JOB DESCRIPTION Clinical Cancer Pathway Tracker Clinical Services Position Title: Clinical Cancer Pathway Tracker Organisation Unit: Clinical Services Location: Whangarei Hospital, Northland District Health Board Responsible to: Service Manager, Ambulatory Services, Clinical Services, Northland District Health Board Primary Functions of the Position: Maintain a database which records all patients in the Northland DHB area who are diagnosed with cancer and receiving treatment targets Functional Relationships The Clinical cancer pathway Tracker will develop and maintain excellent relationships with: Internal The Clinical Services treating patients with cancer Clinical Nurse Specialists Clinical teams Surgical Medical Pathology Oncology Cancer Control Governance Group Decision Support Group analysts Portfolio Manager SD&F External Northern Cancer Network Cancer Trackers for Auckland, Counties Manukau and Waitemata DHBs Laboratories Clinical Cancer Pathway Tracker Clinical Services Page 1 of 5 Key Responsibilities and Expected Outcomes Northland District Health Board has established a set of values by which the organisation will respond, in part, to achieving its goals and objectives through their workforce. Use patient information systems and liaison with services to identify key information such as first suspicion of cancer, decision to treat and date of first treatment, according to MoH rules Record key information in electronic form Prepare monthly reports Ensure all data is collected to report national cancer treatment targets on time With Decision Support Group assistance, generate monthly report, with narrative if necessary Recommend process changes to data collection and entry practices where necessary Identify opportunities to improve data collection or entry, and specify recommendations to Faster Times to Cancer Treatment (FCT) Steering Group Provide information on how cancer treatment time targets data collection can be automated Clinical Cancer Pathway Tracker Clinical Services Page 2 of 5 Key Responsibility Area Initiate improved care coordination processes Expected Outcomes Together with cancer services, design improvements to care coordination processes within each tumour group so to ensure that Northland meets any current and future treatment time targets, and addresses ethnic and other inequalities in cancer outcomes Toward end of year in position, design system(s) to allow ongoing collection of cancer treatment target data by the cancer services. Treaty of Waitangi Health & Safety Contribute to the promotion of the principles of Treaty of Waitangi and the involvement if Maori within the decision making process for their health and independence, within District Health Board management processes and procedures Include the principles of the Treaty of Waitangi within all aspects of the role and its outcomes Ensure that consultation and engagement processes include appropriate mechanisms to meet the need of Maori in a culturally appropriate and safe manner Attend the Northland District Health Board Treaty of Waitangi Training Observe and promote safe work practices, rules and instructions relating to work, and be pro-active in hazard management Willingly co-operate in the achievement of all health and safety goals and initiatives by: Practicing and observing safe work methods; The use of safety equipment; Reporting unsafe conditions or equipment; and Reporting and documenting all accidents or incidents Variation of Duties Duties and responsibilities described above should not be construed as a complete and exhaustive list as it is not the intention to limit in any way the scope or functions of the position.
