Needing Immediate Attention 2 𝑥1 2 Total Score Actual Score 1 1 0.5 1 Justification The problem is a health deficit because the client is experiencing a nutritional deficit, evidenced by the client’s low weight for age and insufficiency when it comes to important vitamins. Malnutrition, in this case, is partially modifiable because of the parent's inability to provide nursing care due to lack of knowledge about the health condition, and lack of necessary equipment and certain supplies for care. The family is also suffering because of financial restraint, and so they can’t provide for their child’s health care needs. Having insufficient knowledge about the condition and a lack of trust in health care professionals will slow down the progression of improvement of the child. The family is not financially stable, which makes it hard for the family to seek help. Thus, the health condition of the child has low preventive potential. They recognized this as a problem that needs immediate attention. 3.5 Problem No.2: Poor Environmental Sanitation-Improper Garbage Disposal as a Health Threat Criteria 5. Partially Modifiable Computation 2 𝑥1 3 1 𝑥2 2 Actual Score 0.6 1 Justification The problem is a health threat because improper garbage disposal presents a danger not just in the surroundings, but also to the family members who live nearby. Unsafe and unclean nature improper garbage disposal may lead to family members acquiring common or worst, unknown diseases especially to those who are immunocompromised. Educational resources for health teachings will be used to help enlighten the family members about the possible dangers of improper garbage disposal. The client and the family have shown positive regard for the problem. The lack of community resources from the barangay for the garbage collection hindered the willingness and cooperation of the family members for this matter. 7. Moderate Preventive Potential 8. Needing Immediate Attention 2 𝑥1 3 2 𝑥1 2 Total Score 0.6 1 The willingness and cooperation from the client and the family to comply with proper garbage disposal is not enough to solve or lessen the problem. Without help and immediate action coming from the barangay the garbage disposal future problems will arise and that can be moderately prevented by the client and the family. They perceived this problem as a condition that needs immediate attention. 3.2 PRIORITIZATION OF PROBLEM: 1. Malnutrition as a Health Deficit 2. Poor Environmental Sanitation-Improper Garbage Disposal as a Health Threat 3.5 3.2 Problem number 1 which is Malnutrition as a Health Deficit, with a score of 3.5, needs to be prioritized because malnutrition needs more attention. After all, it involves one’s life and future. The child needs immediate attention and care. This can lead to a more serious condition if not immediately treated. The second problem which is Poor Environmental Sanitation-Improper Garbage Disposal as a Health Threat, with a score of 3.2, is the next to be given attention because it is a threat to health that does not need any immediate medical attention. FAMILY NURSING CARE PLAN Malnutrition as a Health Deficit Health Conditions/s or Problems and Family Nursing Problems Malnutrition as a Health Deficit Objectives of Nursing care After the nursing interventions, the client and family will be able to: Age: 2 years old Weight: 7 Kg 1. Verbalize understanding of Height: 70 cm the health consequences BMI: 14.3 – underweight of malnutrition The patient is pale and lethargic Inability to provide adequate nursing care to a member suffering from malnutrition due to: a. Lack of knowledge about the health condition b. Lack of knowledge on the nature and extent of nursing care needed c. Inaccessibility of appropriate resources of 2. Realize what are the contributing factors to malnutrition 3. Identify ways on how to plan and prepare a simple balanced meal within their family’s budget to acquire their child’s nutritional needs 4. Avail for food supplements provided by the government health agencies for malnourished children Plan of Interventions 1. Provide information regarding malnutrition Evaluation Plan Outcome Criteria/Indicators, Standards 1. The family verbalizes understanding of the health consequences of malnutrition if left untreated. Rationale: Some family is not aware of malnutrition. Providing information about it can help the family cope with present 2. The family verbalizes circumstances and prepare and plan for willingness to prepare a simple treatment. 2. Discuss the implications of signs & symptoms of malnutrition and its possible consequences if left untreated. Rationale: Children who do not reach their optimum height or consistently experience bouts of weight loss during childhood are affected in the long term in numerous ways. 3. Observe the family’s food selection and preparation, and their eating habits and practices. Rationale: To note what needs to change and what needs to be improved. 4. Provide information and skills on proper food selection and preparation and affordable balanced meal for their child 3. The Family indicates a willingness to avail of food supplements and essential vitamins for their child. They also verbalized willingness to consult a health center for further help needed. 4. After one week, the child's weight increased by 1 pound. - Methods / Tools Home visits Visual aids: Books, infographics, and pictures for discussing the following: a. Consequences of malnutrition if left untreated b. A simple affordable balanced meal c. DOH’s programs care, especially cost constraints 5. Increase their child’s weight by at least one pound per week. feeding practices Rationale: A selection of foods from the five food groups is necessary to support growth, energy needs, physical activity, and health and wellbeing 5. Inform the family about the feeding programs present in their community, as well as Rationale: Government and nongovernment health agencies have programs focusing on the nutrition of a child 6. Discuss the nutritional needs and recommended energy and nutritional intake of a child 2 years of age. Rationale: Good nutrition during the first 2 years of life is vital for healthy growth and development. Starting good nutrition practices early can help children develop healthy dietary patterns.
