabled or has a mental health illness, substance or alcohol misuse problem. A carer can be any age. They need not necessarily live in the same house as the person they care for. Every situation is different. You may be caring full time or you may regularly help out with tasks such as shopping and cleaning. Caring for another person can be extremely rewarding, but also very demanding. Does your GP know you are a Carer? It is important that you look after your own health too. Make sure you let the practice know if you are a carer, as this will help staff understand the pressure that you may face and they can also help you obtain the support you need. These places can usually only share information from your records by letter, email, fax or phone. At times, this can slow down treatment and sometimes information can be hard to access. Summary Care Records have been introduced to improve the safety and quality of patient care. Because the Summary Care Record is an electronic record it will give healthcare staff faster, easier access to essential information about you, to help provide you with safe treatment when you need care in an emergency or when your GP practice is closed. Summary Care Records contain important information about any medications you are taking, allergies you suffer from and any bad reactions to medicines that you have had. The record will also include your name, address, date of birth and your unique NHS number to help identify you correctly. Giving healthcare staff access to this information can prevent mistakes being made when caring for you in an emergency. You may want to add other details about your care to your summary care record. This will only happen if you ask for the information to be included. Your Choices A summary care record will automatically be created for you. If you do not wish to have one then you will need to fill out an opt-out form and hand it in to the surgery. Travel Vaccinations Going Travelling? As travel to foreign countries is relatively simple these days, it is easy to forget or not realise what health risks we may be subjecting ourselves to. The administering of travel vaccinations and travel health advice is a large part of our Practice Nurses' role here at St. George's. Look up the country/countries to be visited and if you think you may need vaccinating or are at all unsure, then make an appointment to see one of our Practice Nurses, preferably 2 months before departure.
