Medical Home Family Partner
Job Description
Purpose:
The overall purpose of the Family Partner is to provide information and support to the Medical Home and the
families with children and youth with special health care needs within the practice.
Responsibilities:













Become familiar with the history, purpose, programs, and policies of Medical Home.
Assist the Medical Home in evaluating current practice, identifying challenges, recommending solutions
and setting future priorities.
Advise the Medical Home in effective methods of incorporating family centered care.
Network and share information between the families, community resources and the Medical Home.
Make presentations and provide training as needed to families and the Medical Home.
Assist in the development and review of materials for families.
Provide families with information, parent to parent support and linkages with community services,
organizations and agencies.
Represent the concerns of children with special health care needs and their families.
Establish and maintain lines of communication between families and Medical Home team.
Attend scheduled meetings.
Promotes collaboration and partnerships with other families, professional and agencies
Responsible for documentation and reporting that is relevant to activities.
Attends ongoing trainings and meetings with the Medical Home and the Family Partner Coordinator.
Qualifications:










High School Diploma with some college preferred.
A family member of a child who has special health care needs with 6+ years of experience.
Scheduling flexibility and reliable personal transportation required.
Strong verbal and written communication skills.
Ability to speak to groups, including staff, providers and parents.
Ability to work both independently and in collaboration with others.
Knowledge of the systems and resources for children, youth and young adults with special needs.
Ability to work with diverse families and provide culturally sensitive information.
Strong computer and web based skills including consistent email communication.
Demonstrates the ability to maintain confidentiality.
Compensation:

A contracted position – approximately 10-12 hours a month with a stipend of $2,000/year funded by the
CHIPRA Grant. The Family Partner will receive a 1099 tax form at the end of the year. It is up to the
Family Partner to explore the tax implications with their tax professional.

Duration:

Based on the needs of the practice, typically one year with the option to extend an additional year at the
end of each year.
Supervision and Support:

Reports to both the Medical Home team and the Family Partner Coordinator.
