the Position:
Maintain a database which records all patients in the Northland
DHB area who are diagnosed with cancer and receiving
treatment targets

The position of Clinical cancer pathway Tracker encompasses the following major functions or

Identify and record key points in cancer pathway
Prepare monthly reports
Recommend process changes to data collection and entry practices where necessary
Initiate improved care coordination processes
Treaty of Waitangi
Health and Safety
The outcome requirements of the above key responsibility areas are outlined below:

 Confirm that diagnosis of cancer for patients identified by
screening process
 Determine the applicable tumour stream for these patients
 Refer on to the clinical lead for the tumour stream, or an
assigned clinical nurse specialist (or other clinician), if the
stream has established roles.
 Use patient information systems and liaison with services to
identify key information such as first suspicion of cancer,
decision to treat and date of first treatment, according to
MoH rules
 Record key information in electronic form
Prepare monthly
reports
 Ensure all data is collected to report national cancer
treatment targets on time
 With Decision Support Group assistance, generate monthly
report, with narrative if necessary
Recommend process
changes to data
collection and entry
practices where
necessary
 Identify opportunities to improve data collection or entry, and
specify recommendations to Faster Times to Cancer
Treatment (FCT) Steering Group
 Provide information on how cancer treatment time targets
data collection can be automated

Initiate improved care
coordination processes
Expected Outcomes
 Together with cancer services, design improvements to care
coordination processes within each tumour group so to
ensure that Northland meets any current and future
treatment time targets, and addresses ethnic and other
inequalities in cancer outcomes
 Toward end of year in position, design system(s) to allow
ongoing collection of cancer treatment target data by the
cancer services.


Contribute to the promotion of the principles of Treaty of
Waitangi and the involvement if Maori within the decision
making process for their health and independence, within
District Health Board management processes and
procedures
Include the principles of the Treaty of Waitangi within all
aspects of the role and its outcomes
Ensure that consultation and engagement processes include
appropriate mechanisms to meet the need of Maori in a
culturally appropriate and safe manner
Attend the Northland District Health Board Treaty of
Waitangi Training
Observe and promote safe work practices, rules and
instructions relating to work, and be pro-active in hazard
management
Willingly co-operate in the achievement of all health and
safety goals and initiatives by:
 Practicing and observing safe work methods;
 The use of safety equipment;
 Reporting unsafe conditions or equipment; and
 Reporting and documenting all accidents or incidents
