Program Descriptions
The following are brief descriptions of all services offered at Portneuf Valley Family Center:
1. Community Based Rehabilitation Services (CBRS): Community Based Rehabilitation
Services is a home and community based program available to adults who are recovering from a Severe
and Persistent Mental Illness, or children with Severe Emotional Disturbance. This program promotes
recovery, assists the member to integrate with his/her community, and provides services aimed at
helping the member improve his/her quality of life. Community based rehabilitation services is
provided by a Bachelor's prepared providers under the supervision of an independently licensed
behavioral health clinician. The Bachelor’s prepared provider must possess a current behavioral health
license or have certification through the Psychiatric Rehabilitation Association. The service aims to help
member’s develop skills needed to increase their capacity to thrive in their home, employment, school or
social environments. Service target skills for adults are those that have been lost due to the symptoms of
the member’s behavioral health condition. Target Population: Children, adolescents, and adults.
Consumers must meet eligibility requirement identified through Optum Idaho.
2. Medication Management: Is a method of treatment that involves diagnostic evaluation and
management of individuals with the use of pharmacological interventions. Target Population:
Medication Management and Psychiatric/Diagnostic Evaluation provided to children, adolescents, and
adults. Dr. Kelly Palmer, Board Certified Psychiatrist and Physician Assistant Renee Katter are the
current providers of all medication management services.
3. Case Management: facilitates access to needed services through comprehensive assessment, service
planning, referral, and monitoring of the member’s service planning activities. Case Management
services are provided to members who are unable to navigate the service system on their own, and do
not have the assistance of other natural supports or resources to help them access needed services. Case
management services do not include advocacy or the direct delivery of services. Case management
services are intended to help the member gain access to behavioral health, medical and social services
until the member is capable of accessing these services independently. Target Population: We
provide mental health Case Management to adults, children and adolescents. Consumers must meet
eligibility requirement identified through Optum Idaho.
4. Crisis Support Services: As part of our treatment services listed here we also offer 24-hour crisis
intervention services to consumers to evaluate and assist in the prevention of out-of-home placement or
hospitalization. Professional staff members who are qualified to provide CBRS, CM or Psychotherapy
services are available after regular office hours and on weekends to answer questions and problem
solve situations within the scope of their training, practice, and licensure. If it is determined that more
intensive interventions are needed, our staff can assist in making the proper referrals to ensure the
safest plan of action including the involvement of law enforcement or medical professionals. These
services are accessed through our on-call phone number that is made available to all consumers or to
the Optum Idaho Behavioral Health Plan Crisis Line.

5. Psychotherapy: This is a method of treating and managing psychiatric disorders, emotional concerns,
and trauma through the use of evidenced-based psychotherapeutic modalities that focuses on behavioral
and cognitive aspects of a participant’s abilities. Target Population: We provide individual and group
counseling to children, adolescents and adults as well as couples and family counseling.
6. Peer Support Services: Peer Support services are recovery support services in which a Certified Peer
Support Specialist utilizes his/her training, lived experience and experiential knowledge to mentor, guide
and coach the member as he/she works to achieve self-identified recovery and resiliency goals. Target
Population: Consumers that are 18 or older. Consumers must meet eligibility requirement identified
through Optum Idaho.
7. Family Support Services: Family Support Services provide assistance to caregivers who are caring for
a child diagnosed with a mental health disorder, or coexisting mental health, developmental and/or
substance use disorder by strengthening their role as parents through the provision of teaching and
support services, and reducing the likelihood that the family and member will become isolated,
disempowered, or disengaged. Services are provided by a Certified Family Support Partner who is a
parent or adult care giver, and through lived experience and specialized training has acquired an
understanding of another parent’s situation via the share emotional and psychological challenges of
raising a child with a mental health diagnosis. Target Population: Consumers that are under the age of
18. Consumers must meet eligibility requirement identified through Optum Idaho.
8. Substance Abuse Treatment: The provision of individual therapy, group therapy, family therapy, case
management, assessment, education, and other services to eliminate or reduce alcohol and substance use
and arrest, reverse or reduce problems associated with alcohol or substance abuse, or both. (IDAPA
16.07.20.012.18). The treatment provided by PVFC is outpatient and will be limited to individual and
group counseling and supportive education including prevention, treatment and supported recovery.
Target Population: Consumers who are age 12 -18 and adults who are identified as needing outpatient
substance abuse treatment.
