Extratemporal Anatomy Intracranial portion ตัง้ แต่ motor cortex จนถึง cerebellopontine angle ก่อนเข ้า internal auditory canal - supranuclear pathway - facial nucleus - infranuclear pathway - upper motor neurone lesion - contralateral lower face palsy - lower motor neurone lesion - ipsilateral whole face palsy Intratemporal portion - meatal segment - labyrinthine segment - tympanic or horizontal segment - mastoid or vertical segment Intratemporal portion Meatal segment อยูใ่ น IAC content ใน IAC ประกอบด ้วย facial nerve, cochlear nerve, superior vestibular nerve, inferior vestibular nerve, labyrinthine artery - Bill's bar (crista falciformis) - Transverse crest. General visceral afferent (GVA) sensory fibers (tympanic cavity, tympanic membrane, EAC) nucleus of tractus solitareus 3. Special visceral efferent (SVE) motor fiber muscles of facial expression, platysma m., posterior belly of digastric m, stylohyoid m. 4. At rest : normal symmetry and tone Motion : Forehead, moderate to good function. :Eye, complete closure with minimal effort. : Mouth, slight asymmetry. Grade III moderate dysfunction Gross : Obvious but not disfiguring difference between two sides At rest : normal symmetry and tone Motion : Forehead, slight to moderate movement. : Eye, complete closure with effort. : Mouth, slight weak with maximum effort. Lacrimation test=Schirmer’s test ้ ประสาททีต เสน ่ รวจ greater superficial petrosal nerve หล ักการ เปรียบเทียบการไหลของนา้ ตาทงั้ 2 ข้าง วิธก ี าร กระดาษกรองขนาด 5 * 35 มม วางที่ บริเวณ inferior fornix นาน 5 นาที การแปลผล < 30% ,<1/3ของข้างทีป ่ กติ ถือว่า impair nerve(ปกติอาจไม่เท่าก ันได้ แต่อย่างน้อยต้องมากกว่าก ัน54%) <25 mm sum of both eyes is abnormal ตาแหน่งรอยโรค บริเวณ geniculate ganglion หรือ Topographic test 2. Stapedial reflex test ้ ประสาททีต เสน ่ รวจ nerve to stapedial muscle หล ักการ ี งทีม เป็นการตรวจ reflex โดยทีม ่ ี input คือเสย ่ ี ความด ังมากพอ (>60 dB above threshold) ผ่านทาง CN VIII มี output ทาให้เกิดการหด ต ัวของ stapedial muscle ผ่านทาง CN VII( ด้านตรงข้าม) ใชเ้ ครือ ่ ง acoustic impedance ตรวจการ ทางานของ stapedial muscle วิธก ี าร การแปลผล negative ในรายทีม ่ ี injury ต่อ nerve (ไม่ม ี โรคของหูและการได้ยน ิ อยูใ่ นเกณฑ์ปกติ) ตาแหน่งรอยโรค บริเวณ pyramidal eminence หรือสูงกว่า 2 genu in vertical (mastoid) part of nerve. Topographic test 3.Taste test (electrogustometry) ้ ประสาททีต เสน ่ รวจ Taste sensation ของ chorda tympani nerve หล ักการ ทดสอบการร ับรสทีบ ่ ริเวณ anteria2/3 ของ tongue เปรียบเทียบผลทง2 ั้ ข้าง ้ ารธรรมชาติ คือ รส 1. ทดสอบโดยใชส ้ ว เค็ม หวาน เปรีย (salt,sugar,citrate,quinine) 2. 2. กระแสไฟฟ้ากระตุน ้ (electrogustometry) วิธก ี าร การแปลผล 1. ไม่สามารถร ับรสได้ ้ ระตุน 2. ค่ากระแสไฟฟ้าทีใ่ ชก ้ มากกว่าข้าง ้ ระแสไฟฟ้า ปกติเกิน 25% หรือใชก เกิน 300 mAmp. ตาแหน่งรอยโรค vertical (mastoid) segment ก่อนออก stylomastoid foramen หรือสูงกว่า Topographic test 4.Salivary flow test ้ ประสาททีต เสน ่ รวจ secretorymotor ของ chorda tympani nerve หล ักการ เปรียบเทียบปริมาณนา้ ลายทีห ่ ลง่ ั จาก submandibular glandทงั้ 2 ข้าง วิธก ี าร แยงท่อเข้าไปใน Wharton’s duct ทงั้ 2 ข้าง แล้วว ัดปริมาณนา้ ลายทีอ ่ อกมา (จานวนหยด ี )ี หรือจานวนซซ การแปลผล ปริมาณนา้ ลาย <25% ของข้างปกติ ถือว่า ผิดปกติ(May & Hawkins) ตาแหน่งรอยโรค vertical segment (mastoid portion)ก่อน ออก stylomastoid foramen หรือสูงกว่า Chorda tympani ทีแ ่ ยกจาก main facial trunk Topographic test 5.Salivary PH Normal: PH .>,= 6.4 PH < 6.1= incomplete recovery in case of Bell’s palsy Accuracy prediction = 91% Unknown cause this test Give an earier prognosis than other test Prognostic test or Electrical test -severity of axonal degeneration -Require serial testing -Compared with the normal side ้ ccess motor function of facial n. at -ใชa distal to stylomastoid foramen -กรณีlesionที่ proximal กว่าตาแหน่ง test ต้อง รอ wallerian degeneration ก่อน test ซงึ่ ใช ้ เวลาอย่างน้อย 72 ชว่ ั โมง (Not determine in the immediate post injury stete ) 7 test Electrical test Nerve excitability test (NET) or Hilger's test Maximum stimulation test Electroneuronography (ENOG) Evoked Electromyography (EEMG) Electromyography (EMG Magnetic stimulation Facial Nerve Monitoring Nerve excitability test (NET) or Hilger's test ใช้ DC current กระตุน้ nerve trunk นาน 0.3msec. เปรี ยบเทียบ muscle contraction 2 side ทาหลัง onset 3 day มักทา marginal mandibular branch เพราะตื้นที่สุด Stimulating electrode is place on skin over stylomastiod foramen or over one of peripheral branch of the nerve Frontal branch - 1 inch lateral to outer canthus Zygomatic branch - bony lateral border of orbit Marginal mandibular branch – ant r to the point that facial a. pass mandible Nerve excitability test (NET) or Hilger's test -minimum electrical stimulation -> 6mA in injury side - different > 3.5 m A Severe degeneration NET เมื่อใช้ NET different >3.5 เป็ น criteria ในการทาการผ่าตัด nerve decompression จะมี accuracy 80% มีประโยชน์ เฉพาะ(Useful only) first 2-3 weeks of complete paralysis,before complete degeneration has occure ใน incomplete paralysis จะมี normal response เมื่อ กระตุ้น distal ต่ อ pathologic site จึงไม่ ควรทา NET ใน incomplete paralysis Clinical recovery จะมาก่อน NET recovery จึงไม่ มี ประโยชน์ ในการ F/U NET Maximum stimulation test (MST) กระตุน้ ด้วย maximal DC current ที่ผปู ้ ่ วยทนได้(5mA) จน เห็น maximal movement (full muscle contraction or pain) เริ่ มทาในข้างที่ดีก่อนแล้วทาข้างเสี ย เปรี ยบเทียบเป็ น % กับข้างดี - result - equal 100%, mild 75%, moderate 50%, severe 25%, complete 0% Stimulate all intact axon,ประเมินสัดส่ วนของ fiber ที่ degenerate ฉะนั้นจึงให้information more reliably guide prognosis and treatment than NET - ถ ้า severe (<25%) 75% incomplete recovery Bell’s palsy ถ้าพบ Normal MST 88% = complete recovery Absence of electrically stimulated movement asso. C incomplete recovery Electroneuronography (ENOG) Evoked Electromyography (EEMG) -หลักการเหมือน MST แต่ recod เป็ นกราฟ Electrical summation potential -เป็ น objective test -เป็ น most acceptable for prognostic indicator - เปรียบเทียบ compound action potential (CAP) กับข ้างดี Normal ต่ างกันน้ อยกว่ า 3% อาจมี Test error 20% 0-50% degeneration : good prognostic indicator 50-90% degeneration : fair recovery >90% degeneration : poor recovery (surgery is indicate in traumatic) ปัจจุบันใช้ 95% degenertion ( =5% normal site)เป็ น surgical indication ใน Bell’palsy (เพราะ ใน 2 wk. จะมี 50% change of poor recovery) ทัง้ NET, MST, ENOG จะทดสอบได ้ก็ตอ ่ เมือ ่ มี axon degeneration ก่อน ซงึ่ จะเกิดขึน ้ 48-72 ชม. หลัง injury เพราะฉะนัน ้ ตรวจและแปลผลได ้ เมือ ่ เกิน 72 ชม. ไปแล ้ว Electromyography (EMG) ใช้ evaluate the motor unit ใช้ electrode วัด electrical activity Recoding of spontaneous and voluntary muscle potential by needle introduced into the muscle Electromyography (EMG) Muscle electrical response at rest and contraction Normal - needle insertion - muscle contraction - rest - no muscle activity - contraction - muscle activity (CAP) Degeneration - at rest - fibrillation wave - contraction - no muscle activity ั ดาห์ หลัง injury พบประมาณ 2 สป แปรผล Biphasic or triphasic = normal motor unit Fibrillation = degeneration (อาจ พบได้ ต้งั แต่ 10-14 วัน หลังinjury ซึ่งยืนยันว่ ามี degeneration จริง Polyphasic สาคัญ ใช้ บอก = regeneration = reinnervation (4-6 weeks after the onset of the paralysis) Magnetic stimulation - กระตุ ้น facial nerve ตัง้ แต่บริเวณ geniculate ganglion, internal auditory canal โดยตรง ข ้อดี 2 ข ้อ -ทดสอบได ้เร็วตัง้ แต่เริม ่ มี injury , บ่งตาแหน่งรอยโรคได ้, test up 4 days after on set of Bell’s palsy -without pain and discomfort ึ ษาถึงประโยชน์ทช ั เจน - ยังอยูร่ ะหว่างการศก ี่ ด Facial Nerve Monitoring Intra-operative monitoring Audible EMG (needle or surface electrode, monopolar or bipolar stimulation) At the end of the case the nerve can be stimulated successfully near the brainstem with low currents (0.05-0.1mA), the prognosis for postoperative function is excellent
