Spring Sports Sign-Up Night
Monday, February 22, from 4:00-5:30PM
All Students interested in trying out for a Spring Sport at Mt. Hebron High School are urged to attend our
Spring Sports Sign-up Night on Monday, February 22nd, from 4:00-5:30PM in the MTH Cafeteria. Please
bring with you the following information: (Bold indicates forms can be down loaded at the HCPSS
website or www.mthebron.com):
1. Pre-Participation Physical Examination (dated on/after April 1, 2015)
2. Impact Concussion Tested (Valid for two years). Students interested in trying out for Allied Softball,
Baseball, Boy’s or Girl’s Lacrosse, Softball, or is a Pole Vaulter, must have been concussion tested
within the previous two years and be current throughout the Spring, 2016, season.
Parents/Guardians must complete and Sign the HCPSS Concussion Permission forms which are
to be turned in to the Trainer at the start of the Impact Test. Mt Hebron High School will conduct the
Impact Concussion Test on Tuesday, Thursday, and Monday, February 16th, 18th and 22nd from
3:00PM-5:00PM in the MTH Media Center. Students will be tested on a first come, first serve basis.

4. Photo copy of their Birth Certificate (only if this is their first time participating in a sport at MTH)
5. ALL Candidates MUST meet the HCPSS Academic Eligibility Requirement (No more than ONE
“E” grade and a Minimum 2.0 GPA on their 2nd Quarter report Card in order to try-out;
6. Residency Verification (in the form of a BGE Bill, Water Bill or Land Line phone bill - required
each of the three athletic seasons)
7. Sudden Cardiac Arrest Form
Spring Sports try-outs begin Tuesday, March 1st. Students MUST meet ALL the
Participation requirements above and hand in all necessary paperwork ON OR
BEFORE March 1st, in order to participate in Spring Sports try-outs.
